What Does Health Justice Look Like for People Returning from Incarceration?

Access to health care is a constitutional right in the United States correctional system, and many incarcerated adults are newly diagnosed with chronic diseases in prison. Despite this right, the quality of correctional health care is variable, largely unmeasured and unregulated, and characterized by patients' widespread distrust of a health system that is intimately tied to a punitive criminal justice system. Upon release, discontinuity of care is the norm, and when continuity is established, it is often hindered by distrust, discrimination, poor communication, and racism in the health system…

"Missed, dissed, and pissed": making meaning of neighborhood risk, fear and anger management in urban black youth.

The risk factor, stress engagement, and coping experiences of African American youth are not well understood. Given the stressors of racism, hopeless perceptions of urban youth, and violence experience and exposure, anger experience and expression are reasonable resilient and risky reactions to this atmosphere of hostility. This study analyzed the impact upon the anger management of adolescents when calamity fears, neighborhood social capital, and kinship social support are known. The findings suggest that when the calamity fears of youth are high, their anger experience and expression is mini…

Allostatic Load Mediates Associations Between Race and Ethnicity and Hypertensive Disorders of Pregnancy.

OBJECTIVE: To evaluate whether chronic stress exposure, measured by allostatic load (a biological measure of chronic stress embodiment, including stressors exacerbated by structural inequities [eg, structural racism]) and patient-reported perceived stress in the first trimester of pregnancy, mediates the association between self-identified race and ethnicity and hypertensive disorders of pregnancy (HDP). METHODS: This was a secondary analysis of data from nuMoM2b (Nulliparous Pregnancy Outcomes Study: Monitoring Mothers-to-Be), a large prospective cohort study. We evaluated self-identified ra…

Trauma informed interventions: A systematic review.

BACKGROUND: Health inequities remain a public health concern. Chronic adversity such as discrimination or racism as trauma may perpetuate health inequities in marginalized populations. There is a growing body of the literature on trauma informed and culturally competent care as essential elements of promoting health equity, yet no prior review has systematically addressed trauma informed interventions. The purpose of this study was to appraise the types, setting, scope, and delivery of trauma informed interventions and associated outcomes. METHODS: We performed database searches- PubMed, Emba…

The effectiveness of a Housing First adaptation for ethnic minority groups: findings of a pragmatic randomized controlled trial.

BACKGROUND: Little is known about the effectiveness of Housing First (HF) among ethnic minority groups, despite its growing popularity for homeless adults experiencing mental illness. This randomized controlled trial tests the effectiveness of a HF program using rent supplements and intensive case management, enhanced by anti-racism and anti-oppression practices for homeless adults with mental illness from diverse ethnic minority backgrounds. METHODS: This unblinded pragmatic field trial was carried out in community settings in Toronto, Canada. Participants were 237 adults from ethnic minorit…

Physiological mechanisms mediating socio-environmental influences on pregnancy outcomes in Black people.

Black women and their infants experience higher mortality rates than their White counterparts. While systemic racism, socioeconomic disadvantage, and environmental stressors are recognized contributors, physiological pathways that mediate their effects on health disparities remain poorly understood. This review explores mechanisms through which stress-related epigenetic processes may contribute to more adverse pregnancy outcomes in Black women. Increased uteroplacental vascular resistance, oxidative stress, and inflammation are established mechanisms associated with leading causes of maternal …

The Spiral of Violence Experienced by Immigrant Domestic Workers: A Qualitative Approach.

Many immigrant women engaged in domestic work encounter precarious employment conditions characterized by uncertain contracts, often experiencing marginalization based on their racial identities. Through qualitative methods, including 23 in-depth interviews and interpretive analysis, this study aims to explore the structural and circumstantial violence embedded within the narratives of migrant women involved in caregiving and domestic work. These women report a spiral of structural violence influenced by classism, racism, and sexism perpetuated through the use of fear, abuse, and economic coer…

'Just be strong and keep going': the influence of Superwoman Schema on Black women's perceived expectations of coping with sexual pain.

Superwoman Schema, a conceptual framework that reflects Black women's ability to overcome gendered racism and stress, affects the way Black women choose to cope with health-related issues. The purpose of this study was to investigate how Black women perceive they should cope with sexual pain using the Superwoman Schema as an analytic and interpretative guide. Data were derived from participants who completed an individual interview on sexual pain and pleasure. Deductive thematic analysis was conducted. Results indicated whereas some Black women endorsed all five components of Superwoman Schema…

Protecting the future child: Foetal alcohol spectrum disorder, easy rescue and the regulation of maternal behaviour.

This paper argues that social contexts of inequality are crucial to understanding the ethics of gestational harm and responsibility. Recent debates on gestational harm have largely ignored the social context of gestators, including contexts of inequality and injustice. This can reinforce existing social injustices arising from colonialism, socio-economic inequality and racism, for example, through increased regulation of maternal behaviour. To demonstrate this, I focus on the related notions of the 'future child' and an obligation of easy rescue, which have been used to discuss the ethics of g…

The development and pandemic-related adaptation of a resilience program for native middle-school youth: A case study.

As of 2017, American Indian/Alaska Natives (AI/AN) had the highest prevalence of illicit drug use of any ethnic group in the United States, with 17.6% of the population aged 12 and older reporting using illicit drugs in the last month. Studies have shown the positive correlation between a history of trauma and substance use disorder. In fact, the majority of youth in treatment for substance misuse reported a history of trauma. Intergenerational trauma, systematic discrimination, and displacement are downstream effects of colonization, and experiences of racism often define the life experiences…

Improving Access to Legal Gender Affirmation for Transgender Women Involved in the Criminal-Legal System.

Transgender women of color experience interlocking systems of oppression rooted in racism and transphobia, which fuel economic vulnerability and overrepresentation in the criminal-legal system. Legal gender affirmation, which refers to changing one's name and gender marker on official documents, has the potential to mitigate these issues by improving access to employment, housing, education, health care, and social services. These services are particularly important for transgender women of color with criminal records, a history of incarceration, or other legal infractions; however, 23 states …

Understanding and effectively addressing breast cancer in African American women: Unpacking the social context.

Black women have a higher incidence of breast cancer before the age of 40 years, more severe disease at all ages, and an elevated mortality risk in comparison with white women. There is limited understanding of the contribution of social factors to these patterns. Elucidating the role of the social determinants of health in breast cancer disparities requires greater attention to how risk factors for breast cancer unfold over the lifecourse and to the complex ways in which socioeconomic status and racism shape exposure to psychosocial, physical, chemical, and other individual and community-leve…
