A 67-year-old woman was admitted to our department because of 5 years' duration of proximal muscle weakness. Serum CK was high, and EMG showed myogenic pattern, and muscle biopsy revealed remarkable inflammatory cells infiltrating around the destroyed muscle fibers. Her muscle weakness and hyperCKemia markedly improved by corticosteroid therapy, suggesting that the diagnosis was compatible with polymyositis (PM). In addition, serum calcium was high and phosphate was low. Serum parathormone level significantly elevated. The findings of diagnostic imaging procedures including echography, scintig…

Nailfold capillary microscopy was performed in 19 patients with polymyositis (PM) and 16 with dermatomyositis (DM). Enlarged capillary loops were seen more frequently in patients with DM (56%) than in those with PM (21%). Avascular lesions occurred in both PM and DM, but were more prevalent and more severe in patients with DM. Raynaud's phenomenon, arthritis, and pulmonary involvement were associated with greater numbers of enlarged capillary loops and more severe avascular lesions. The severity of the observed microvascular abnormalities did not correlate with the occurrence of malignancy or …

We report a 3-year-old boy with proximal painful muscle weakness associated with Kawasaki disease. The muscle biopsy revealed inflammatory cell infiltration with vasculopathy. This unique coexistence of polymyositis and Kawasaki disease is quite uncommon and suggests a newly recognized complication during Kawasaki disease.

Using the leucocyte migration inhibition (LIF) test we looked for evidence of cell-mediated hypersensitivity against myoglobin in 8 patients with polymyositis (PM) or dermatomyositis (DM). The migration index for PM-DM patients was 47.5 +/- 17%, while in the controls the index was 86 +/- 12% (p less than 0.001). The presence of serum antibodies against myoglobin was also investigated by passive haemagglutination (PH) and counterimmunoelectrophoresis (CIE). By PH the reciprocal titre of antimyoglobin antibodies was 150 +/- 28 in PM-DM compared with 16.2 +/- 15.9 and 8.7 +/- 7.7 in control patie…
