BACKGROUND AND AIMS: Although prostatitis can contribute to the lack of prostate specific antigen (tPSA) specificity, there is disagreement concerning the effect of histological inflammation on free PSA (fPSA). We investigated the correlation between aggressiveness of histological inflammation and tPSA, fPSA and f/tPSA in patients without detectable prostate cancer (PC).
METHODS: The study included 106 patients with tPSA <10 ng/mL, without clinical prostatitis and with biopsy negative for PC. The inflammation in prostate biopsies was scored for aggressiveness using the four-point scale report

PURPOSE: Percent free prostate specific antigen (PSA) is useful to select patients for prostate biopsy with total PSA 4 to 10 ng./ml. However, 20% of men with PSA between 2.6 and 4 ng./ml. harbor significant prostate cancer and percent free PSA has been suggested to aid in the decision to biopsy in this total PSA range as well. Concerns exist that the number of biopsies needed to detect 1 cancer in this range may be inappropriately high. In a prospective referral population we evaluated sensitivity and specificity of various percent free PSA cutoffs and determined the biopsy-per-cancer ratio i

OBJECTIVE: To investigate the correlation of the levels of interleukin-8 (IL-8) and IL-6 in the prostatic fluid with serum levels of serum prostate-specific antigen (PSA) in patients with benign prostatic hyperplasia (BPH) complicated by prostatitis.
METHODS: A series of 211 patients undergoing surgery of BPH were divided into BPH group (n=75) and BPH with prostatitis group (n=136) according to the white blood cell count in the prostatic fluid. The clinical and laboratory findings were compared between the two groups, and stepwise regression analysis was used to assess the association of IL-8…

BACKGROUND: Prostate specific antigen (PSA) testing is widely used for detection and monitoring of prostate disease. Age-specific thresholds are applied to improve clinical interpretation of PSA results. However, the demographic and clinical characteristics associated with elevated PSA in large population-based cohorts remain incompletely understood.
METHODS: A cohort of 5325 men underwent PSA testing applying NICE (NG12) age-specific thresholds. Participants were then classified as having normal or elevated PSA concentrations. Demographic characteristics, BMI, self-reported prostate disease,

BACKGROUND AND OBJECTIVE: Prostate-specific antigen (PSA) density (PSAD) improves risk stratification for clinically significant prostate cancer (csPCa) but depends on prostate volume estimation. Transrectal ultrasound is used widely, whereas transabdominal ultrasound (TAUS) is underexplored due to accuracy concerns. We compared PSAD derived from manual magnetic resonance imaging (MRI) segmentation, MRI ellipsoid, and TAUS ellipsoid.
METHODS: In a single-centre retrospective cohort, men who underwent biopsy or prostatectomy with histopathological confirmation were included. (1) Manual MRI seg…

BACKGROUND: In ARAMIS, darolutamide statistically significantly prolonged metastasis-free survival by 2 years and reduced the risk of death by 31% in patients with nonmetastatic castration-resistant prostate cancer (nmCRPC). We report post hoc analyses of ARAMIS evaluating patterns of disease progression overall and by prostate-specific antigen (PSA) response.
METHODS: Patients were randomized 2:1 to darolutamide (n = 955) or placebo (n = 554), with androgen-deprivation therapy (ADT). Progression included metastases on conventional imaging, PSA progression based on Prostate Cancer Working Gro…

Prostate-specific antigen (PSA) is a cornerstone of screening for prostate cancer (PC); however, serum PSA levels may remain deceptively low in certain high-grade, aggressive subtypes. Aggressive-variant prostate cancer (AVPC) is a clinical entity characterized by rapid progression, bulky lymphadenopathy, and visceral metastases despite low or non-correlative PSA levels, often requiring treatment approaches beyond conventional therapies. Here, we report the case of a 69-year-old Japanese man with multiple comorbidities, including diabetes, hyperlipidemia, hypertension, angina pectoris, and low…

INTRODUCTION: MRI as an initial test for elevated PSA increases the detection of clinically significant prostate cancer while reducing overdiagnosis. Despite its proven benefit, access to MRI in rural areas is limited. Previous studies on utilization of prostate MRI use cancer registries and therefore have limited ability to assess MRI use in the prediagnostic setting. We assess rural/urban disparities in MRI use among Medicare beneficiaries with elevated PSA.
METHODS: Our sample included Medicare beneficiaries with elevated PSA. The outcome variable was receipt of prostate MRI in the prediag

