We report a patient with systemic large cell non-Hodgkin lymphoma in remission who presented with the rare combination of optic neuropathy and central retinal artery occlusion. Another unusual feature of this case is the lack of enhancement in the affected region on magnetic resonance imaging only hours after the first dose of steroids. Despite prompt treatment with steroids and radiotherapy, lymphomatous infiltration of the meninges developed 2 months later and was ultimately fatal.

An unusual case of a 32-year-old woman with branch retinal artery occlusion is presented. The patient was affected by Ménière's disease and had a history of occasional migraine headaches. A thorough clinical and laboratory investigation showed a high cholesterol level. Retinal arterial obstruction is rare in young people and it is difficult to establish the precise cause. The presumed multifactorial etiologic factors are discussed.

We report a case of central artery obstruction in a 55-year old man who had non proliferative diabetic retinopathy. Three years later, examination found no diabetic retinopathy in the eye that had central artery obstruction, and more advanced non proliferative diabetic retinopathy in the fellow eye. Pathogenic mechanisms for diabetic retinopathy regression after central artery occlusion could be the optic atrophy and ischemia of the inner retina associated to the decrease in retinal blood flow.

PURPOSE/METHODS: Central retinal artery occlusion occurs infrequently in children. We treated a child with central retinal artery occlusion before a systemic disease was discovered.
RESULTS/CONCLUSION: Vision improved after paracentesis and anticoagulation. Systemic T-cell lymphoma was subsequently diagnosed. A case of a 40-year old marine with bilateral optic neuritis and a branch retinal artery occlusion after vaccination is presented. Blood investigations showed no abnormalities. Cerebrospinal fluid studies revealed a lymphocytic pleocytosis and IgG antibodies against hepatitis A and rabies. Computerized tomography and magnetic resonance imaging of the brain were negative. A diagnosis of vaccine-induced auto-immune demyelinative optic neuritis was made. The clinical picture improved after systemic corticosteroid treatment.

We present a case of acute central retinal artery obstruction in association with Herpes zoster ophthalmicus and delayed cerebral vasculopathy. Retinal vascular obstruction is rare in zoster, and its occurrence during postherpetic cerebral vasculopathy has not been reported previously to our knowledge. The syndrome of delayed cerebral vasculopathy is discussed as is its possible relationship to central retinal artery obstruction.

Endarterectomy is often the treatment of choice in carotid artery disease displaying significant stenosis of the internal carotid artery. This procedure is not without its complications, the most visually devastating being central retinal artery occlusion and secondary neovascularization. Such a case is presented here along with the interesting finding of a cholesterol irido-embolus, apparently the first citation in the ophthalmic literature to date.

We examined fundus photographs of 3,506 eyes and noted uncommon retinal vessel patterns in the temporal quadrants of the posterior fundus. Of 1,753 subjects, 20 patients (21 eyes) had unusual retinal vessel patterns. Seven eyes had an abnormally distributed branch of the inferotemporal artery; 8 eyes had an enlarged macular artery. Of these 20 patients, 1 had visual complaints associated with the unusual vascular patterns.
