New solutions against the dengue global threat: opportunities for Wolbachia interventions.

Reliance on traditional vector control methods, such as insecticides and "clean up" source reduction efforts for reducing mosquito breeding sites, has proven increasingly ineffective and costly in the battle against dengue. The Wolbachia-based mosquito replacement strategy, which replaces wild mosquitoes with mosquitoes trans-infected with Wolbachia bacteria, preventing up to 77% dengue transmission, represents an advancement in prevention. Integrating this approach into current routine dengue control efforts could reduce dengue incidence. However, challenges such as implementation costs, the

Autopsy of a failed trial part 1: A qualitative investigation of clinician's views on and experiences of the implementation of the DAISIES trial in UK-based intensive eating disorder services.

OBJECTIVE: The DAISIES trial, comparing inpatient and stepped-care day patient treatment for adults with severe anorexia nervosa was prematurely terminated in March 2022 due to poor recruitment. This qualitative study seeks to understand the difficulties faced during the trial by investigating stakeholders' views on and experiences of its implementation. METHOD: Semi-structured interview and focus group transcripts, and trial management and oversight group meeting minutes from May 2020-June 2022 were analysed using thematic analysis. Participants were 47 clinicians and co-investigators involv

Targeting systems not individuals: Institutional and structural drivers of absenteeism among primary healthcare workers in Nigeria.

Universal Health Coverage (UHC) can only be achieved if people receive good quality care from health workers, yet in Nigeria, as in many other low- and middle-income countries (LMICs), many health workers are absent from work. Absenteeism is a well-known phenomenon but is often considered as the self-serving behaviour of individuals, independent from the characteristics of health systems structures and processes and the broader contexts that enable it. We undertook a qualitative inquiry among 40 key informants, comprising health facility heads and workers, community leaders and state-level hea

A Refugee Rose of competencies and capabilities for mental healthcare of refugees.

In this paper, I set out the challenges of care for refugees and suggest approaches to assessment and intervention. I discuss clinical interventions that can address the immediate concern of the clinician in a bio-psycho-social framework, and the value of considering eco-social and structural influences that can hinder recovery and perpetuate inequalities. Refugees face multiple adversities before, during and after escaping from life-threatening situations, political violence, torture and persecution. They present with complex health needs and encounter hostility from host countries and public

Rethinking the Top-Down Approach to Schistosomiasis Control and Elimination in Sub-Saharan Africa.

The control and elimination of schistosomiasis have over the last two decades involved several strategies, with the current strategy by the World Health Organization (WHO) focusing mainly on treatment with praziquantel during mass drug administration (MDA). However, the disease context is complex with an interplay of social, economic, political, and cultural factors that may affect achieving the goals of the Neglected Tropical Disease (NTD) 2021-2030 Roadmap. There is a need to revisit the current top-down and reactive approach to schistosomiasis control among sub-Saharan African countries and

Treatment of chronic venous insufficiency in Latin America.

OBJECTIVE: Venous disease is common in Latin America, with an estimated 68.11% prevalence of chronic venous disease. The diverse social, political, and economic characteristics of the many nations that make up Latin America mean that different conditions affect how these diseases are diagnosed and treated, which may differ markedly from the way they are treated by the health care systems of the United States and Europe. Our goal was to review the current state of treatment of chronic venous insufficiency (CVI) in Latin America. METHODS: This is a narrative review of the medical literature on

Cross-border COVID-19 spread amidst malaria re-emergence in Venezuela: a human rights analysis.

BACKGROUND: Since 2016 Venezuela has seen a collapse in its economy and public health infrastructure resulting in a humanitarian crisis and massive outward migration. With the emergence of the novel coronavirus SARS-CoV-2 at the end of 2019, the public health emergency within its borders and in neighboring countries has become more severe and as increasing numbers of Venezuelans migrants return home or get stuck along migratory routes, new risks are emerging in the region. RESULTS: Despite clear state obligations to respect, protect and fulfil the rights to health and related economic, social

Money, power and HIV: economic influences and HIV among men who have sex with men in sub-Saharan Africa.

Despite consistent evidence, effective interventions and political declarations to reduce HIV infections among men who have sex with men (MSM), coverage of MSM programmes in sub-Saharan Africa (SSA) remains low. Punitive legal frameworks and hostile social circumstances and inadequate health systems further contribute to the high HIV burden among MSM in SSA. The authors use the Modified Social Ecological Model to discuss economic influences in relation to HIV and MSM in SSA. Nigerian, South African and Ugandan case studies are used to highlight economic factors and considerations related to HI

Trauma history and psychopathology in war-affected refugee children referred for trauma-related mental health services in the United States.

There is an increasing need to deliver effective mental health services to refugee children and adolescents across the United States; however, the evidence base needed to guide the design and delivery of services is nascent. We investigated the trauma history profiles, psychopathology, and associated behavioral and functional indicators among war-affected refugee children presenting for psychological treatment. From the National Child Traumatic Stress Network's Core Data Set, 60 war-affected refugee children were identified (51.7% males, mean age = 13.1 years, SD = 4.13). Clinical assessments

Toward a national framework for the secondary use of health data: an American Medical Informatics Association White Paper.

