Prevalence of sleep breathing disorders in outpatients on a heart transplant waiting list.

Sleep related periodic breathing with recurrent episodes of apnea and hypopnea is known to occur in patients with heart failure. We investigated the prevalence of sleep related breathing disorders (SRBD) in 14 outpatients on a heart transplant waiting list. All were younger than 60 years and had severe stable heart failure. Three patients (21%) exhibited 10 or more apneas and hypopneas per hour of sleep; these apneas and hypopneas were predominantly of the central type and occurred during Cheyne-Stokes respiration. There were no statistically significant differences between the apneic and non-

[Use of EEG response following intravenous diazepam administration in the etiologic diagnosis of brain lesions].

The authors take into consideration the reactivity of the E.E. Graphic focal anomalies to the i.v. infusion of 10 mg Diazepam. The cases includ 28 patients (15 with cerebral neoplasia, 8 with severe vascular accident, 5 with light vascular accident); among these, 19 showed typical E.E. Graphic focuses after administration of Diapezam, like those previously described by Weber and other Authors. Subjects with severe cerebral lesions and in the third age, after i.v. administration of 10 mg Diazepam showed vegetative disorders (hypotension, tachycardia or bradycardia, apnoea, periodic respiration)

Occurrence of breathing disorders during CPAP administration in obstructive sleep apnoea syndrome.

The spectrum of breathing events during sleep in patients with obstructive sleep apnoea syndrome (OSAS) after the abolition of obstructive apnoeas has been extensively studied in tracheostomized patients, but has received much less attention in patients submitted to continuous positive airway pressure (CPAP). We analysed the breathing pattern during sleep in forty patients while CPAP was administered. A regular breathing pattern throughout the sleep study was observed in 15 patients. In the remaining 25 subjects, one or more of the following events was observed: central apnoeas, hypopnoeas, pe

[Peculiarities of disorders in physical development of children consuming drinking water with increased nitrate content].

Peculiarities of physical development disorders in 334 children were examined in the course of comparative clinical-laboratory research; all these children consumed drinking water with various nitrates content (11.2±1.1 mg/l and up to 3.14 MPC, i.e. 141 mg/l), in a dose equal up to 9.0 mg/kg of body weight (RfD 1.6 mg/kg). A focus group consisted of 158 children aged from 3 to 10, a control group was made up of 176 children of the same age. Nitrates content in urine taken from children of focus group (49.22±11.69 mg/l) was 2.4 fold higher than the same parameter in control group (20.2±3.13 mg/

Bi-Allelic UQCRFS1 Variants Are Associated with Mitochondrial Complex III Deficiency, Cardiomyopathy, and Alopecia Totalis.

Isolated complex III (CIII) deficiencies are among the least frequently diagnosed mitochondrial disorders. Clinical symptoms range from isolated myopathy to severe multi-systemic disorders with early death and disability. To date, we know of pathogenic variants in genes encoding five out of 10 subunits and five out of 13 assembly factors of CIII. Here we describe rare bi-allelic variants in the gene of a catalytic subunit of CIII, UQCRFS1, which encodes the Rieske iron-sulfur protein, in two unrelated individuals. Affected children presented with low CIII activity in fibroblasts, lactic acidos

Mitochondrial Membrane Dynamics and Inherited Optic Neuropathies.

Inherited optic neuropathies are a genetically diverse group of disorders mainly characterized by visual loss and optic atrophy. Since the first recognition of Leber's hereditary optic neuropathy, several genetic defects altering primary mitochondrial respiration have been proposed to contribute to the development of syndromic and non-syndromic optic neuropathies. Moreover, the genomics and imaging revolution in the past decade has increased diagnostic efficiency and accuracy, allowing recognition of a link between mitochondrial dynamics machinery and a broad range of inherited neurodegenerati

Diagnostic approach to sleep-disordered breathing.

Respiratory disorders in sleep are highly prevalent and increasingly recognized. Among these, obstructive sleep apnea, resulting in daytime fatigue and somnolence, increased risk of workplace and traffic accidents but also psychosocial dysfunction, is most often diagnosed. As an independent risk factor for cardiovascular and metabolic disease, obstructive sleep apnea has recently attracted even more attention. Apart from continuous positive airway pressure, only a few alternative treatment options are available. Individual history is still most important for selecting patients for sleep studie

Chronic mild stress damages mitochondrial ultrastructure and function in mouse brain.

Increasing evidence implicates mitochondrial failure as a crucial factor in the pathogenesis of mental disorders, such as depression. The aim of the present study was to investigate the effects of exposure to chronic mild stress (CMS), a paradigm developed in the late 1980s as an animal model of depression, on the mitochondrial function and mitochondrial ultrastructure in the mouse brain. The results showed that the CMS regime induced depressive-like symptoms in mice characterized by reduced sucrose preference and body weight. Moreover, CMS exposure was associated with a significant increase i

The relationship between the stomatognathic system and body posture.

In recent years, many researchers have investigated the various factors that can influence body posture: mood states, anxiety, head and neck positions, oral functions (respiration, swallowing), oculomotor and visual systems, and the inner ear. Recent studies indicate a role for trigeminal afferents on body posture, but this has not yet been demonstrated conclusively. The present study aims to review the papers that have shown a relationship between the stomatognathic system and body posture. These studies suggest that tension in the stomatognathic system can contribute to impaired neural contr

Oxidative stress and mitochondrial dysfunction in neurodegeneration.

