AIM: Internal resorption is a rare condition that may complicate endodontic treatment and tooth retention strategies. There is a lack of information in the literature on the nature of these lesions, as well as prognosis after treatment. The aims of this study were to determine which factors may be associated with treatment outcomes, and to identify and characterise clinical and radiographic features of internal resorption lesions.
METHODOLOGY: A retrospective chart review was conducted to identify cases of internal resorption at the Tufts University School of Dental Medicine Postgraduate Endo

Internal root resorption (IRR) is an uncommon but clinically significant condition traditionally managed with complete pulpectomy and root canal therapy. Advances in vital pulp therapy and bioactive calcium silicate-based cements have enabled conservative strategies aimed at preserving residual pulp vitality. A 33-year-old woman presented with an asymptomatic, unperforated IRR in the maxillary left lateral incisor, confirmed by periapical radiography and cone-beam computed tomography. The tooth responded positively to sensibility testing and showed healthy periapical tissues. A partial pulpect…

BACKGROUND: Dental pain due to pulpal involvement is difficult to diagnose due to the apparent inaccessibility of pulp to the clinical tests, indistinct symptoms, and referred toothache originating from the periodontal tissues. Though we have various clinical classification systems to categorize pulpal diseases, we are yet biased about the exact pathophysiology and pain pathway associated with it. Dental pulp has a complex physiology, and so is its pathophysiology.
AIMS & OBJECTIVES: To concisely reviews the basic understanding of the pathophysiology of pulp, pain pathway, and its correlation

BACKGROUND: ADAMTS expression can be associated with several inflammatory processes, and has been correlated with tumorigenesis of some neoplasms, but its participation in the development of periapical lesions has not been investigated. Therefore, our objective was to verify the expression of ADAMTS-1, versican and pEGFR in Periapical Granuloma (PG) and in the Radicular Cyst (RC) since they are the most common lesions of the periapex.
METHODS: 25 samples of RC and 10 of PG were used. As a control, 10 samples of inflammatory fibrous hyperplasia (IFH) and 10 of dental follicle (DF) were used. T

Apical periodontitis (AP) is an inflammation affecting the periapical region of tooth root. Microbial pathogens activate inflammasomes and promote the production of pro-inflammatory cytokines. Caspase-1-mediated pyroptosis is a possible mechanism involved in the initiation and progression of AP. The purpose of this study was to evaluate the role of caspase-1 and pyroptosis on AP at different stages. Human periapical inflammatory tissue was collected to study chronic AP stage. Human periodontal ligament fibroblasts (hPDLFs) were stimulated with lipopolysaccharide in vitro for 24h to simulate ea

BACKGROUND: Neurofibromatosis type 1 (NF1) is the most common form of neurofibromatosis. While typically considered a dermatologic disorder, intraoral signs of neurofibromatosis occur quite commonly. This clinical entity can be confused with periodontitis because of the presence of periodontal pockets. In this report, we present the case of a palatal neurofibroma with radiographic involvement in a patient with NF1.
METHODS: A 40-year-old female patient was referred from her general dentist to evaluate advanced periodontitis in the maxillary left quadrant. The patient's medical history was sig

BACKGROUND: Early implantation may preserve the alveolar anatomy, and the placement of a fixture in a fresh extraction socket helps to maintain the bony crest. Although a number of clinical studies exist, no histological reports show the outcome of implantation in fresh extraction sockets without the use of membranes in humans compared to implants placed in mature bone.
METHODS: Forty-eight healthy patients, receiving at least 4 fixtures in each of 2 symmetrical quadrants, underwent placement of 1 experimental fixture placed in a fresh extraction socket (TI) and 1 contralateral fixture in mat

Mandibular buccal bifurcation cyst (MBBC) is a rare inflammatory odontogenic cyst that frequently mimics chronic periapical pathosis, leading to potential misdiagnosis and unnecessary endodontic treatment. This case report describes the diagnosis and management of an MBBC associated with a vital mandibular left second molar (#18) in a healthy 16-year-old female, highlighting the value of pulp vitality testing, cone beam computed tomography (CBCT), and a custom 3D-printed surgical guide for conservative management. The patient presented with buccal bony expansion and a 12-mm periodontal probing