OBJECTIVE: To examine the role of Prostate Health Index (PHI) in detecting clinically significant cancer among biopsy-naïve men with a prostate specific antigen (PSA) between 10-20 ng/mL.
METHODS: We studied biopsy-naïve men who underwent PHI testing followed by prostate biopsy after a screening PSA of 10-20 ng/mL. We performed logistic regression analysis to determine associations between clinicopathologic characteristics and the outcome of interest, which was the presence of grade group (GG) ≥2 at biopsy. We examined the combined effect of PHI with MRI findings in cancer detection.
RESULTS

OBJECTIVE: PSMA PET/CT has demonstrated superior sensitivity over conventional imaging in the detection of local and distant recurrence in biochemically relapsed (BCR) prostate cancer. We prospectively investigated the management impact of 68 Ga-PSMA PET/CT imaging in men with BCR, with the aim of identifying baseline clinicopathological predictors for management change.
PATIENTS AND METHODS: Men with BCR who met eligibility criteria underwent 68 Ga-PSMA-11 PET/CT at Monash Health (Melbourne, Australia). Intended management plans were prospectively documented before and after 68 Ga-PSMA PET/C

Metastatic deposit in the inguinal canal is an uncommon finding. Few tumors such as pancreatic cancer, rectal cancer, and ovarian cancer have been described previously with metastatic inguinal canal deposit. We present here a case of prostate cancer with an uncommon inguinal canal deposit that was detected on Ga-prostate-specific membrane antigen PET/CT.

The aim of the present study was to construct a dendritic cell (DC) vaccine transfected with a prostate-specific membrane antigen (PSMA) recombinant adenovirus and to observe the ability of the recombinant DCs in eliciting a PSMA-directed T‑cell response to prostate cancer (PCa) in vitro. Replication‑defective adenoviral vectors, were constructed using the Adxsi system. The virus titer was measured by TCID50 assay, and the expression of the PSMA gene was identified by polymerase chain reaction (PCR) generation of peripheral blood mononuclear cell (PBMC) derived DCs in vitro. In addition, a PE‑

PURPOSE: We are often confronted with patients in the "gray zone" (prostate-specific antigen [PSA]<10 ng/mL) whose biopsies reveal no malignancy but only inflammation. We investigated the relationship between histological inflammation and total PSA (tPSA), free PSA (fPSA), and percentage of free PSA (f/tPSA) levels in patients without prostate cancer (PC).
MATERIALS AND METHODS: We studied 106 men with tPSA<10 ng/mL who had undergone biopsy that was negative for PC and who had no clinical prostatitis. Inflammation observed at biopsies was scored for inflammation type in each biopsy core by us…

PURPOSE: There is continuing controversy over the most appropriate treatment for screen detected and clinically localized prostate cancer, and increasing interest in monitoring such men initially with radical treatment targeted at cancers showing signs of progressive potential but while they are still curable. Current evidence on monitoring protocols and biomarkers used to predict disease progression was systematically reviewed.
MATERIALS AND METHODS: The MEDLINE and Excerpta Medica (EMBASE) bibliographic databases were searched from 1988 to October 2004, supplemented by manual searches of re

BACKGROUND: Prostate specific antigen doubling time (PSAdt) is a dynamic model of prostate tumor biology. It predicts aggressive disease and subsequent clinical recurrence after radical treatment. However, as yet there is only limited evidence for its validity in the watchful waiting population.
METHODS: One hundred and thirteen previously untreated patients with adenocarcinoma of the prostate who were referred to the British Columbia Cancer Agency for a management opinion subsequently were placed into a prospective watchful waiting program. The reasons for watchful waiting, previous medical

Transrectal, ultrasound guided needle biopsies were performed in 27 men greater than 18 months (mean 5.2 years) after radiation therapy for prostate cancer. Of the patients 18 (67 per cent) originally had localized (stage A or B), 6 had stage C and 3 had stage D1 prostate cancer. In addition to biopsies directed at any hypoechoic regions of the prostate, multiple systematic biopsies were performed from the apex to the base bilaterally and the seminal vesicles also were biopsied regardless of appearance. Of 27 patients 25 (93 per cent) had post-irradiation biopsies positive for cancer, includin
