There is a shortage of data concerning the long-term follow-up of patients with lumboperitoneal (LP) shunts, especially in the pediatric population. A retrospective study of 143 patients who underwent LP shunting between 1974 and 1991 was therefore performed. The mean age at the time of shunt insertion was 3.3 years (range, 18 d to 17.8 yr), and the indication for shunting was: hydrocephalus (81%), cerebrospinal fluid fistula (12%), and pseudotumor cerebri (7%). In a retrospective study of 411 children with cerebral convulsions over a period of 4 years, 160 patients with febrile seizures were found. This group consisted of 94 boys and 66 girls. The main purpose of this study was to establish the age of the first convulsive fit in each child. Seventeen dogs were diagnosed with leptospirosis on the basis of clinical findings, laboratory abnormalities, and serology. The factors involved in the development of 'summer lesion syndrome' were studied on 13 salmon farms based in the coastal waters of the west of Ireland. The primary causal factor in the development of the syndrome appeared to be overexposure to solar ultraviolet radiation. The pathology of the condition is described, the predisposing and reinforcing factors are considered and a hypothesis for the cause of the syndrome is proposed.

Lyme disease is caused by a spirochete, Borrelia burgdorferi, and represents a potential transfusion hazard. Some Borrelia burgdorferi-infected blood donors may not be disqualified by standard donor selection procedures, thus possibly transmitting the disease. In a follow-up of 14 recipients of blood products donated by such a donor, no clinical signs or serologic evidence of a transfusion-transmitted borreliosis could be demonstrated.

During the period of study of 3 years (1985-1987), 4698 (18.68%) cases of labour in teenage mothers were recorded out of a total of 25,142 deliveries in the obstetric unit. Preponderance of primigravida (76.6%) and cases from rural areas (51.3%) were recorded. Antenatal care was nil or inadequate in 48.6% cases. Eclampsia and pre-eclampsia affected teenage mothers (10.6%) were much more frequent than mothers of 20 years of age and above (5.2%). Incidence of 30% low birth weight baby, 20.1% prematurity and 16.4% perinatal mortality were recorded.

The frequency of congenital malformations in a retrospective study done on 10,000 consecutive births from January 1983 to March 1989 was 3.6%. The predominant system involved was CNS. The obstetric risk associated with congenital uterine malformations was studied in a group of 67 women who had undergone hysterosalpingography (HSG) during an eight-year period. HSG was performed to investigate primary (21% of the cases) and secondary (19%) infertility (group 1) or recurrent early fetal loss (60%) (group 2). The study group was compared with a random control group of 130 patients with HSG-proven normal uteri, matched for the presenting symptom. This paper is a summary of our observations on 105 cases of infantile spasms. The age of onset was around six months after birth, but the patients came for treatment mainly about one year after onset. Fever of unknown cause, asphyxia, birth injury, infection of the central nervous system, tuberous sclerosis, phenylketonuria and recent immunization etc. were complained. Clinically, it is characterized by head nodding, mental retardation, myoclonic jerks and various neurologic deficits. Eight hundred and two digits (592 complete amputation and 210 incomplete amputations) of 549 patients were replanted. The male to female ratio was 3:1 and the age range 1-63 years. 728 digits survived and 74 digits failed. The overall survival rate was 90.8%. The case reports of 4 pediatric patients illustrate the complex clinical scenarios in which childhood rhabdomyolysis/myoglobinuria occurs. Children ranged in age from 8-18 years. Presumed etiologies of rhabdomyolysis/myoglobinuria included Neisseria sepsis, exertion-related episodes, dialysis disequilibrium, and diabetic ketoacidosis. No child developed respiratory or renal failure. all children were discharged with normal muscle power, indicating the benign nature of this disease and the importance of aggressive management.

We reviewed 96 cases of ulcerative colitis diagnosed during the period between 1970 and 1988, with the intention of analysing the presentation, complications, evolution and diagnosis aspects. The most frequent presentation symptoms were mucoid and bloody diarrhea (91.3%) and abdominal pain (84%), isolated rectal bleeding being predominated in distal type. Paresthesia is a rare clinical finding subsequent to surgery accompanied by the administration of local anesthetics. A small patient population was identified whose clinical problem may be explained by neurotoxicity due to a local anesthetic metabolite. Reasonable questions arise from these clinical observations that would benefit from prospective studies to explain sensory loss on a biochemical basis.

The objective of this study was to determine if flexible FOB could be performed safely without premedication other than topical anesthesia. A total of 281 procedures performed during a 12-month period at a VA medical center were reviewed. In a five year period 73 cases of pemphigus vulgaris (PV) and 94 of bullous pemphigoid (BP) were studied to investigate important parameters for these diseases. From the results it was found that the incidence of PV was higher in women whereas that of BP was almost equal in both sexes. Renal images shown on 254 bone scans employing Tc 99m MDP were analyzed to evaluate the renal abnormalities. Among these cases, there were 43 cases of abnormal renal visualization. The presenting features of 236 thyrotoxic patients seen in the thyroid clinic were reviewed. 18.65% of these patterns had one or more dermatological complaints at presentation. We had 170 female patients with presenting symptoms of vaginal itching, dysuria, dyspareunia, burning and vaginal discharge. We performed 200 times vaginal microbial exploration on the said 170 patients, with the repetition of the procedure on 30 out of these 200 patients for the second time. We had a positive result for vulvovaginal candidosis on 60 occasions. These positive results for yeast infection can be related to the incidence of other microbial infections such as mycoplasma-, ureaplasma-, trichomonas-, gonococcal- and chlamydia infections.

Small-bowel enema was performed in 536 patients referred for suspected malabsorption, enteritis, abdominal pain or obstruction. Lesions were found in 54% of cases. The positivity of the investigation was particularly high in clinically well-defined cases, such as suspected obstruction (80%) and enteritis (72%). The main indications and criteria of application of the investigation are discussed.
