The value of skin prick testing in the management of patients with chronic rhinitis has still to be established. The aim of the study was to determine how accurately atopic status could be predicted from the clinical history of allergy. We carried out a prospective audit of 103 patients undergoing skin tests in three ORL departments over a three-month period. To evaluate the roles of eosinophils in allergic rhinitis, eosinophil cationic protein (ECP) levels in sera and nasal secretions were measured in 28 patients with allergic rhinitis. In patients, blood eosinophil counts and serum ECP levels were significantly higher than in controls (p < 0.05). Population surveys of asthma have shown total asthma prevalence rates of 3.8 (2-5) and 3.2 (3-4) in 1978-1979 and 1987. These figures do not fit with the general statements of an increased asthma prevalence during recent decades. The anterior ethmoidal nerve (AEN) contains parasympathetic fibres of sneeze reflex arc, it is possible to obtain improvement of clinical symptoms of vasomotor rhinitis through anterior ethmoidal neurectomy. The authors performed intranasal electrocoagulation of AEN on 30 patients with vasomotor rhinitis and followed up for nine months. The results revealed that the total effective rate was 86%, and the procedure provides a simple, effective and painless method for treating vasomotor rhinitis.

Magnet-laser therapy was used to treat vasomotor rhinitis in 90 patients. A constant magnet of 50 mT was applied to the exterior of the nose, and an infrared laser beam was applied via a light-guide to the reflexogenic zones of concha inferior. The radiation power density was 5 mW/cm2. The magnetic exposure time was 6-10 min and the laser exposure time was 3-5 min for each half of the nose. The therapeutic course was 8-12 sessions. The effects of a special balanced physiological saline on nasal symptoms of 93 patients with chronic rhinitis were studied. Twelve patients entered a long-term study of budesonide treatment in vasomotor rhinitis. The efficacy of the treatment was studied. Possible systemic side effects were studied via an ACTH stimulation test prior to and during treatment. The local effect in the nasal mucosa was studied by means of a nasal biopsy before and after one year of treatment. The treatment has proved effective, while producing no systemic or local side effects during a one-year treatment period.

Twenty patients with perennial rhinitis took part in a double-blind, randomized, cross-over comparison of flunisolide nasal spray (Syntaris; Syntex) and its vehicle. The sprays were used twice daily for 4 weeks and a total daily dose of 200 micrograms flunisolide was used. Flunisolide was superior to the placebo in relieving sneezing, runny nose and nose-blowing (P less than 0,05). Patient and doctor treatment preferences were significantly in favour of the active spray. Side-effects were mainly confined to nasal irritation and occurred equally in both treatment groups.
