Fine-tuned machine learning models for the discovery of dye nanoparticles with enhanced lung delivery.

Drug delivery using self-assembling drug-excipient nanoparticles offers a scalable strategy to improve the solubility, bioavailability, and biodistribution of poorly soluble therapeutics. However, progress in the development and translation of these materials has been limited by the lack of safe excipients capable of stabilizing drug-rich nanoparticles that allow for extrahepatic delivery. Here, we present an adaptive machine learning-guided workflow for excipient discovery that prospectively prioritizes candidate dyes and identifies clinically relevant drugs compatible with dye-based nanopart

Regorafenib in Pediatric Patients With Advanced Osteosarcoma: A Case Series from a Single Institution.

BACKGROUND/AIM: Pediatric osteosarcoma is a rare and aggressive malignancy with limited treatment options in cases of progression or metastasis. Regorafenib is an oral multikinase inhibitor targeting vascular endothelial growth factor receptor (VEGFR), platelet-derived growth factor receptor (PDGFR), and other kinases, currently approved in Japan for certain gastrointestinal cancers. Its clinical use in osteosarcoma, especially in pediatric patients, has not been well established. PATIENTS AND METHODS: We retrospectively analyzed six pediatric patients with advanced osteosarcoma who were trea

Lenvatinib as salvage therapy in advanced and progressive GI-NET.

The efficacy of lenvatinib in treating gastrointestinal neuroendocrine tumors (GI-NETs) has been explored in preclinical studies and early-phase clinical trials, but real-world data remain limited. Data of sixteen patients (median age 65 years; 62.5% female) with advanced and progressive GI-NET who were treated with lenvatinib at the ENETS Center, University Hospital Essen, between July 2019 and February 2024 were analyzed. Patients received all other approved therapies and showed progression within 2 months. Salvage therapy with lenvatinib was initiated after approval from health insurance. M

Relugolix in treatment of prostate cancer: a review.

BACKGROUND: Androgen deprivation therapy (ADT) is a cornerstone treatment for advanced prostate cancer. While effective, traditional injectable luteinizing hormone-releasing hormone (LHRH) agonists are associated with an initial testosterone flare and potential cardiovascular risks. Relugolix is a novel, oral gonadotropin-releasing hormone (GnRH) antagonist developed to provide rapid suppression without a testosterone flare. This review synthesizes the latest evidence on the efficacy, safety, and clinical utility of relugolix. METHODS: This non-systematic review was conducted via a search of

Degradable Multifunctional Microspheres for Combined Thermotherapy-Embolization-Immune Activation Therapy of Hepatocellular Carcinoma.

Transarterial chemoembolization (TACE) is the first-line treatment for unresectable intermediate and advanced hepatocellular carcinoma (HCC). Nevertheless, the upregulation of vascular endothelial growth factor A (VEGF-A) and the angiogenesis response triggered by the hypoxic environment following embolization can facilitate tumor recurrence. The combination of TAE/TACE and anti-angiogenic drugs has become one of the important strategies in the treatment of liver cancer. However, anti-angiogenic strategies using monotherapies that only target VEGF or vascular endothelial growth factor receptor

Anaplastic thyroid carcinoma with osteoclast-like giant cells: a case report and a study of a potential therapeutic approach.

Anaplastic thyroid carcinoma (ATC) is a highly aggressive neoplasm with no effective treatment options. ATC with osteoclast-like giant cells (OGCs; ATC/OGC) is a rare variant of ATC, and no detailed pathological examination has been reported to date. A 59-year-old woman presented with sudden neck swelling. Computed tomography revealed a 5 cm tumor in the thyroid gland, which was surgically resected. Pathological examination revealed a diagnosis of ATC/OGC. The patient succumbed to progressive lung metastases within four months despite postoperative lenvatinib therapy. Immunohistochemical (IHC)

Case report and literature review: tislelizumab combined with lenvatinib and polymeric micellar paclitaxel for thymic squamous cell carcinoma.

Thymic squamous cell carcinoma (TSCC) is a rare malignancy with an annual incidence of 0.15-0.32 per 100,000 population and exhibits aggressive behavior including early metastasis. Anti-PD-1 immunotherapy combined with chemotherapy has emerged as a potential strategy for TSCC. We report a 60-year-old male with a 1-month history of persistent dry cough and generalized fatigue. Diagnostic evaluation revealed advanced TSCC (cT3N2M1b), with bilateral lung, right pleural, left adrenal, mediastinal and right hilar lymph nodes metastases. Significantly elevated CA125 and CYFRA211 levels were observed

Ultrasound-triggered nano delivery of lenvatinib for selective immunotherapy treatment against hepatocellular carcinoma.

