A comparative study of one-step vs. two-step approach in the treatment of pediatric gallbladder stones with common bile duct stones.

OBJECTIVE: This study aims to compare the effects of the one-step and two-step methods of ERCP combined with laparoscopic cholecystectomy in the clinical application of pediatric gallbladder stones with common bile duct stones. METHODS: Clinical data were retrospectively collected from pediatric patients diagnosed with gallbladder stones combined with common bile duct stones at our hospital from January 2023 to January 2026. Patients were divided into two groups based on the surgical approach: the one-step group (underwent ERCP and laparoscopic cholecystectomy under the same anesthesia) and t

ECMO and rigid bronchoscopy-assisted surgical resection of a giant pulmonary mesenchymal tumor in a child: a case report.

BACKGROUND: Giant pulmonary mesenchymal tumors in children pose significant challenges in pediatric anesthesia practice. During resection, these tumors can lead to complications such as cardiovascular collapse during anesthesia induction and injury to adjacent organs or blood vessels that may be compressed or displaced. CASE PRESENTATION: A 3-year-old girl presented to the pediatric clinic with cough, wheezing, and orthopnea (inability to lie flat due to dyspnea). After a diagnosis of giant pulmonary mesenchymal tumor, she underwent complete resection under extracorporeal membrane oxygenation…

Ionizing radiation from computed tomography versus anesthesia for magnetic resonance imaging in infants and children: patient safety considerations.

In the context of health care, risk assessment is the identification, evaluation and estimation of risk related to a particular clinical situation or intervention compared to accepted medical practice standards. The goal of risk assessment is to determine an acceptable level of risk for a given clinical treatment or intervention in association with the provided clinical circumstances for a patient or group of patients. In spite of the inherent challenges related to risk assessment in pediatric cross-sectional imaging, the potential risks of ionizing radiation and sedation/anesthesia in the ped

Anesthetic management of the pediatric patient with multiple congenital anomalies including severe hemifacial hypertrophy.

An anesthesia induction technique not widely utilized is described for a pediatric patient presenting with severe hemifacial hypertrophy, severe mental retardation, as well as a seizure disorder and other congenital anomalies. The anatomically and/or pathologically altered airway as well as other anomalies increase the risks of anesthesia.

Dual Tunneled Epidural Wound Catheters for Postoperative Analgesia Following Posterior Spinal Fusion.

Pain management following posterior spinal fusion (PSF) in pediatric patients can present significant challenges for clinicians. Opioids continue as the primary modality for managing postoperative pain in these patients, despite well-known concerns regarding their adverse effect profile such as the risk of dependence or abuse. Therefore, there has been increased focus on multimodal analgesic approaches that incorporate non-opioid medications, non-pharmacologic techniques, and regional anesthesia. Commonly used non-opioid adjuncts include non-steroidal anti-inflammatory drugs, acetaminophen, ga

Effects of propofol combined with lidocaine on hemodynamics, serum adrenocorticotropic hormone, interleukin-6, and cortisol in children.

BACKGROUND: Children are a unique patient population. Anesthesia for pediatric abdominal surgery has long been achieved mainly with intravenous amiodarone and propofol alone or combined with other anesthetics. The incidence of complications and postoperative adverse reactions is relatively high owing to the imperfect development of various protocols for children. Choosing the most appropriate anesthesia program is an important means of reducing adverse reactions. AIM: To explore the clinical value of propofol combined with lidocaine-assisted anesthesia in pediatric surgery. METHODS: A total

Possibility of Avoiding Anesthesia in the Reduction of Greenstick and Angulated Forearm and Distal-End Radius Fractures in Children: A Comparative Study.

Introduction Greenstick and angulated forearm bone fractures are the most common fractures in children and invariably require closed reduction under anesthesia. However, pediatric anesthesia is somewhat risky and not always available in developing countries like India. Therefore, this study aimed to evaluate the standard (quality) of closed reduction without anesthesia in children and to determine satisfaction among parents. Materials and methods The present study included 163 children with closed angulated fractures of the distal radius and fracture shafts of both forearm bones, who were trea…

Anesthetic considerations for magnetic resonance imaging-guided right-heart catheterization in pediatric patients: A single institution experience.

Cardiac catheterization is an integral part of medical management for pediatric patients with congenital heart disease. Owing to age and lack of cooperation in children who need this procedure, general anesthesia is typically required. These patients have increased anesthesia risk secondary to cardiac pathology. Furthermore, multiple catheterization procedures result in exposure to harmful ionizing radiation. Magnetic resonance imaging-guided right-heart catheterization offers decreased radiation exposure and diagnostic imaging benefits over traditional fluoroscopy but potentially increases an

[Anesthetic management for cesarean section in a patient with hydrops foetalis].

In cases of hydrops foetalis, respiratory management is frequently required after delivery. We experienced 7 anesthetic managements for cesarean section in patients with hydrops foetalis. Spinal anesthesia was performed in 3 patients, because the transition of local anesthesia through the placenta was little. On the other hand, general anesthesia was performed in 4 patients because of babies requiring endotracheal intubation or fetal anesthesia immediately after a delivery. Before surgery, we discussed perioperative and anesthetic management among pediatricians, pediatric surgeons, and obstetr

Is ultrasound guidance mandatory when performing paediatric regional anaesthesia?

PURPOSE OF REVIEW: Since Kapral in 1994 first described the use of real-time ultrasound-guided regional anaesthesia, this novel technique has gained widespread recognition in adult practice and has been shown to be associated with clinically relevant advantages. The aim of this manuscript is to review the currently published paediatric data associated with the use of ultrasound-guided regional anaesthesia. RECENT FINDINGS: Compared with alternative techniques ultrasound guidance is associated with an increased success rate, reduced onset time, moderately prolonged duration, reduced need for l

Use of a Brain Monitor in a Pediatric Scoliosis Spinal Fusion with Instrumentation to Prevent a Full Wake-Up Test: A Case Report.

Intraoperative use of brain monitoring during anesthesia has increased in recent years due to the development of more advanced monitors. However, it is still not a standard practice, even in spine fusion with instrumentation surgeries. This case report describes the use of a brain monitor with real-time changes in spectral edge frequency (SEF) to assess anesthetic levels and to guide the neurophysiologist to perform timely motor evoked potential (MEP) testing during a wake-up test process in a pediatric patient when MEPs were lost. We should consider using SEF monitoring more regularly when pr

Prevention of emergence agitation in seven children receiving low-dose ketamine and propofol total intravenous anesthesia.

Emergence agitation (EA) can be a distressing side effect of pediatric anesthesia. We retrospectively reviewed the records of 7 pediatric oncology patients who received low-dose ketamine in conjunction with propofol for total intravenous anesthesia (TIVA) repeatedly for radiation therapy. EA signs were observed in all 7 patients in association with propofol TIVA but did not recur in any of 123 subsequent anesthetics sessions during which low-dose ketamine was added to propofol. Based on this experience, we suggest that low-dose ketamine added to propofol may be associated with prevention of EA…
