Effects of type II thyroplasty on adductor spasmodic dysphonia.

OBJECTIVES: Type II thyroplasty, or laryngeal framework surgery, is based on the hypothesis that the effect of adductor spasmodic dysphonia (AdSD) on the voice is due to excessively tight closure of the glottis, hampering phonation. Most of the previous, partially effective treatments have aimed to relieve this tight closure, including recurrent laryngeal nerve section or avulsion, extirpation of the adductor muscle, and botulinum toxin injection, which is currently the most popular. The aim of this study was to assess the effects of type II thyroplasty on aerodynamic and acoustic findings in …

Vocal Behavior of Teachers Reading with Raised Voice in a Noisy Environment.

(1) Objective: Teaching is a particularly voice-demanding occupation. Voice training provided during teachers' education is often insufficient and thus teachers are at risk of developing voice disorders. Vocal demands during teaching are not only characterized by speaking for long durations but also by speaking in noisy environments. This provokes the so-called Lombard effect, which intuitively leads to an increase in voice intensity, pitch and phonation time in laboratory studies. However, this effect has not been thoroughly investigated in realistic teaching scenarios. (2) Methods: This stud…

Japanese macaque phonatory physiology.

Although the call repertoire and its communicative function are relatively well explored in Japanese macaques (Macaca fuscata), little empirical data are available on the physics and the physiology of this species' vocal production mechanism. Here, a 6 year old female Japanese macaque was trained to phonate under an operant conditioning paradigm. The resulting 'coo' calls and spontaneously uttered 'growl' and 'chirp' calls were recorded with sound pressure level (SPL) calibrated microphones and electroglottography (EGG), a non-invasive method for assessing the dynamics of phonation. A total of…

[Chronic aspiration secondary to a Staffieri fistula: successful therapy with Provox voice prosthesis].

BACKGROUND: The Staffieri technique is a method for surgical rehabilitation of the voice after laryngectomy. A fistula between trachea and esophagus is created to achieve a so-called "Neoglottis phonatoria". The assignment of the neoglottis is to protect the airways during ingestion and to give the patient a chance to phonate during expiration. PATIENT: The case of a 71-year-old man with aspiration via his Staffieri fistula is presented. The fistula was created in 1979 after laryngectomy because of a laryngeal carcinoma. He also suffered from a metastasized gastric carcinoma which was cured b

Noninvasive assessment of vocal fold mucosal wave velocity using color doppler imaging.

The vibratory movement of the vocal folds (VF) plays an important role in normal function of phonation. We developed a noninvasive technique to quantify the human mucosal wave velocity (MWV) in vivo using color Doppler imaging (CDI). During phonation, the motion of mucosa-air interface generates a unique pattern of US color artefacts that assist the identification of true VF location. An in vitro study using a vibrating string phantom was conducted to investigate how the CDI displayed a vibrating soft tissue at high frequency. The vibrating amplitude, frequency, mass density and the acoustic i…

Finite Element Simulation of Two Vowel Changes in a Male Vocal Tract Model With an Aramany Class I Maxillectomy.

BACKGROUND: Acoustic analysis of vowels offers an objective method for evaluating speech in maxillectomy patients. Prior studies have focused primarily on lower formants linked to vowel identification, while changes in higher formants-critical for voice quality and individualization-remain underexplored. OBJECTIVES: This study used finite element (FE) modeling to analyze vocal tract transfer function up to 10 kHz, acoustic mode shapes, and phonation of the vowels /aː/ and /iː/ in Aramany class I maxillectomy models. METHODS: Healthy and maxillectomy vocal tract models were created using magn…

Integration of Dysphagia Therapy Techniques into Voice Rehabilitation: Design and Content Validation of a Cross-Therapy Protocol.

BACKGROUND: The intricate relationship between swallowing and phonation, sharing anatomical and physiological substrates, underscores a clinical demand for integrated therapeutic approaches. Existing interventions often address these functions in isolation, overlooking their interconnected dynamics. OBJECTIVE: To design and validate a cross-therapy protocol incorporating dysphagia therapy techniques (maneuvers/exercises) into voice rehabilitation. This protocol aims to exploit the shared biomechanical components of swallowing and phonation to improve both functions simultaneously in patients …

Do tapered tracheostomy cuffs improve translaryngeal gas flow when compared to barrel cuffed fenestrated tubes: A laboratory study.

OBJECTIVE: The benefits of tracheostomy are well documented and include improved comfort and a reduction in sedative requirements that may facilitate more rapid ventilation weaning. A stable airway established with tracheostomy allows pulmonary toilet that may help in addressing aspiration. It is postulated that it may also increase translargyngeal airflow and allow phonation. We hypothesized that taper-shaped cuffed tracheostomy tubes have less bulk upon cuff deflation, and on this basis, gas flow past the deflated tapered cuff is better than non-tapered barrel cuffs and equal to gas flow in …

Evaluation of voice acoustic parameters related to the vocal-loading test in professionally active teachers with dysphonia.

INTRODUCTION: Teachers are at risk of developing voice disorders. A clinical battery of vocal function tests should include non-invasive and accurate measurements. The quantitative methods (e.g., voice acoustic analysis) make it possible to objectively evaluate voice efficiency and outcomes of dysphonia treatment. MATERIALS AND METHODS: To identify possible signs of vocal fatigue, acoustic waveform perturbations during sustained phonation were measured before and after the vocal-loading test in 51 professionally active female teachers with functional voice disorders, using IRIS software. All …

Oral breathing challenge in participants with vocal attrition.

Vocal folds undergo osmotic challenge by mouth breathing during singing, exercising, and loud speaking. Just 15 min of obligatory oral breathing, to dry the vocal folds, increases phonation threshold pressure (Pth) and expiratory vocal effort in healthy speakers (M. Sivasankar & K. Fisher, 2002). We questioned whether oral breathing is more detrimental to phonation in healthy participants with a history of temporary vocal attrition. The effects of a 15-min oral or nasal breathing challenge on Pth and perceived expiratory vocal effort were compared for participants reporting symptoms of vocal a…