Although there have been remarkable advances in the treatment of hepatocellular carcinoma (HCC), the prognosis remains poor in those with advanced stage and more effective therapeutic options are urgently needed. Lenvatinib (Len), a multiple receptor tyrosine kinase inhibitor, is an emerging molecular targeted agent for HCC, whose immunomodulatory activities have been investigated. However, Len utilization is limited because of its low metabolic stability, poor bioavailability and dose-dependent toxicity, rendering its direct use insufficient for immune modulation. Stimuli-responsive nanoparti

Histology-Specific Clinical Trial of Lenvatinib and Pembrolizumab in Patients with Sarcoma.

PURPOSE: Survival of patients with metastatic sarcoma remains poor, and there is a pressing need for new therapies. Most sarcoma subtypes are not responsive to immune checkpoint inhibition alone. Lenvatinib, a multireceptor tyrosine kinase inhibitor targeting tumor vasculature, has an immunomodulatory activity that contributes to its antitumor effects. Therefore, we hypothesized that a combination of lenvatinib and pembrolizumab would lead to improved clinical outcomes in patients with sarcoma. PATIENTS AND METHODS: This was an open-label, single-arm study of lenvatinib and pembrolizumab in t

Plain language review: the safety of relugolix combination therapy for advanced prostate cancer.

WHAT IS THIS SUMMARY ABOUT?: Advanced prostate cancer is a cancer that began in the prostate (a part of the male body) and has spread to other parts of the body. This is a review of two clinical research studies of patients with advanced prostate cancer who were treated with relugolix combination therapy. Relugolix is a medicine taken by mouth that lowers a male sex hormone, called testosterone. Relugolix is sometimes combined with other medicines such as novel hormonal therapies (NHTs) or chemotherapy to treat advanced prostate cancer. In one study, patients were treated with relugolix combin

Testosterone recovery post discontinuation of androgen deprivation for the treatment of advanced prostate cancer.

While suppressing testosterone to castration levels is the aim of androgen deprivation therapy for the treatment of advanced prostate cancer, studies have shown that prolonged low testosterone levels can have negative effects on patients' overall health and quality of life. This podcast covers two recently published papers that examined testosterone recovery in different ways. One real-world study assessed the impact of delayed testosterone recovery on clinical outcomes in patients with prostate cancer. A second subgroup analysis of the HERO trial assessed rates of testosterone recovery in pat

Well-controlled viremia reduces the progression of hepatocellular carcinoma in chronic viral hepatitis patients treated with lenvatinib.

Lenvatinib has been approved as one of the first-line treatments for advanced hepatocellular carcinoma (HCC) due to its high treatment efficacy being non-inferior to sorafenib. Previous studies have shown well-controlled viremia contributes to the prognosis of HCC patients receiving first-line sorafenib; hence, we postulated this association might also exist in HCC patients with lenvatinib treatment. From April 2018 to December 2021, 201 unresectable HCC patients with first-line lenvatinib treatment in our institute were assessed. High-effect nucleoside analogues were administered for hepatiti

Metformin Enhances the Anti-Cancer Efficacy of Sorafenib via Suppressing MAPK/ERK/Stat3 Axis in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) incidence, as well as related mortality, has been steadily increasing in the USA and across the globe, partly due to the lack of effective therapeutic options for advanced HCC. Though sorafenib is considered standard-of-care for advanced HCC, it only improves median survival by a few months when compared to placebo. Sorafenib is also associated with several unpleasant side effects that often lead to early abatement of therapy. Here, we investigate whether a combination regimen including low-dose sorafenib and a non-toxic dose of anti-diabetic drug metformin can a

Lenvatinib Synergistically Promotes Radiation Therapy in Hepatocellular Carcinoma by Inhibiting Src/STAT3/NF-κB-Mediated Epithelial-Mesenchymal Transition and Metastasis.

PURPOSE: This study suggested that lenvatinib may incapacitate hepatocellular carcinoma (HCC) to radiation treatment by abrogating radiation-induced Src/signal transducer and the activator of transcription 3 signaling (STAT3)/nuclear factor-κB (NF-κB) to escalate radiation-induced extrinsic and intrinsic apoptosis. These findings uncover the role of targeting Src and its arbitrating epithelial-mesenchymal transition (EMT), which could increase the anti-HCC efficacy of radiation therapy (RT). Lenvatinib and sorafenib are multikinase inhibitors used to treat HCC. Lenvatinib is noninferior to sor
