Chronic exertional compartment syndrome of the leg in the military.

CECS is a common source of lower extremity disability among young athletic cohorts and military personnel. The five cardinal symptoms are pain, tightness, cramps, weakness, and diminished sensation. History and clinical examination remain the hallmarks for identifying CECS, although ICP measurements during exercise stress testing may be used to confirm diagnosis. Nonsurgical management is generally unsuccessful, although gait retraining may have benefits in selected individuals. When conservative measures have failed, operative management may be considered with fascial release of all affected

Mouth rinsing with a bitter solution without ingestion does not improve sprint cycling performance.

PURPOSE: Recently, we have shown that combining mouth rinsing with the ingestion of a 2 mM quinine solution immediately before a 30-s cycling sprint significantly improves performance. However, the strong bitterness of such a solution produces an unpleasant taste and evokes nausea at higher concentrations. Given the possibility that mouth rinsing with quinine without ingesting it may not produce nausea, a mouth rinse only protocol may be a more practical approach to administer quinine for improving exercise performance. The purpose of the present study was to determine whether mouth rinsing wi

Kinematics of cross-country sit skiing during a Paralympic race.

The study had three purposes: to verify a hypothesized speed decrease during the 15 km cross-country sit skiing (CCSS) race; documenting this possible fatigue effect (speed decrease), to evaluate changes among the four laps in kinematics parameters (cycle speed, cycle duration, cycle length, duty cycle (percentage ratio between pushing and total cycle duration), pole inclination, trunk inclination and shoulder-hand distance); to compare the kinematics parameters in cross-country sit skiers of different level. Video recordings were carried out during the 2006 Turin Winter Paralympic Games with

Complex training in ice hockey: the effects of a heavy resisted sprint on subsequent ice-hockey sprint performance.

The aim of the study was to investigate the acute effect of a heavy resisted sprint when used as a preload exercise to enhance subsequent 25-m on-ice sprint performance. Eleven competitive ice-hockey players (mean ± SD: Age = 22.09 ± 3.05 years; Body Mass = 83.47 ± 11.7 kg; Height = 1.794 ± 0.060 m) from the English National League participated in a same-subject repeated-measures design, involving 2 experimental conditions. During condition 1, participants performed a 10-second heavy resisted sprint on ice. Condition 2 was a control, where participants rested. An electronically timed 25-m spri

Absence of exertional hyperthermia in a 17-year-old boy with severe burns.

An important safety concern when exercising burned patients is the potential for an excessive increase in core body temperature (hyperthermia=body core temperature>39 degrees C) during exercise. We examined the thermoregulatory response to exercise in the heat (31 degrees C, relative humidity 40%) in a 17-year-old boy with a 99% TBSA burn. A 30-minute exercise test was performed at an intensity of 75% of his peak aerobic capacity. Intestinal temperature was assessed via telemetry with an ingestible capsule. Intestinal temperature was measured before, during, and postexercise. The patient compl

The impact of resistance exercise on the cognitive function of the elderly.

PURPOSE: The purpose of this study was to assess the impact of 24 wk of resistance training at two different intensities on cognitive functions in the elderly. METHODS: Sixty-two elderly individuals were randomly assigned to three groups: CONTROL (N = 23), experimental moderate (EMODERATE; N = 19), and experimental high (EHIGH; N = 20). The volunteers were assessed on physical, hemodynamic, cognitive, and mood parameters before and after the program. RESULTS: On the 1 RM test (P < 0.001), the two experimental groups performed better than the CONTROL group, but they did not show differences b

Heat and cold: what does the environment do to marathon injury?

The medical work load seems to increase both with heat and humidity, and with cold and rainy conditions. Heat tolerance during exercise is variable and heat intolerance may contribute to collapse and increase medical encounters. Exposure to cold, wet conditions results in increasing incidence of hypothermia in exhausted marathon runners. Finish-line encounters and course dropouts increase as conditions cool and warm away from the most advantageous conditions in the 4.4-15 degrees C (40-59 degrees F) wet bulb globe temperature (WBGT) range. The risk of requiring medical attention and not finish

Pushing and pulling: personal mechanics influence spine loads.

This study assessed several mechanical issues related to low back loading during pushing and/or pulling tasks. Nine male participants performed two-handed pushing and pulling tasks at two handle heights with three loads, using a cable pulley system. Four of these men were professional firefighters trained in performing pushing and pulling tasks while the other five were graduate students who lacked manual work experience. The more experienced firefighters produced less spinal compression and shearing forces when compared to the less experienced students under the same conditions. The firefight

Mechanical energy in toddler gait. A trade-off between economy and stability?

Mechanical energy expenditure was investigated in children who are just learning to walk and compared with adult mechanical energy expenditure during walking. First, we determined whether the inverted pendulum (IP) mechanism of energy exchange was present in toddlers. It seems that new walkers partially make use of this energy saving mechanism, but it is less efficient than in adults. The reduced recovery values (R=40% at optimal speeds in toddlers compared to 70% in adults) can be explained by their low self-selected walking speed in combination with their tossing gait (large vertical oscilla

Recording of intratracheal pressure in the horse under field conditions as a method for evaluation of upper airway resistance.

