Nocardia endophthalmitis following uncomplicated phacoemulsification and implantation of a posterior chamber intraocular lens.

A patient developed endophthalmitis 15 days after uncomplicated cataract extraction. Excised infectious material sequestered in the capsular bag revealed Nocardia on culture. She required multiple intravitreal injections of Amikacin and dexamethasone, pars plana vitrectomies with explantation of the lens, and chronic topical and oral sulfonamide antibiotics to control the infection. Clinicians should also consider Nocardia when the suspicion of fungal endophthalmitis is entertained, as Nocardia are resistant to antifungals, but respond to intravitreal amikacin and chronic topical and oral sulf

Phacoemulsification and posterior chamber intraocular lens implantation after scleral buckling, vitrectomy, or both.

BACKGROUND AND OBJECTIVE: Previous studies have documented the risk of complications for eyes with prior vitreoretinal surgery that later undergo intracapsular or extracapsular cataract extraction. The authors reviewed the records of patients with a history of vitreoretinal surgery and subsequent phacoemulsification to assess the visual outcome in the relatively early postoperative period and to determine the risk of postoperative complications. PATIENTS AND METHODS: The authors identified 21 eyes of 21 patients with a history of scleral buckling, vitrectomy, or both, who subsequently underwe…

Photic phenomena after phacoemulsification and posterior chamber lens implantation of various optic sizes.

Anecdotal reports and in vitro and clinical studies indicate that 5.0 mm x 6.0 mm ovoid posterior chamber lenses may have a higher incidence of photic phenomena, perhaps caused when the thicker truncated lens edge presents within the pupillary aperture. To evaluate these subjective phenomena, we gave a questionnaire to a random sample of patients who received one of three different poly(methyl methacrylate) (PMMA) posterior chamber lens optic styles (5.0 x 6.0 mm ovoid, 7.0 mm round, 5.5 mm round) placed in the bag. About 20% of all IOL groups reported photic phenomena, with symptoms similar b

[Minimal endothelial cell loss following phacoemulsification and implantation of a posterior chamber lens].

Fifty patients with senile cataract were treated consecutively by phacoemulsification in the posterior chamber or pupillary plane using a bimanual technique followed by implantation of a Kratz posterior chamber lens. Central endothelial cell density was determined both before and after surgery. The average central endothelial cell loss was 4.1 +/- 3.02%. Follow-up examinations of a small number of patients 27 months after phacoemulsification and posterior chamber lens implantation showed no additional lens-induced endothelial cell loss.
