Quality of life in lower limb amputees: a cross-sectional study from a tertiary care center of South India.

BACKGROUND: Various literature have dealt with the physical aspect of amputation and the functional outcome after amputation. There are a fe … e, and they were found to have a significantly better QOL. The prevalence of residual limb pain and phantom limb pain was 52.9% and 37%, respectively. CONCLUSION: Nearly half of the population perceived that their QOL was neither poor nor good. The QOL of the physical domain was better than the other domains. The most important factors that were found to be a …

Targeted muscle reinnervation following external hemipelvectomy or hip disarticulation: An anatomic description of technique and clinical case correlates.

BACKGROUND: Targeted muscle reinnervation (TMR) has been shown to decrease or prevent neuropathic pain, including phantom and residual limb pain, after extremity amputation. Currently, a paucity of data and lack of anatomical description exists regarding TMR in the setting of hemipelvectomy and/or hip disarticulations. We elaborate on the technique of TMR, illustrated through cadaveric and clinical correlates. METHODS: Cadaveric dissections of multiple transpelvic exposures were performed. The major mixed motor and sensory nerve branches were identified, dissected, and tagged. Amputated perip

Phantom Radiculopathy: Case Report and Review of the Literature.

BACKGROUND: Phantom radicular pain is very uncommon. To the best of our knowledge, only 14 cases have been described in the literature. A review of the literature revealed the most common cause of phantom radicular pain to be lumbar disc herniation and, furthermore, that treatment with epidural steroid injection or surgical decompression relieves pain in almost all cases. A significant number of patients with superimposed phantom radiculopathy may be missed because of the high incidence of degenerative lumbar spine diseases in the adult population, as well as the fact that amputee patients ver

The pathophysiology of the chronic pain cycle.

The diagnosis and treatment of chronic pain is an area in which the field of medicine is failing to offer the best possible care to patients. This article presents a clinically driven view of the condition with an emphasis on active recognition and intervention. Defining aspects of chronic pain, including the emotional and psychologic components, are introduced with regard to recognition and diagnosis. The pathophysiologic mechanisms that underlie the transition from acute pain to chronic pain are reviewed from the standpoint of treatment intervention. Finally, specific chronic pain states tha

[Residual limb pain from bone overgrowth after amputation].

Bone overgrowth is classified as either heterotopic or orthotopic ossification. Three cases are reported in which bone overgrowth caused complications in three adult amputees. The cases demonstrate that bone overgrowth is an important differential diagnosis to residual limb pain following amputation. Bone overgrowth after amputation is very seldom described in literature. Furthermore, heterotopic ossification has not previously been reported following complications to initial amputation but only in traumatic amputees.

Other surgical interventions.

Optimal management of patients with chronic neuropathic pain requires a multidisciplinary approach that may include surgery. Yet despite the fact that lumbosacral spinal surgery, for example, is performed in thousands of patients every year, there is very little controlled clinical data to support its use or that of other surgical techniques in the treatment of chronic nonmalignant pain, especially neuropathic pain. Nevertheless, there is evidence of some success for ablative techniques such as dorsal root entry zone lesioning for phantom limb pain and girdle-zone neuropathic pain, and sympath…

[Rehabilitation of invalids with sequelae and late complications of gunshot wound of the locomotor system].

The authors have generalized their experience in treating 2178 invalids of the Great Patriotic War with consequences of gunshot injuries and diseases of the locomotor system. 49.24% of the patients had chronic gunshot osteomyelitis, 36.38% had diseases of stumps of the extremities, 6.5% had false joints and 7.81% had ulcerated cicatrices and trophic ulcers. The authors give examples of late complications of the consequences of the gunshot injuries, changes in the morbidity structure among the invalids in the recent years, associated diseases and methods of their treatment and prevention.

Sex-specific Differences in Multisite Pain Presentation among Adults with Lower-Limb Loss.