Recording of intratracheal pressure during exercise as a method for evaluating upper airway obstruction in the horse has previously been carried out on the treadmill. In the present study the method was applied to normal horses, outdoors under field conditions, to assess its usefulness for analysis of respiratory problems mainly occurring under racing conditions. The horses examined performed a standardized exercise test during which the intratracheal pressure was recorded with a tape recorder receiving amplified signals from a transducer connected to a percutaneously inserted intratracheal ca

Pulmonary haemodynamics in the exercising horse and their relationship to exercise-induced pulmonary haemorrhage.

Exercise-induced pulmonary haemorrhage (EIPH) is a common occurrence in race horses. Although blood in cases of EIPH has been suspected to originate from the bronchial circulation, which receives approximately 1% of the left ventricular output, physiological evidence has recently emerged to indicate that the pulmonary circulation, which receives the entire output of the right ventricle, is a more likely source. High transmural pulmonary capillary pressures have been shown to cause breaks in the capillary endothelium, basement membrane as well as in the alveolar epithelium. Blood constituents e

Kegel dyspareunia: levator ani myalgia caused by overexertion.

BACKGROUND: The onset of unexplained dyspareunia with a normal pelvic examination requires a careful evaluation to search for an etiology. CASES: The initiation of Kegel exercises led to dyspareunia in several patients whose pain could be localized to the levator ani muscles. Elimination of the muscular overexertion led to resolution of the dyspareunia in all cases. CONCLUSION: Dyspareunia associated with the initiation of Kegel exercises may be due to levator ani myalgia. Once recognized as a potential source of dyspareunia, levator myalgia is easy to diagnose and treat.

[Effect of occupational physical work on blood pressure].

The present knowledge on blood pressure variability during occupational physical activity is based almost exclusively on the results obtained from the exercise tests, either isotonic (dynamic) or isometric (static). The findings of several studies indicate that during isotonic exercise in normotensive subjects the systolic blood pressure increases whereas the diastolic blood pressure shows only minor changes. In the hypertensives, both systolic and diastolic pressures tend to increase more substantially. Isometric exercise causes transient systemic hypertension but with individual differences.

A comparative study of the metabolic effort expended by horse riders during a jumping competition.

The three main Olympic horse riding disciplines are dressage, jumping, and three-day eventing (including dressage, cross country and jumping). In the jumping discipline (obstacle race), the 'team' (horse rider) is judged under the different conditions that might take place in a varied run. The horse is expected to show power and ability; the rider must show riding skill and good physical condition. However, the different conditions encountered by the rider during competition (duration of event, continuous isometric working level, especially in the inferior trunk, lead us to consider the need f

Cholinergic pruritus in a horse.

An 8-year-old gelding, with a 1-year history of intense pruritus only after exercise, was diagnosed as having cholinergic pruritus. Provocative testing, using exercise and hot-water baths to increase core body temperature, assisted in the diagnosis. Cholinergic pruritus in human beings is a variant of the more common syndrome, cholinergic urticaria, which is characterized by intense pruritus and pinpoint urticaria. With cholinergic pruritus, intense itching results without urticaria. Specific diagnostic laboratory tests have not been developed.

[Intolerance to exercise caused by carnitine palmitoyltransferase deficiency].

Exercise intolerance with slight myalgia worsened by a cold environment resulted in the diagnosis of CPT deficiency in a young boy with no family history. Recurrent myoglobinuria is not the hallmark of such a deficit. Biochemical examination of muscle biopsy in suggestive situations is the way to diagnosis. Concomitant myoadenylate deficiency in this patient could represent a precipitating factor. Partial CPT deficiency in all the patients reported was first explained through the mechanism of intra mitochondrial fatty acid transport but raises questions about the regulation of CPT synthesis.

Exertion-induced heat stroke in a military setting.

Heat stroke is a medical emergency characterized by sudden loss of consciousness and by failure of the heat-regulating mechanism, as manifested by high fever (usually above 104 degrees F) and cessation of sweating. Fatality rates for patients with heat stroke can vary from 0% to more than 40%. Immediately after heat stroke has occurred or is suspected, aggressive cooling measures should be initiated. This case illustrates a 24-year-old commissioned officer who developed heat stroke while running in PT formation dressed with shorts and a T-shirt. Although this patient was in good shape and the

Muscle mass as a factor limiting physical work.

Maximal exercise has been performed by eight men and eight women, using four types of ergometer (2-leg, 1-leg, arm + shoulder, and arm) while breathing room air and while breathing 12% O2. Results have been related to anthropometric estimates of muscle mass in the active limbs. Although significant sex differences of O2 transfer and power output are shown, the sex-specific aerobic performance was roughly proportional to active muscle volume (both when comparing individuals on a given type of ergometer and when comparing average scores of the several types of ergometer). However, the relationsh
