Developing Federal Clinical Care Recommendations for Women.

UNLABELLED: The provision of family planning services has important health benefits for the U.S. POPULATION: Approximately 25 million women in the U.S. receive contraceptive services annually and 44 million make at least one family planning-related clinical visit each year. These services are provided by private clinicians, as well as publicly funded clinics, including specialty family planning clinics, health departments, Planned Parenthoods, community health centers, and primary care clinics. Recommendations for providing quality family planning services have been published by CDC and the O

Overuse of health care services in the United States: an understudied problem.

BACKGROUND: Overuse, the provision of health care services for which harms outweigh benefits, represents poor quality and contributes to high costs. A better understanding of overuse in US health care could inform efforts to reduce inappropriate care. We performed an extensive search for studies of overuse of therapeutic procedures, diagnostic tests, and medications in the United States and describe the state of the literature. METHODS: We searched MEDLINE (1978-2009) for studies measuring US rates of overuse of procedures, tests, and medications, augmented by author tracking, reference track

Adherence to hepatitis B virus vaccination at syringe exchange sites.

Injection drug users (IDUs) are at high risk for hepatitis B virus (HBV); however, they often do not receive preventive vaccination. IDUs who use mobile health care services linked to a syringe exchange program in New Haven were routinely screened for HBV, hepatitis C virus, and syphilis. Individuals without prior exposure to HBV were offered three-part vaccination series. Of the 212 IDUs screened for HBV infection, 134 (63%) were eligible (negative for HBV surface and core anti-bodies) for vaccination and 10 (4.7%) had evidence of chronic HBV infection. Compared to those with previous exposur

Examination of rural-urban disparities in utilization of preventive dental procedures in the US pediatric population: A cross-sectional study.

PURPOSE: To determine the extent to which there were disparities in access to recommended preventive oral services between US-based children living in rural versus urban areas. METHODS: The study used cross-sectional, parent-reported National Survey of Children's Health data from 2022, which is the most current year of publicly available data when analyses were performed. Inclusion criteria included all children from the ages of 2 through 17. The US Census Bureau definition of rural was used to guide the analyses. Survey procedures within Stata were used to build the multiple regression model

Childhood blindness: dateline Africa.

Prevalence rates of childhood blindness and infant and childhood mortality in Africa are the highest in the world. Major factors contributing to these high rates are similar for both blindness and mortality: malnutrition, measles and other acute febrile infections, malnutrition with xerophthalmia, and limited or poor access to preventive and curative health services. Because of high national growth rates, stagnant economies, and the failure of national health care systems to expand with rapidly growing populations, childhood blindness in Africa likely will remain a significant international pu

Oral Health Behaviors and Challenges Among Migrants in Italy: A Cross-Sectional Study.

BACKGROUND: Oral health disparities remain a significant public health concern among migrant populations, often driven by socioeconomic barriers, limited access to dental care, and challenges in health information acquisition. OBJECTIVE: This study aimed to assess oral health behaviors, access to dental care, and sources of oral health information among migrants residing in Italy and to examine their association with the presence of untreated dental caries. METHODS: A cross-sectional study was conducted among 427 migrants living in Italy. Data were collected using a structured online questio

Comparing costs and quality of care at retail clinics with that of other medical settings for 3 common illnesses.

BACKGROUND: Retail clinics are an increasingly popular source for medical care. Concerns have been raised about the effect of these clinics on the cost, quality, and delivery of preventive care. OBJECTIVE: To compare the care received at retail clinics for 3 acute conditions with that received at other care settings. DESIGN: Claims data from 2005 and 2006 from the health plan were aggregated into care episodes (units that included initial and follow-up visits, pharmaceuticals, and ancillary tests). After 2100 episodes (700 each) were identified in which otitis media, pharyngitis, and urinary

Undocumented migrants lack access to pregnancy care and prevention.

BACKGROUND: Illegal migration is an increasing problem worldwide and the so-called undocumented migrants encounter major problems in access to prevention and health care. The objective of the study was to compare the use of preventive measures and pregnancy care of undocumented pregnant migrants with those of women from the general population of Geneva, Switzerland. METHODS: Prospective cohort study including pregnant undocumented migrants presenting to the University hospital from February 2005 to October 2006. The control group consisted of a systematic sample of pregnant women with legal r

Community co-design of contextually relevant rheumatic heart disease primary prevention.

INTRODUCTION: Primary prevention of rheumatic heart disease (RHD) is aimed at timely diagnosis and treatment of superficial Group A Streptococcal infections, most commonly Strep sore throat. However, uptake and delivery of primary prevention for RHD-endemic regions has room for improvement. Community co-design is an important aspect of intervention development to increase sustainability and reach. METHODS: Group-level assessment (GLA) sessions were used to understand the community's priorities and insights around sore throat. Personas, or user archetypes, were then developed to capture the be

Optimising passive surveillance of a neglected tropical disease in the era of elimination: A modelling study.

