Descending necrotizing mediastinitis: a retrospective surgical experience.

OBJECTIVE: Descending necrotizing mediastinitis (DNM) is a primary complication of cervical or odontogenical infections that can spread to the mediastinum through the anatomic cervical spaces. We reviewed the last 10 years of our surgical experience in DNM and commented on early diagnosis and aggressive surgical treatment in these patients. METHODS: Five males (71%) and two females (29%), mean age 34 years, with DNM, were surgically treated. Primary oropharyngeal infection occurred in three (43%) and odontogenic abscess in four (57%) patients. All had serious cervical and mediastinal infectio

Adjunctive antimicrobial photodynamic therapy for treating periodontal and peri-implant diseases.

BACKGROUND: Periodontitis and peri-implant diseases are chronic inflammatory conditions occurring in the mouth. Left untreated, periodontitis progressively destroys the tooth-supporting apparatus. Peri-implant diseases occur in tissues around dental implants and are characterised by inflammation in the peri-implant mucosa and subsequent progressive loss of supporting bone. Treatment aims to clean the pockets around teeth or dental implants and prevent damage to surrounding soft tissue and bone, including improvement of oral hygiene, risk factor control (e.g. encouraging cessation of smoking) a

Balancing the Oral Redox State: Endogenous and Exogenous Sources of Reactive Oxygen Species and the Antioxidant Role of Lamiaceae and Asteraceae.

Background/Objectives: Oral health is a complex concept involving physical, psychological, emotional, and social components. A key factor in maintaining oral tissue integrity is redox balance, which is disrupted by oxidative stress (OS) through an imbalance between reactive oxygen species (ROS) and antioxidant defenses. This study examines the contribution of endogenous and exogenous sources to OS and explores the therapeutic potential of medicinal plants from the Asteraceae and Lamiaceae families in restoring redox homeostasis and improving oral health. Methods: A literature review was conduc

Methanobrevibacter oralis: a comprehensive review.

Methanobrevibacter oralis (M. oralis) has predominated human oral microbiota methanogenic archaea as far back as the Palaeolithic era in Neanderthal populations and gained dominance from the 18th century onwards. M. oralis was initially isolated from dental plaque samples collected from two apparently healthy individuals allowing its first characterization. The culture of M. oralis is fastidious and has been the subject of several studies to improve its laboratory growth. Various PCR methods are used to identify M. oralis, targeting either the 16S rRNA gene or the mcrA gene. However, only one

The Role of Varicella Zoster Virus in the Development of Periapical Pathoses and Root Resorption: A Systematic Review.

INTRODUCTION: Varicella zoster virus (VZV) and subsequent herpes zoster (HZ) infection have been proposed as a causative agent of periapical pathoses and root resorption. This review aimed to identify, synthesize, and present a critical analysis of the available data on the association among VZV, subsequent HZ infection, and the development of periapical pathoses and root resorption and to analyze the level of evidence of available studies. METHODS: The literature search covered MEDLINE, Science Citation Index Expanded, and Scopus. A qualitative critical appraisal of the included articles was

An unusual double supernumerary maxillary mesiodens in a Middle Iron Age skeleton from South Uist, Western Isles, Scotland.

OBJECTIVE: Bilaterally occurring and fully erupted mesiodens occur infrequently in the modern population and have been little documented from past populations. This research presents an unusual case of a double maxillary mesiodens in the permanent dentition occurring in skeletal remains dating to the Middle Iron Age (250-410 AD) from South Uist, Western Isles of Scotland. METHODS: Osteological analysis of an adult male included the inventory of teeth present/absent and analysis of dental pathologies (caries, calculus, periodontal disease, ante-mortem tooth loss, periapical abscesses), develop

Osteonecrosis of the jaw in a patient with rheumatoid artritis treated with an oral aminobisphosphonate: a clinical case report.

Osteonecrosis of the jaw (ONJ) has been recently described after intravenous administration of amino-bisphosphonates and - less frequently - in association with the use of oral bisphosphonates. Bisphosphonate-Related Osteonecrosis of the Jaw (BRONJ) may affect mandible bone (65%), maxilla bone (26%) and rarely (9%) both sites simultaneously. Although causality may never be proven, emerging experimental data have established a strong association between monthly intravenous bisphosphonate administration and ONJ. Current level of evidence does not fully support a cause and effect relationship bet

Oral inflammatory process and general health. Part 2: How does the periapical inflammatory process compromise general health?

