Reduced fetal cerebral blood flow predicts perinatal mortality in a mouse model of prenatal alcohol and cannabinoid exposure.

BACKGROUND: Children exposed prenatally to alcohol or cannabinoids individually can exhibit growth deficits and increased risk for adverse b … bis-exposed human fetuses. Post-exposure cerebroplacental ratios significantly predicted subsequent perinatal mortality (p = 0.019, area under the curve, 0.772; sensitivity, 81%; specificity, 85.70%) and retroactively diagnosed prior drug exposure (p = 0.005; AUC, 0.861; sensitivity, 86.40%; specificity, 66.7%). CONCLUSIONS: Fetal cerebrovasculature is significantly …

Determinants of perinatal mortality in public secondary health facilities, Abuja Municipal Area Council, Federal Capital Territory, Abuja, Nigeria.

INTRODUCTION: in Nigeria, perinatal mortality rate remains high among births at the health facility. Births occur majorly at the secondary healthcare level in Abuja Municipal Area Council (AMAC) of the Federal Capital Territory (FCT). Identifying factors influencing perinatal deaths in this setting would inform interventions on perinatal deaths reduction. We assessed perinatal mortality and its determinants in public secondary health facilities in AMAC. METHODS: delivery and neonatal data from two selected public secondary health facilities between 2013 and 2016 were reviewed and we extracted…

Relationship of isolated single umbilical artery to fetal growth, aneuploidy and perinatal mortality: systematic review and meta-analysis.

OBJECTIVE: To review the available literature on outcome of pregnancy when an isolated single umbilical artery (iSUA) is diagnosed at the time of the mid-trimester anomaly scan. METHODS: We searched MEDLINE (1948-2012), EMBASE (1980-2012) and the Cochrane Library (until 2012) for relevant citations reporting on outcome of pregnancy with iSUA seen on ultrasound. Data were extracted by two reviewers. Where appropriate, we pooled odds ratios (ORs) for the dichotomous outcome measures: small for gestational age (SGA), perinatal mortality and aneuploidy. For birth weight we determined the mean dif…

Foetal and neonatal complications in gestational diabetes: perinatal mortality, congenital malformations, macrosomia, shoulder dystocia, birth injuries, neonatal complications.

OBJECTIVE: To evaluate the risks of perinatal complications in infants born to mothers with treated or untreated gestational diabetes mellit … ted based on the available data, severe clinical forms are rare. The risks of neonatal asphyxia and perinatal mortality are no higher in infants born to women with GDM (EL2). Birth injuries and brachial plexus injuries are rare, and no more likely to occur in cases of untreated GDM. It is difficult to assess the risk of respiratory distress, regardless of its cause. …

[Fetal and neonatal complications of gestational diabetes: perinatal mortality, congenital malformations, macrosomia, shoulder dystocia, birth injuries, neonatal outcomes].

OBJECTIVE: To evaluate perinatal outcomes in case of gestational diabetes (GD) treated or not. METHODS: This study is based on Pubmed search and on NICE and HAS recommendations. RESULTS: Moderate or severe GD increases the risk of foetal and neonatal complications [EL1]. The risk of malformation is slightly increased in case of GD compared with non diabetic population [EL2]. The increased risk of malformation is probably linked to undiagnosed cases of type 2 diabetes among cases of GD [EL2]. There is a continuous association of maternal glucose levels with increased birth weight [EL2]. The i

[Can clinical risk factors for perinatal mortality in West Africa be detected during antenatal care or during labor?].

INTRODUCTION: Perinatal mortality (PNM) is a public health problem in West Africa. Aims. To evaluate the rate of PNM in 6 countries of West Africa, to identify clinical risk factors for PNM, to quantify the relative contribution of antenatal care and labor survey in the detection of these risk factors. METHODS: The MOMA study was a prospective population-based study that collected data about 20326 pregnant women in various, primarily urban, areas of Burkina-Faso, Ivory-Coast, Mali, Mauritania, Niger and Senegal. The present report analyses 19.870 singleton births and 31 simple clinical variab…

Mobile phone survey estimates of perinatal mortality in Malawi: A comparison of data from truncated and full pregnancy histories.

OBJECTIVES: In many low- and middle-income countries, perinatal mortality estimates are derived retrospectively from periodically conducted household surveys. Mobile phone surveys offer advantages in terms of cost and ease of implementation. However, their suitability for monitoring perinatal mortality has not been established. METHODS: We use data from the Malawi Rapid Mortality Mobile Phone Survey (RaMMPS) to estimate perinatal mortality rates from two versions of the survey instrument: a full pregnancy history and a shorter truncated pregnancy history. Female respondents of reproductive ag…

Association of Prenatal Care Services, Maternal Morbidity, and Perinatal Mortality With the Advanced Maternal Age Cutoff of 35 Years.

IMPORTANCE: Maternal and perinatal mortality remain high in the US despite growing rates of prenatal services and spending, and little rigorous evidence exists regarding the impact of prenatal care intensity on pregnancy outcomes. Patients with an expected date of delivery just after their 35th birthday may receive more intensive care owing to the advanced maternal age (AMA) designation; whether this increase in prenatal care is associated with improvements in outcomes has not been explored. OBJECTIVE: To determine the association between the AMA designation and prenatal care services, severe…

Myocardial dysfunction as a predictor of the severity and mortality of hypoxic ischaemic encephalopathy in severe perinatal asphyxia: a case-control study.

Background: In perinatal asphyxia, hypoxia often leads to myocardial ischaemia. Few studies have assessed the degree of myocardial dysfunction in severely asphyxiated term neonates. Aim: To assess the extent of myocardial damage in newborns with severe perinatal asphyxia. Methods: A case-control study was conducted in asphyxiated newborns with hypoxic ischaemic encephalopathy (HIE) and in controls who were term non-asphyxiated newborns. Total (T) creatinine kinase (CK), CK-MB, troponin-T and 12-lead electrocardiography (ECG) and echocardiography were performed in both groups within 24-48 h aft