INTRODUCTION: Pulp necrosis in immature teeth and the resulting periodontal apical inflammation negatively affect root formation. Resolvin E1 (RvE1) is a lipid-derived endogenous pro-resolution molecule that controls inflammation. The aim of this investigation was to evaluate the impact of RvE1 applied as an intracanal medication on root formation in nonvital immature teeth.
METHODS: To arrest root development, pulpectomy was performed in the lower first molars of 4-week-old Wistar rats. After 3 weeks, irrigation with 2.5% sodium hypochlorite and 0.9% sterile saline was performed, and either

Infection of the root canal system following dental trauma induces pulp and periapical disease and prevents healing of previously healthy pulp. A clinical goal in treating trauma is the maintenance of pulp vitality, and clinicians should be aware of factors that influence pulp healing. The learning objective of this article is to review the factors and techniques that influence pulp vitality and examine the influence pulp has on the healing of adjacent tissues. The potential routes for bacterial infection of the root canal system are discussed, with the clinical crown as the primary portal of

Transplantation of a nonfunctional mandibular third molar into the opposite mandibular second molar site, where it was necessary to remove the second molar of a 61-year-old patient, has been reported. There was long-term follow-up and evaluation, with radiographs and clinical examination during the ninth postoperative year. The patient had no complaints related to the transplant at the long-term follow-up visit. The periodontal tissues were in excellent condition. The tooth was firm, and the radiograph demonstrated growth of new attachment by showing lamina dura and periodontal ligament space.

The terms inflammation and infection are often used interchangeably. Technically this is incorrect. Inflammation is defined as the body's response to a potentially damaging stimulus. Infection involves tissue invasion by microorganisms. An inflammatory reaction will be provoked when infection occurs in an otherwise healthy patient. Inflammation can be recognized clinically by 5 cardinal signs: rubor (redness), calor (warmth), tumor (swelling), dolor (pain), and functio laesa (loss of function). These clinical signs can be explained by the biochemical and cellular processes activated during the

Replacing both missing maxillary anterior teeth is particularly challenging, especially in compromised sockets. This case report describes the management of an 18-year-old female patient, who suffered avulsion of both maxillary central incisors at 7 years of age. This multidisciplinary implant technique, called Immediate Dentoalveolar Restoration (IDR), included extraction of the injured teeth and a single procedure for immediate implant placement and restoration of the compromised sockets after root fracture and periapical lesion development were detected during orthodontic treatment. Success

Biopsy reports from 150 periradicular tissue specimens obtained from teeth refractory to nonsurgical endodontic therapy were reviewed. The specimens were submitted by postdoctoral dental students in the Department of Endodontics, and the biopsy reports were prepared by oral pathologists at the University of Texas Health Science Center at San Antonio. The study found that 59.3% of the periradicular lesions were granulomas, 22% cysts, 12% scars, and 6.7% other pathoses. The majority (56%) of endodontically treated cases which failed to heal were recognized within 2 yr after the completion of the

The prognosis of injured teeth involving horizontal mid-root fractures is poor and depends heavily on pulpal vitality, periodontal health and the repositioning of the tooth to its original position. Here, we report the case of an upper right central incisor presenting a horizontal mid-root fracture, complicated by impaction of the coronal fragment in the external alveolar bone, which in turn prevented the repositioning of the tooth fragment. As a consequence, it was necessary to raise a full-thickness flap and perform window-osteotomy to free the coronal fragment and reset the fractured fragme

Enterococcus faecalis is a micro-organism that can survive extreme challenges. Its pathogenicity ranges from life-threatening diseases in compromised individuals to less severe conditions, such as infection of obturated root canals with chronic apical periodontitis. In the latter situation, the infecting organisms are partly shielded from the defense mechanisms of the body. In this article, we review the virulence factors of E. faecalis that may be related to endodontic infection and the periradicular inflammatory response. The most-cited virulence factors are aggregation substance, surface ad