BACKGROUND: Surveillance is an essential component of global programs to eliminate infectious diseases and avert epidemics of (re-)emerging diseases. As the numbers of cases decline, costs of treatment and control diminish but those for surveillance remain high even after the 'last' case. Reducing surveillance may risk missing persistent or (re-)emerging foci of disease. Here, we use a simulation-based approach to determine the minimal number of passive surveillance sites required to ensure maximum coverage of a population at-risk (PAR) of an infectious disease. METHODOLOGY AND PRINCIPAL FIND

Child Sexual Abuse: The Need for a Perpetration Prevention Focus.

In the evolution of efforts to reduce child sexual abuse (CSA) rates in the United States, there has been a gradual shift to add preventive measures to after-the-fact interventions (i.e., interventions and policies implemented after the abuse has already happened, such as services and treatment for victims and punishment, treatment, and management of offenders). Prevention of CSA perpetration, however, is often missing from these efforts, despite decades of experts calling for broader prevention solutions. The current paper describes the scope of the problem of CSA, highlights problems with an

The difference transport makes to child mortality and preventive healthcare efforts: Riders for Health.

The 20th and 21st century witnessed the development of many sophisticated vaccinations and other key preventive health interventions, and yet child mortality in sub-Saharan Africa remains unacceptably high. One of the barriers identified to reducing child mortality in rural regions of Africa is distance and lack of transportation. In order to address this, a growing charitable organisation, Riders for Health (http://www.riders.org) has developed a reliable and cost-effective system for managing vehicles used in all types of healthcare delivery. The system intends to ensure that the delivery of

Covid-19 pandemic and clinical activity stop: oral health study in a group of children.

AIM: Public and private health services, which provide both preventive and health promotion interventions, were forced to suddenly stop their activities to limit the risk of infections during the pandemic emergency. Oral health administration, including that of children, was affected by these planned medical service closures, from both therapeutic and preventive perspectives. This study aims to analyse the consequences, at the oral cavity level, of failures to treat patients of childhood age, considering the impact of carious pathology on quality of life and incorrect eating and oral hygiene h

Elimination of blinding trachoma by 2020: a call for rapid scale-up and strengthening programmes.

Elimination of blinding trachoma is targeted for the year 2020, making scale-up extremely urgent. Preferred practices have been developed for mass drug administration and trichiasis surgery to assist new countries and districts. However, these need to be utilised on a broader scale to ensure quality output of programmes and the highest coverage possible of their implementation. Although in recent years there has been a significant increase in programmatic funding, there are still gaps. In addition, continued insecurity in several regions and outbreaks that require refocusing of staff threaten

Disease prevention: saving lives or reducing health care costs?

BACKGROUND: Disease prevention has been claimed to reduce health care costs. However, preventing lethal diseases increases life expectancy and, thereby, indirectly increases the demand for health care. Previous studies have argued that on balance preventing diseases that reduce longevity increases health care costs while preventing non-fatal diseases could lead to health care savings. The objective of this research is to investigate if disease prevention could result in both increased longevity and lower lifetime health care costs. METHODS: Mortality rates for Netherlands in 2009 were used to

Harm reduction actions as a health care construction process at a public health center in Rosario, Argentina.

Even faced with serious numbers of human immunodeficiency virus (HIV) and other infections among injection drug users (IDUs), Argentina lacks large-scale harm reduction programs at national, state, and local levels. We describe the strong relationship of IDUs with the HIV/acquired immunodeficiency syndrome epidemic in Rosario. We discuss a 2-year-long ongoing experience of drug-related harm reduction actions developed at a public health center, which began as a response to a spontaneous demand for needles and syringes and became a permanent and sustainable set of health actions for a group of

[The long road for psychiatric care in prisons].

From the 19th century to the present day, the history of psychiatry in prisons has evolved considerably. In parallel with successive laws, codes and articles, psychiatry has gained in structure. From the "medical prison", mental health consultations in every detention centre, the regional medico-psychological services, to today's specially equipped hospital units (UHSA), prisoners receive both preventive care as well as curative treatment.

Use of General Health Examination and Cancer Screening among People with Disability Who Need Support from Others: Analysis of the 2016 Comprehensive Survey of Living Conditions in Japan.

Research on preventive healthcare services among people with disability in Japan is scarce. This study aimed to (1) examine the relationship between disability and the use of general health examination (GHE) and cancer screening (lung, gastric, colorectal, breast and cervical cancer) and (2) explore the reasons for not using GHE. This cross-sectional study used secondary data from individuals aged 20-74 years (n = 15,294) from the Comprehensive Survey of Living Conditions of 2016. Binomial logistic regression analysis was conducted to examine the relationship between disability and non-partici
