Sleep-disordered Breathing in Neuromuscular Disease.

Sleep-disordered breathing in neuromuscular diseases is due to an exaggerated reduction in lung volumes during supine sleep, a compromised physiologic adaptation to sleep, and specific features of the diseases that may promote upper airway collapse or heart failure. The normal decrease in the rib cage contribution to the tidal volume during phasic REM sleep becomes a critical vulnerability, resulting in saw-tooth oxygen desaturation possibly representing the earliest manifestation of respiratory muscle weakness. Hypoventilation can occur in REM sleep and progress into non-REM sleep, with conti…

Mini-open lateral retro-pleural thoracic corpectomy for osteomyelitis.

Thoracic vertebral body corpectomy with associated deformity and neural element compression can be challenging. Multiple approaches have been proposed including trans-pedicular, costotransversectomy and trans-thoracic. Approach related pitfalls, nuances, morbidity and complication profile differ with each technique. A mini-open retro-pleural approach provides great access to bony pathology and neural elements without the need to violate the thoracic cavity or chest-tube placement postoperatively. Here we present a 56-year-old male with progressive back pain and suspected osteomyelitis/discitis

Anterior and posterior vertebral column resection for severe and rigid idiopathic scoliosis.

A total of 16 patients with severe and rigid idiopathic scoliosis treated by anterior and posterior vertebral column resection (APVCR) were retrospectively reviewed after a minimum follow-up of 2 years. The indication for APVCR was scoliosis more than 90° with flexibility less than 20%. The radiographic parameters were evaluated, and clinical records were reviewed. All patients underwent APVCR with posterior pedicle screw instrumentation in a two-stage surgery. The rib hump was reduced from 7.2 cm preoperatively to 1.8 cm at final follow-up (75% correction). Preoperative curves ranged from 93°

The effects of acid perfusion of the esophagus on ventilation and respiratory sensation.

The relationship between gastroesophageal reflux (GER) and asthma remains controversial. Asthma symptoms worsen with GER, but are not consistently related to changes in lung function. The purpose of this study was to determine whether acid perfusion (AP) of the esophagus alters ventilation and causes respiratory symptoms. Nonasthmatic patients with normal lung function and esophageal disease (16 females and nine males, FEV1 %predicted = 99+/-9.6), underwent a Bernstein test after motility testing. Airflow, rib cage (Vrc), and abdominal (Vab) tidal volumes, esophageal (Pes) and gastric (Pga) pr…

Comparison of three noninvasive methods for measuring scoliosis.

The premise behind most noninvasive techniques for the measurement of scoliotic conditions of the spine is that the lateral distortion of the spine relates directly to transverse rib cage deformity within the transverse plane. The focus of this study was to examine this assumption by comparing different noninvasive methods for the assessment of scoliotic curves. The three techniques examined were (1) use of the Scoliometer (SCOL), (2) use of the back-contour device (BCD), and (3) use of moiré topographic imaging (MTI). Fourteen subjects (10 female, 4 male) with idiopathic adolescent scoliosis …

Assessment of heart and lung morphology in a single case during cardiopulmonary resuscitation: A virtual simulation.

BACKGROUND: Basic science research in cardiopulmonary resuscitation (CPR) is limited by challenges in obtaining haemodynamic data from models that simulate physiological processes. In this study, we assessed the morphology of the heart and lungs and calculated the ejection fractions of cardiac chambers during CPR using a virtual simulation. METHODS: A finite element model of a complete thorax, including internal organs, thoracic rib cage, spine, musculature, and a generic material representing soft tissues, was constructed from magentic resonance images of a man. Twelve chest compression simu…

Biallelic loss-of-function variants in the centriolar protein CCP110 leads to a ciliopathy-like phenotype.

CCP110 (centriolar coiled coil protein 110, also known as CP110) is one of the essential proteins localized in the centrosome that plays critical roles in the regulation of the cell cycle and also in the initiation of ciliogenesis. So far, no human congenital disorders have been identified to be associated with pathogenic variants of CCP110. Mice with biallelic loss-of-function variants of Ccp110 (Ccp110-/-) are known to manifest multiple organ defects, including a small body size, polydactyly, omphalocele, congenital heart defects, cleft palate, short ribs, and a small thoracic cage, a patter

Unlikely Double Trouble: Two Cases of Haemobilia From One Motor Vehicle Accident.

Blunt abdominal trauma frequently results in visceral injury to either solid or hollow organs; however, injury to the gallbladder is rare. This is most likely due to the anatomical position of the gallbladder, which is well-insulated posterior to the liver and rib cage. Gallbladder injuries can be in the form of avulsion, contusion, or laceration. In the same clinical spectrum as contusion is haemobilia, which is defined as bleeding into the biliary system and may result from injury to any of the structures associated with the biliary tree (liver, gallbladder, bile ducts or pancreas). Risk fac…

Prenatal Diagnosis of Skeletal Dysplasia and Review of the Literature.

INTRODUCTION: Obstetric ultrasonography is routinely used to screen for fetal anomalies. Thanatophoric dysplasia (TD) is one of the common though rare lethal skeletal dysplasia, detected during routine ultrasound scan. TD is caused by a mutation in FGFR3 gene. Characteristic features include shortening of limbs, macrocephaly and platyspondyly. In our local setting, it is common to miss the diagnosis in the early scans due to lack of expertise of the sonographers. To the best of our knowledge, this is the first publication from Ghana. Case Presentation. We present the case of a 33-year-old woma

[Subxiphoid incisional hernia treatment: a technique using a double mesh adjusted to the defect].

INTRODUCTION: Subxiphoid incisional hernia has characteristics that differentiate it from the rest and make it a distinctive entity. The fact that it has its sac very near the rib cage and sternum determines the pressure in the margins. The repair, by open or by laparoscopic approach, has not demonstrated good results despite the generalised use of a prosthesis. They are uncommon, and have a significant comorbidity in patients (severe heart diseases, transplants, immunosuppressed), after surgery of the hepato-bilio-pancreatic area with transverse incisions, or very high mid-laparotomies for ga…

The patient-specific brace design and biomechanical analysis of adolescent idiopathic scoliosis.

Brace application has been reported to be an effective approach in treating mild to moderate idiopathic adolescent scoliosis. However, little attention is focused on the biomechanical study of patient-specific brace treatment. The purpose of this study was to propose a design method of personalized brace and to analyze its biomechanical behavior and to compare the brace forces with the I-Scan measurement system. Based on a three-dimensional patient-specific finite element model of the spine, rib cage, pelvis, and abdomen, a parametric patient-specific model of a thoracolumbosacral orthosis was…

Mechanics of the canine diaphragm in ascites: a CT study.

Ascites causes an increase in the elastance of the abdomen and impairs the lung-expanding action of the diaphragm, but its overall effects o … was well explained by the increase in the diameter of the ring of insertion of the diaphragm to the rib cage and the concomitant increase in the radius of diaphragm curvature. These observations indicate that the pressure-generating ability of the diaphragm is determined not only by muscle length as conventionally thought but also by muscle shape.
