PURPOSE: To assess skeletal stability after mandibular setback surgery with and without an intermaxillary fixation (IMF) screw.
PATIENTS AND METHODS: The subjects were 40 patients with mandibular prognathism. The subjects underwent sagittal split ramus osteotomy with titanium plate fixation and were divided into 2 groups, 1 with and 1 without an IMF screw. At the Philipps-University Marburg 14 patients with obstructive sleep apnoea syndrome (OSAS) have so far been successfully treated by surgical maxillo-mandibular advancement. These results clearly indicate that surgical treatment and cure of OSAS by maxillo-mandibular osteotomy is possible in case of an appropriate indication.

The transjugal osteosynthesis technique of the Obwegeser-Dalpont type is used for treatment by sagittal osteotomy of ascending rami. The material used is a direct compression mini-screw derived from that described by Michelet-Champy. Principles underlying all types of osteosynthesis (rigidity, solidity, miniaturization, according to the authors) are discussed and advantages and inconveniences of their material compared with those of other existing procedures. On the basis of 35 maxillary osteotomies of the Le Fort I and III types, the authors discuss the main problems connected with treatment of maxillary hypoplasia and midfacial retrusion. Special attention is paid to cases that demand coordinated treatment including skeletal as well as soft tissue repair and reconstruction. Thanks to combined treatment in all cases, including 7 patients with partial backward displacement of the osteotomized fragments whose occlusion could not be entirely corrected, striking or highly satisfactory improvement of appearance was achieved.