In the last 4 years much progress has been made in the understanding of mitochondrial disorders. Point-mutations, deletions and depletion of the mitochondrial genome are associated with disorders like Leber's disease, MERRF (Myoclonus Epilepsia with Ragged Red Fibers), MELAS (mitochondrial Myopathy, Encephalopathy, Lactic acidosis and Stroke-like episodes) and several others. Recently, mitochondrial dysfunctions have been also related to neurodegenerative disorders like Parkinson's disease and to aging. Since the brain depends mostly on mitochondrial energy supply, mitochondrial dysfunctions m

Panic disorder is associated with the Val308Iso polymorphism in the hypocretin receptor gene.

BACKGROUND: Orexin A and B are neuropeptides influencing, for example, arousal and respiration. Although panic disorder is characterized by both enhanced proneness for arousal and by respiratory abnormalities, the possible influence of orexin-related genes on the risk of developing this disorder has not been studied until now. METHODS: We have analyzed the Ile408Val polymorphism in the hypocretin receptor 1 (HCRTR1) gene and the Val308Iso (G1246A) polymorphism in the hypocretin receptor 2 (HCRTR2) gene in a sample of 215 panic disorder patients and 454 controls. RESULTS: Although the polymor

[Central sleep apnea in patients with chronic heart failure].

Cheyne-Stokes respiration (CSR) is a form of sleep-disordered breathing seen in approximately half of patients with chronic heart failure and low left ventricular ejection fraction. The authors describe clinical features of CSR, mortality rate, treatment variants. Effects of continuous positive airway pressure (CPAP), bi-level ventilation, adaptive servoventilation (ASV) in patients with CSR and chronic heart failure are discussed. Diuretic acetazolamide is one more therapeutic option for CSR. It improves central sleep apnea and related daytime symptoms in patients with heart failure.

Spinal cord injury in the newborn infant.

Birth injury to the spinal cord secondary to traction forces during delivery is a common but frequently undiagnosed disorder. The injury usually affects the cervicodorsal junction, with both extradural hematoma and direct cord damage at that level. Clinical findings of a paraplegic infant with abdominal breathing are sometimes obscured by secondary pneumonia and/or hypoxia. Radiologic manifestations include a bell shaped chest indicative of loss of the external muscles of respiration; spinal roentgenograms are usually normal. Myelography in neonatal spinal injury demonstrates a block in the su

The prevalence of sleep-disordered breathing and associated risk factors in patients with decompensated congestive heart failure in Mozambique.

STUDY OBJECTIVES: Sleep-disordered breathing (SDB) is common in patients with congestive heart failure and has important implications regarding symptoms and prognosis. However, the burden of SDB on those with heart failure has not been well characterized in developing countries, including Mozambique in sub-Saharan Africa. Diagnosing SDB in individuals with congestive heart failure is important because treatment of SDB may improve outcomes. METHODS: Between September 2014 and April 2017, patients hospitalized in a specialized cardiology unit in Maputo, Mozambique with decompensated congestive

Enkephalins: Endogenous Analgesics with an Emerging Role in Stress Resilience.

Psychological stress is a state of mental or emotional strain or tension that results from adverse or demanding circumstances. Chronic stress is well known to induce anxiety disorders and major depression; it is also considered a risk factor for Alzheimer's disease. Stress resilience is a positive outcome that is associated with preserved cognition and healthy aging. Resilience presents psychological and biological characteristics intrinsic to an individual conferring protection against the development of psychopathologies in the face of adversity. How can we promote or improve resilience to c

Biosynthesis of coenzyme Q in eukaryotes.

Coenzyme Q (CoQ) is a component of the electron transport chain that participates in aerobic cellular respiration to produce ATP. In addition, CoQ acts as an electron acceptor in several enzymatic reactions involving oxidation-reduction. Biosynthesis of CoQ has been investigated mainly in Escherichia coli and Saccharomyces cerevisiae, and the findings have been extended to various higher organisms, including plants and humans. Analyses in yeast have contributed greatly to current understanding of human diseases related to CoQ biosynthesis. To date, human genetic disorders related to mutations

[Indications in sleep-apnea syndrome. When and why is further assessment meaningful?].

Forms of sleep apnea syndrome: Interrupted breathing and hypoventilation during sleep lead to sleep disorders and to cardiovascular sequelae. In the common obstructive sleep apnea syndrome (OSAS) apneas are related to intermittent obstruction of the upper airways. In the rarer central sleep apnea syndrome certain cardiovascular or central nervous system disorders lead to disturbed regulation of respiration connected with periodic breathing. Signs indicating OSAS: Loud, cyclic snoring, interrupted by cessation of breathing during sleep observed by relatives and excessive daytime to diurnal slee

Relevance of the habenular complex to neuropsychiatry: a review and hypothesis.

Since the initial observation by Brown (1914) that electrical stimulation applied to the habenular efferent bundle in the chimpanzee evoked a pattern of respiration which closely resembled the act of laughter, the habenular complex has remained a mysterious structure. The anatomy of the habenular complex is well delineated (Jones, 1985) forming a major component of the dorsal diencephalic conduction system. Data derived mainly from animal experimentation over the past decade point to the fact that the habenular complex functions as an important link between the limbic forebrain and the midbrai