OBJECTIVES: Multisite pain remains significantly understudied following lower-limb loss (LLL), especially among females. This study aimed to explore sex-specific differences in the presentation of multisite pain post LLL. Hypotheses were multisite pain would be more prevalent among females post LLL as compared with males, and female sex would be significantly associated with multisite pain prevalence. METHODS: In this cross-sectional study, participants answered standardized questions regarding the presence of amputation-specific (ie, phantom-limb, residual-limb) and secondary (ie, contralate

Towards a neural network based therapy for hallucinatory disorders.

Pattern completion in a neural network model of the thalamus and a biologically plausible model of synaptic plasticity are the key concepts used in this paper for analyzing some cognitive disorders that involve hallucinations of several kinds: visual hallucinations in the Charles Bonnet syndrome and psychedelic drugs consumption, somatic hallucination in phantom limbs, cognitive hallucinations in schizophrenia and even in multiple personality disorders. All these types of hallucinations are proposed to be the result of a pattern completion dynamics performed in thalamic deafferented areas. Eff…

Electrical nerve stimulation for sensory-neural pathway reconstruction in upper-limb amputees.

INTRODUCTION: The loss of the neural sensory function pathways between the stump limbs and the brain greatly impacts the rehabilitation of limb function and the daily lives of amputees. Non-invasive physical stressors, such as mechanical pressure and transcutaneous electrical nerve stimulation (TENS), could be potential solutions for recovering somatic sensations in amputees. Previous studies have shown that stimulating the residual or regenerated nerves in the stumps of some amputees can produce phantom hand sensations. However, the results are inconclusive due to unstable physiological respo

Ejection associated injuries within the German Air Force from 1981-1997.

From 1981-1997 there were 86 ejections from 56 aircraft within the German Air Force. Of these, 24 accidents were associated with the F-104 Starfighter, 14 with the PA 200 Tornado, 12 from the F-4 Phantom, 5 from the Alpha Jet and 1 from a MiG 29 Fulcrum. One case involved a front seat pilot, who had already sustained fatal injuries from midair collision, being command ejected by the rear seat pilot. The remaining 85 ejections are the basis of this study. One weapons system officer died from hypothermia after landing in the sea and another from bleeding into the medulla oblongata after flailing

Mirror visual feedback therapy. A practical approach.

Mirror visual feedback (MVF) was first proposed as a therapy to relieve amputee phantom limb pain in the early 1990s. It is increasingly used to treat a range of other chronic pain conditions. The evidence base to date is limited. Much of the literature consists of pilot projects or case study designs although larger randomized controlled trails are now emerging. However, the described protocols for MVF are inadequate to adapt to clinical practice. In addition, the therapist sees a heterogeneous population whose characteristics may fall outside those of the tight inclusion/exclusion criteria o…

Amputations in the treatment of Dupuytren's disease.

23 finger amputations in 19 patients operated on for Dupuytren's disease were reviewed 6 months to 8.5 years after operation (mean 4 years). The distribution of amputations were 17 little fingers and six ring fingers. We found a recurrent lack of extension in nine out of 16 finger amputations distal to the MP joint and painful neuroma or phantom limb pain in five out of seven little finger amputations through or proximal to the MP joint. When amputation in the little finger is necessary, disarticulation of the MP joint may be preferable to amputation at a more distal level. Alternatives to fin…

Mechanism of Action of Peripheral Nerve Stimulation for Chronic Pain: A Narrative Review.

The use of stimulation of peripheral nerves to test or treat various medical disorders has been prevalent for a long time. Over the last few years, there has been growing evidence for the use of peripheral nerve stimulation (PNS) for treating a myriad of chronic pain conditions such as limb mononeuropathies, nerve entrapments, peripheral nerve injuries, phantom limb pain, complex regional pain syndrome, back pain, and even fibromyalgia. The ease of placement of a minimally invasive electrode via percutaneous approach in the close vicinity of the nerve and the ability to target various nerves h…
