Drug use disorders (DUDs) frequently co-occur with alcohol use disorders, affecting approximately 1.1 percent of the U.S. population. Compared with alcohol use disorders or DUDs alone, co-occurring disorders are associated with a greater severity of substance dependence; co-occurring psychiatric disorders also are common in this patient population. Many effective medications and behavioral treatments are available to treat alcohol dependence and drug dependence when these occur independent of one another. There is a paucity of research, however, specifically focused on the treatment of persons

INTRODUCTION: Planning and problem solving, subdomains of executive functioning, are important for daily functioning and clinical outcomes. Although executive functioning encompasses several domains, whether planning-specific deficits are a transdiagnostic feature across psychiatric disorders remains unclear.
METHODS: Planning and problem solving in 658 young adults aged 18-29 was assessed in a cross-sectional study using the One Touch Stocking of Cambridge (OTS) task, with the number of problems solved correctly on first choice and mean latency to correct response as outcomes. Participants h

To approach the questions as to why migraine appears to be associated with functional somatic syndromes (FSSs) and whether central sensitization may be an underlying mechanism, the present study investigated (a) comorbidity with diagnoses of three FSSs, three psychiatric disorders, and three inflammatory diseases, (b) degree of mental health problems (stress, burnout, anxiety, depression and somatization), and (c) prevalence of ten cognitive/affective and nine airway symptoms in persons with migraine in the general population. From a randomized and stratified Swedish adult sample, questionnair

Pathological gambling is known to be associated with a higher than average prevalence of psychological co-morbidities including a range of psychiatric conditions. A problem with much of this literature, however, is that such problems are also frequently found in populations affected by alcohol use disorder, which is common in populations of pathological gamblers. Accordingly, the principal aim of this study was to profile the comorbidities present in a sample of pathological gamblers, comparing those who did, and did not, additionally meet criteria for alcohol use disorder. A sample of 140 com

The relationship existing between personality disorders (classified according to DSM III) and alcohol treatment outcome was evaluated in 404 alcoholics admitted to the Psychiatric Clinic of the University of Trieste, Italy. One quarter of the subjects had personality disorders, mainly of the 'antisocial', 'dependent', and 'borderline' subtypes. Alcohol abuse was specifically related to 'antisocial' personalities. Marked differences in treatment outcome among personality disorders were found. Group therapy for alcoholism was not beneficial to 'antisocial' personalities; it was most useful among

PURPOSE OF THE REVIEW: This narrative review examines the newest findings from clinical trials of chronotherapeutics for psychiatric disorders. We reviewed the potential effects of the most commonly-studied chronotherapeutics such as bright light therapy, dark therapy, melatonin, and chronotherapy on the psychiatric disorders of depression, bipolar disorder, and anxiety disorders.
RECENT FINDINGS: The preponderance of recent clinical trials in chronotherapeutics has focused on bright light therapy in depression. However, there is an emerging body of preliminary studies testing chronotherapeut

This study investigates the presence of personality disorders, impulsiveness, and novelty seeking in probands with DSM-IV pathological gambling (PG), controls, and their respective first-degree relatives using a blind family study methodology. Ninety-three probands with DSM-IV PG, 91 controls, and their 395 first-degree relatives were evaluated for the presence of personality disorder with the Structured Interview for DSM-IV Personality. Impulsiveness was assessed with the Barratt Impulsiveness Scale (BIS). Novelty seeking was evaluated using questions from Cloninger's Temperament and Characte

Records of all inmates committed to a state forensic hospital in fiscal year 1982 (N = 472) were studied to examine the inmates' hospital utilization between September 1977 and April 1984 and the reasons for their admissions. Medication refusal, hallucinations and delusions, and threatened or potential violent behavior toward others were the most frequently documented reasons for admission. Medication refusal was associated with a greater number of hospitalizations, shorter hospitalizations, diagnoses of paranoid schizophrenia or schizophreniform disorders, longer prison sentences, and convict

Emotion dysregulation (ED) has primarily been described in patients suffering from borderline personality disorder (BPD) and is an integral part of this diagnosis, but it is also a transdiagnostic construct that can be found in several other psychiatric disorders. The strong relationships between ED and BPD may lead clinicians to underestimate ED associated to other clinical contexts. This can lead to difficulties in diagnostic and treatment orientation, especially in the context of comorbidities. In this article, after reviewing the literature on the development and functioning of emotion dys

Drug abusers with psychiatric comorbidity are at high risk for becoming exposed to HIV. To address this compelling public health issue, our randomized HIV prevention study compares the effectiveness of the NIDA standard HIV testing and counseling protocol to a four session, peer-delivered, educational intervention for out-of-treatment cocaine users with and without antisocial personality disorder (ASPD) and major depression. Among the 966 out-of-treatment cocaine users who have completed the 3 month follow-up, all groups, regardless of assignment to standard vs. peer-delivered intervention or

Avoidant personality disorder was a new addition to DSM-III. Reaction to its inclusion was mixed. Critics cited the lack of empirical data and the overlap with schizoid disorder. The authors consider the overlap and covariation among avoidant, schizoid, and dependent symptoms and diagnoses in a sample of 84 inpatients diagnosed by using a semistructured interview. Items for avoidant disorder covaried with criteria for dependent disorder but not with criteria for schizoid disorder. The authors point out the implications of these results for the revision of DSM-III (DSM-III-R).

This case study describes a 14-year-old male suffering from significant academic and personal difficulties, who has been diagnosed with depression, schizotypal personality disorder, and learning disabilities. Because of excessive sleepiness, assessment for a potential sleep disorder was performed. An overnight polysomnographic study revealed no primary sleep disorders. Wrist actigraphy revealed a non-24-hour sleep-wake pattern. Delay in temperature rhythm and dissociation with melatonin rhythms were also noted. Treatment with oral melatonin restored normal sleep-wake schedule. In a follow-up p
