The mechanisms of orthopnea and the role of changes in respiratory mechanics in left ventricular failure (LVF) are poorly understood. We have measured total respiratory airflow resistance (Rrs) using forced oscillation in the sitting and supine positions in 10 patients with chronic LVF (NYHA II-III) shortly after recovery from acute LVF and in 10 matched control subjects (CON). The objective of this study was to investigate possible relationships between the incidence of sports injury and the existence of body mechanics defects in players of various codes of football. Injuries were recorded prospectively over a 24 month period. Fifteen aspects of body mechanics were evaluated at the start of the study using a specially developed photogrammetric technique known to have high reliability and sensitivity. The development of posture during the prenatal period was studied longitudinally in 12 matched pairs of low-risk and high-risk preterm infants. The gestational age of the infants in both groups ranged from 28 to 36 weeks. Ten patients were studied at periods ranging from 6 months to 25 years after oesophagogastrostomy and gastric drainage. Gastric emptying studies and overnight gastric aspirates in both the supine and erect positions were carried out. Evidence was found in these patients of increased duodenogastric reflux and poor gastric emptying in the supine posture. All of the patients had gastritis, and some had gastric mucosal ulceration. These findings have led to the conclusion that in the supine posture the combination of duodenogastric reflux and poor gastric emptying leads to gastric mucosal damage.