Secondary use of health data applies personal health information (PHI) for uses outside of direct health care delivery. It includes such activities as analysis, research, quality and safety measurement, public health, payment, provider certification or accreditation, marketing, and other business applications, including strictly commercial activities. Secondary use of health data can enhance health care experiences for individuals, expand knowledge about disease and appropriate treatments, strengthen understanding about effectiveness and efficiency of health care systems, support public health

Maiming by design: Structural harm in oral health systems globally.

Tooth extraction is among the most common dental interventions, particularly in underfunded health systems where preventive, restorative, and rehabilitative dental services remain scarce. Drawing on Jasbir Puar's concept of 'maiming' tooth extraction is reframed from technical/economic inevitability to care that results in structural harm. Extraction-based oral healthcare reflects a transnational logic in which injury is normalised, repair withheld, and debility managed rather than resolved. The analysis shows how tooth extraction becomes an organised response to resource constraints; absence

Secondary Use of Health Data in Canada and the European Union: Expectations, Frictions and the Politics of Scale.

In this article, we trace expectations associated with efforts to promote secondary use of health data in Canada and the European Union. In Canada, we focus on initiatives enabling cross-jurisdictional data access in a highly devolved federal system with parallel commitments to Indigenous data sovereignty. In the EU, we focus on the European Health Data Space, a new regulatory regime intended to facilitate access to health data for clinical care, research, policymaking and innovation across 27 member states. Drawing on a comparative analysis of policy documents and semi-structured interviews a

Climate change as a threat multiplier: conflict pathways, inequities, and mental health impacts.

Climate change is increasingly recognized as a major driver of global instability, understood here as the interconnected disruption of social, economic, political, and security systems, acting as a threat multiplier that intensifies existing vulnerabilities. Its mental health impacts emerge through direct exposures-such as extreme heat, wildfires, floods, and other disasters-and through indirect pathways, including livelihood disruption, resource insecurity, displacement, and social fragmentation. These pressures contribute to heightened risks of post-traumatic stress disorder, depression, anx

Valuing Invisible Labor: A Feminist Political Economy Analysis of the Cost and Burden of Critical Care Nursing in Low- and Lower-Middle-Income Countries.

Critical care nursing is central to intensive care delivery, yet its costs and burdens remain largely invisible in low- and lower-middle-income countries. As critical care services expand in response to epidemiological transitions and global health emergencies, economic evaluations increasingly guide policy and resource allocation. However, prevailing health economic frameworks privilege infrastructure, technology, and physician-led interventions while treating nursing labor as a fixed or residual input. Using a feminist political economy and social reproduction lens, this discursive paper arg

Unlocking Africa's vaccine Independence: The critical role of technology transfer and intellectual property.

The COVID-19 pandemic exposed Africa's profound dependency on external vaccine suppliers, with the continent producing less than 1% of global vaccines and hosting only 1.1% of clinical trials. Despite emerging initiatives, most African facilities remain limited to downstream fill-and-finish operations, while full vaccine antigen manufacturing capacity is largely absent. This perspective paper examines the critical role of Technology Transfer (TT) and Intellectual Property (IP) access in achieving Africa's vaccine independence. Effective TT must extend beyond documentation to include unspoken k

Artificial and emotional intelligence: two key forces for teachers' professional development in an era of uncertainty.

The expansion of artificial intelligence in education is moving faster than institutions' capacity to protect teacher wellbeing. Current debates often oscillate between technocentric optimism, which equates success with personalisation and scale, and pre-emptive rejection driven by fears of dehumanization. This conceptual and critical synthesis proposes a different evaluative criterion: AI may support teachers' socio-emotional professional development only when it functions as relational infrastructure rather than as a symbolic substitute for human accompaniment. The manuscript introduces rela

New horizons in improving research capacity in English care homes for older adults.

As the care home sector expands over time, the health and social care needs of both residents and staff intensify and diversify. These sector-wide changes call for significant growth in research capacity to deliver useful, pertinent and timely evidence. In this paper we highlight growing pressures in the care home sector, and the major and enduring barriers to conducting research across that sector, within homes and amongst staff, drawing primarily on our experiences conducting public health research in England. These obstacles include a lack of national infrastructure, tradition and culture,

"From helper to survivor and back: Do's and don'ts from professionals who endured mass Trauma".

In an era marked by recurring large-scale disasters, whether political, man-made, or natural, the mental health community is increasingly confronted with the challenge of dual identity: the caregiver as survivor. At the 2025 PreAct Conference, the panel titled "From Helper to Survivor and Back: Do's and Don'ts from Professionals Who Endured Mass Trauma" brought together three seasoned professionals who offered raw, personal accounts of surviving mass trauma while maintaining their roles as mental health clinicians. Together, their experiences formed a powerful narrative of vulnerability, resil

Breaking the under-reporting cycle for zoonotic diseases in low-income and middle-income countries through national-level integration of community-based surveillance and response: insights from Cameroon, Chad, Côte d'Ivoire, and Mali.

Zoonotic diseases remain a major threat to both human and animal populations, especially in low-income and middle-income countries. A key challenge in managing these diseases is insufficient coordination between human and animal health sectors and the communities they serve, which often results in delayed outbreak detection and minimal disease control. Community health workers and community animal health workers can help fill this gap by identifying suspected cases early and facilitating communication between communities and health services. However, challenges, such as a scarcity of resources
