Isolated Distal Radius Fracture Reductions in Adult Emergency Department Patients in a Large Healthcare System.

INTRODUCTION: Distal radius fractures account for up to 18% of fractures in older adults and up to 20% of all fractures treated in the emergency department (ED). These fractures often require reduction and immobilization, with different modalities to provide analgesia. Our objective in this study was to summarize the management for distal radius fracture reductions in the real world of community and academic EDs. METHODS: We identified adult visits for isolated distal radius fractures over a four-year period across three academic and 18 community hospital EDs from more than 490,000 per annum

Neonatal painful stimuli: skin conductance algesimeter index to measure efficacy 24% of sucrose oral solution.

Pain management is one of the main challenges in addressing the improved care of hospitalized newborns. The administration of oral sucrose with and without non-nutritive suction has been proposed as a nonpharmacological intervention to relieve procedural pain in newborns. The effects have not yet been well characterized. The aim of this study is to investigate, using skin conductance algesimeter (SCA) pain monitor index, the effects of 24% sucrose solution on pain perception during capillary and arterial blood sampling. It is a prospective, randomized controlled study: sucrose versus placebo. …

Sedoanalgesia Versus Infraclavicular Block for Closed Reduction of Pediatric Forearm Fracture in Emergency Department: Prospective Randomized Study.

OBJECTIVE: Procedural sedoanalgesia is commonly used in pediatric patients in the emergency department (ED) for interventional procedures, diagnosis, and treatment. However, this method causes serious systemic complications, such as respiratory and cardiac depression. To minimize these complications, ultrasound-guided regional anesthesia methods have been used in recent years. We aimed to compare the use of procedural sedoanalgesia (PSA) and infraclavicular block (ICB) in the pain management of pediatric patients who underwent closed reductions of forearm fractures. MATERIALS AND METHODS: Thi

Precision Meets Pain Relief: Image-guided Nerve Blocks in Interventional Radiology.

Nerve blocks are a valuable opioid-sparing analgesic technique that play a critical role in multimodal pain management. As interventional radiology continues to expand its scope, proficiency in nerve block techniques has become increasingly relevant. Unlike general anesthesia, nerve blocks provide targeted pain relief, facilitating procedural success while minimizing systemic opioid use and associated complications as well as decreasing health care costs. The authors provide an in-depth discussion of nerve block mechanisms, indications, procedural techniques, imaging guidance, and safety consi

Diagnostic evidence in suspected discogenic low back pain: a pain-source attribution perspective.

Discogenic low back pain (DLBP) remains difficult to identify clinically because degenerative disc changes are common and symptom patterns overlap with other lumbar or pelvic pain sources. This mini review summarizes evidence relevant to suspected DLBP from a pain-source attribution perspective, focusing on clinical features, disc and endplate magnetic resonance imaging (MRI) findings, assessment of other pain sources, provocative discography, and diagnostic blocks. Clinical features such as axial low back pain, sitting or flexion intolerance, non-radicular referral, and centralization may inc

Patient-Controlled Sedation in Port Implantation (PACSPI 2)-A Randomised Clinical Trial.

Optimising pain management during subcutaneous venous port (SVP) implantation is essential for patient-centred cancer care. However, evidence-based approaches to minimise intraoperative pain remain underexplored. This trial evaluated the clinical effectiveness and safety of the propofol-alfentanil patient-controlled sedation (PCS) technique as an adjunct to local anaesthesia (LA) for pain reduction during SVP implantation. Adult cancer patients at two Swedish anaesthesia departments received either LA + PCS or LA alone for SVP implantation. The primary outcome was the maximum intraoperative pa
