Pediatric distal femur fixation by proximal humeral plate.

Distal femoral metaphyseal fractures are common injuries in children. Multiple treatment options have been described for this type of injury. For older children with distal metaphyseal fracture, there is still no optimal method of fixation. We propose that the commonly used proximal humeral plate can provide good method of fixation for this fracture in adolescents. Two children (12 and 14 years old) with distal metaphyseal femoral fracture were treated with proximal humeral plate. We describe the surgical technique and postoperative management. The two children healed with good alignment and f

Dual mobility total hip arthroplasty vs. bipolar hemiarthroplasty in treating patients with displaced femoral neck fractures: a systematic review and meta-analysis.

BACKGROUND: Total hip arthroplasty (THA) has been reported with superior outcomes in hip pain, reoperation rate, function, but higher rate of dislocation compared with bipolar hemiarthroplasty (BHA) for treatment of displaced femoral neck fractures (DFNFs). A dual mobility cup (DMC) has been designed to reduce the rates of dislocation and re-operation. However, whether DMC-THA achieves better clinical outcomes than BHA remains ambiguous. Therefore, we have conducted this meta-analysis to investigate the clinical outcomes of DMC-THA vs. BHA in the treatment of DFNFs. METHOD: We identified stud

Traumatic anterior hip dislocation with associated bilateral femoral fractures in a child: a case report and review of the literature.

Traumatic anterior hip dislocation is rare, because the hip joint is a highly stable joint. It is extremely rare for the anterior hip dislocation with combined bilateral femoral fracture in children. We present a case of 7-year-old boy with traumatic anterior hip dislocation with associated bilateral femoral fractures. Radiographic examination showed the right femoral head was dislocated anteroinferiorly. The ipsilateral femoral shaft showed a transverse femoral shaft fracture and proximal and distal femoral bifocal fractures of the contralateral femur. The dislocation of the right hip was red

Uncemented femoral stem design influences the occurrence rate of postoperative fractures after primary hip arthroplasty: a comparison of the Image and Profile stems.

Two consecutive series of hip arthroplasties with closely similar anatomic uncemented femoral implants [207 Profile (DePuy) and 171 Image (Smith&Nephew) stems] were compared with regard to postoperative femoral fractures. All arthroplasties were performed by senior staff surgeons, mainly on patients below 65 years of age. In the Image group the occurrence rate of postoperative periprosthetic femoral fractures was higher (9.36%) compared to the Profile group (2.99%) and fractures occurred earlier (1.69 y vs. 8.84 y). The bulkier proximal part, the thinner cylindrical distal two thirds and the l

Late Periprosthetic Femoral Stress Fracture Associated with Extra-articular Deformity in a Patient with Fibrous Dysplasia: A Case Report.

CASE: In the setting of a total hip arthroplasty performed in a patient with a proximal femoral deformity, atraumatic periprosthetic femoral stress fracture may arise as a complication. We report a rare case of a late periprosthetic femoral stress fracture around a cemented stem in a patient with a history of fibrous dysplasia of the proximal femur. After a 10-year uneventful period, the patient complained about a subtle, subacute pain in his left thigh induced by exercise but not with daily axial load. Diagnosis of a nondisplaced, incomplete (i.e., only compromising the lateral femoral cortex

Survival analysis of patients with femoral metastases.

BACKGROUND: Previous studies reported on surgical indications for patients with femoral metastases. However, few studies analyzed the spectrum of femoral metastatic presentation. We performed this study to evaluate the survival of patients with femoral metastases, and clarify the treatment of femoral impending and actual pathological fractures. MATERIALS AND METHODS: We retrospectively studied 110 patients with femoral metastases from various cancers treated with nailing or resection and megaprosthetic reconstruction from 1995 to 2010. The mean follow-up was 18 months. Survival was analyzed w

Ipsilateral femoral neck, intertrochanteric and acetabular fractures with posterior dislocation of the hip: A case report and literature review.

INTRODUCTION: Traumatic hip dislocation with ipsilateral femoral neck fracture, intertrochanteric fracture and acetabular fracture is a rare and serious injury that occurs mostly in young patients and after high-energy trauma. The treatment of these injuries is a major challenge for orthopedic surgeons; it can have devastating consequences if not treated properly, including avascular necrosis of the femoral head and traumatic osteoarthritis of the hip. In previous case reports only internal fixation of femoral neck fracture was performed without revascularisation and there was a lack of long-t

The use of the supracondylar nail in the management of femoral fractures in the presence of other femoral implants in the very elderly.