At present, the focal infection theory still has very controversial aspects. In spite of the great number of studies, there is no evidence that focal infections or even antigenic mimicry are responsible for anything other than sporadic abscesses/infections and possibly rare autoimmune disorders. linflammation of endodontic origin (i.e., apical periodontitis--AP) has not received the same attention as inflammation originating from the periodontium. Endodontics is a microbiological problem, since the bacterial infection is the "prime mover" of pulp (before) and periapical (later) disease. The ai

Eggerthia catenaformis bacteremia in a patient with an odontogenic abscess.

Eggerthia catenaformis is a Gram-positive anaerobic rod, which has been rarely reported in human diseases. We report the second case of bacteremia due to this microorganism in an elderly patient. A 73-year-old man, without underlying diseases presented with fever, odynophagia and swelling of the cervical lymph node for several days. Culture of drained cervical fluid resulted in the isolation of Raoultella ornithinolytica and Streptococcus anginosus. Anaerobic blood cultures yielded a rare anaerobic microorganism, identified as Eggerthia catenaformis. No resistance to tested antimicrobials was

An audit of antimicrobial prescribing in an acute dental care department.

INTRODUCTION: Antimicrobial resistance is a growing problem that is likely to have a major negative impact on healthcare in the future. Dentists have a key role in ensuring that antimicrobials are prescribed correctly to reduce the emergence of resistant strains. OBJECTIVE: To audit how appropriately antimicrobials were prescribed in the oral surgery acute dental department of Guy's Hospital in London, when compared to the standards set within the Faculty of General Dental Practice (UK) and Scottish Dental Clinical Effectiveness guidelines on antimicrobial prescribing in dentistry. TARGET: 1

Comparative and phylogenomic analysis of Eikenella genus suggest taxonomic reclassification of Eikenella corrodens strain KCOM 3110 as novel species and reveals unique CE1 carbohydrate-active enzyme family.

The Eikenella genus includes opportunistic pathogens of clinical importance, commonly associated with infections such as endocarditis, periodontitis, wound abscesses, and respiratory tract infections; however, its taxonomy, genetic diversity, and evolutionary relationships remain poorly understood. This study aims to clarify the genomic diversity and taxonomic boundaries within the genus through comprehensive comparative and phylogenomic analyses. We conducted a genomic comparison of 63 Eikenella genomes, including 35 high-quality drafts and 5 complete genomes, to understand their phylogenetic

Implant-derived titanium particles impair macrophage bacterial clearance via TRPC1 and lysosomal dysfunction.

Peri-implantitis (PI) is a treatment-resistant inflammatory condition that affects millions of patients with dental implants, leading to destructive loss of jawbone, implant failure, and escalating healthcare costs. It is classically attributed to bacterial biofilms; however, unlike periodontitis, it frequently progresses despite resolution of infection with standard antimicrobial therapy, suggesting a critical gap in our understanding of host-response dysfunction. Here, we identify implant-derived titanium particles (i-TiPs) as a persistent, abiotic pathogenic co-factor that impairs macrophag

Impact of saliva on the efficacy of antiseptics against selected periopathogens.

OBJECTIVE: Oral anaerobic bacteria are key contributors to infectious diseases, including endodontic-, periodontal-, peri-implant infections, and oral abscesses. The management of these conditions frequently involves the use of antiseptics. However, the influence of saliva on the antimicrobial efficacy of antiseptics remains insufficiently elucidated. This study assessed the activity of boric acid (BA), chlorhexidine (CHX), ethacridine lactate (ET), hydrogen peroxide (H2O2), octenidine (OCT), polyhexanide (PHMB), potassium permanganate (KMnO4), and povidone-iodine (PVI), against three represen

Management of Traumatized Teeth With Severely Calcified Canals and Minimally Invasive Access Cavity Using the AReneto® System: A Case Report.

Traumatic injuries to anterior teeth often lead to complications such as calcification and periapical lesions, which frequently necessitate root canal treatment. Traditional methods of preparing access cavities in these cases can compromise the tooth's structural integrity, impacting long-term outcomes. With the advancements in minimally invasive techniques, guided endodontics has emerged as a critical innovation in modern dentistry. This article introduces the AReneto® system, developed by Roots to Cusps® Private Limited, which integrates augmented reality (AR) into endodontic procedures, ena

Root canal treatment of a six-canal first mandibular molar with extensive periapical lesion: A case report.

RATIONALE: There is an increasing tendency for case reports to reveal anatomical aberrances in mandibular first molars, such as the lateral and accessory canals. Thus, clinicians should pay special attention to anatomic variances when dealing with mandibular first molars requiring endodontic treatment to prevent reinfection within the root canal system, which is responsible for the failure of root canal treatment. PATIENT CONCERNS: This article presents the clinical report and successful root canal treatment of a 24-year-old healthy female patient with an extensive periapical lesion in a 6-ca