In a retrospective study we review the use of the supracondylar nail for the treatment of distal femoral fractures above existing knee prostheses or below upper femoral implants in a very elderly population of mean age 85. Over a 2 year period we have used the Smith and Nephew supracondylar nail to stabilise seven distal third femoral fractures either distal to a femoral implant (six cases) or proximal to a total knee replacement. One patient died soon after surgery, one patient had an extension of the fracture proximal to the nail but the others made an excellent functional recovery with earl

Closed displaced femur fractures in children with nonambulatory cerebral palsy.

Femoral fractures in children withcerebral palsy (CP) represent a frequent medical problem, and treatment represents a challenge. The purpose of this study was to review the closed displaced femoral fractures in our population of nonambulatory children with CP to compare the results of nonoperative and operative treatment modalities to improve the care of these children. From 2006 to 2020, children with nonambulatory CP were selected with inclusion criteria of displaced femoral fracture and were divided into nonoperative and operative groups. Forty-four children met the inclusion criteria. The

Long-term use of nerve block catheters in paediatric patients with cancer related pathologic fractures.

We report three cases of children with osteosarcoma and pathologic fractures treated with long-term continuous nerve blocks for preoperative pain control. One patient with a left distal femoral diaphysis fracture had a femoral continuous nerve block catheter for 41 days without complications. Another with a fractured left proximal femoral shaft had three femoral continuous nerve block catheters for 33, 26 and 22 days respectively. The third patient, whose right proximal humerus was fractured, had a brachial plexus continuous nerve block catheter for 36 days without complication. In our experie

Weight-bearing impact after surgically treated proximal femur fracture in patients aged 65 years and older: systematic review.

The incidence of proximal femur fractures is increasing among older adults. Although surgical treatment is often required to restore pre-fracture functional status, the optimal postoperative weight-bearing strategy remains controversial. This systematic review aimed to evaluate the impact of early weight-bearing after surgically treated proximal femur fractures in patients aged 65 years and older. A systematic search of Medline (via Ovid and PubMed), Embase, Scopus, Google Scholar, and the grey literature was conducted in accordance with the PRISMA 2020 guidelines for studies published between

In-hospital mortality among patients with proximal femur fractures infected with COVID-19.

The COVID-19 pandemic has significantly reduced the number of orthopedic surgeries world- wide. The main reason for this was an introduction of numerous restrictions in the field of mobility, which resulted in reduction of circumstances predisposing to injuries. This had a positive impact on the overall readiness of the health system to fight the pandemic, thanks to saving human and material resources that could be successfully allocated in order to fight a new, previously unknown threat. On the other hand, such aimed administration of the system was not indifferent in terms of care quality di

[Hip fractures].

Hip fractures are ranked among the frequent injuries. These fractures have been often coupled with high energy trauma in children and in patients with normal bone structure, low energy trauma and osteoporotic fracture (fragility fracture) is typical in elder patients. Hip fractures are divided into five groups: femoral head fracture, femoral neck fracture, pertrochanteric, intertrochateric and subtrochanteric fracture. Surgical treatment is indicated in all patients unless contraindications are present. Long bed rest has been accompanied by a high risk of development of thromboembolic disease,

The choice of locking plate in the treatment of peri-implant femoral fracture eight years after trans-trochanteric rotational osteotomy: A case report.

INTRODUCTION: Transtrochanteric anterior rotational osteotomy of the femoral head (TRO) was developed as a joint preserving surgery for osteonecrosis of the femoral head. To the best of our knowledge, peri-implant fractures after femoral osteotomy have rarely been reported. PRESENTATION OF CASE: We report a 58-year-old female who suffered a peri-implant femoral shaft fracture following a fall from a stepladder eight years after TRO. Fracture union was achieved six months after a preferred proximal femoral locking plate. DISCUSSION: The entry point of the ante-grade femoral nail would have be

Concomitant ipsilateral femoral neck and shaft fractures--analysis of cases.

Concomitant ipsilateral shaft and femoral neck or pertrochanteric fractures are uncommon. They result from high-energy traumas and the mechanism of injury is usually complex. The proximal fracture is often not displaced and may be hardly visible on x-rays. Overlooking the proximal fracture may result in therapeutic problems. The best method of treatment is by closed stabilization of both fractures with a reconstruction nail. In our opinion, concomitant neck and femoral shaft fractures demand particular alertness in x-ray analysis, precision in fracture stabilization and usually longer patient
