Bridging markers defining the map position of X linked hypophosphataemic rickets.

Hypophosphataemic rickets is commonly an X linked dominant hereditary disorder associated with a renal tubular defect in phosphate transport and bone deformities. The gene causing this disorder has been mapped to Xp22.31----p21.3 by using cloned human X chromosome sequences identifying restriction fragment length polymorphisms (RFLPs) in linkage studies of affected families. The hypophosphataemic rickets gene locus (HPDR) was previously mapped distal to the X linked polymorphic locus DXS41 (99.6) but its position in relation to the distal loci DXS43 (D2) and DXS85 (782) was not established. In…

[Type III osteogenesis imperfecta associated with hypophosphatemic vitamin D-resistant rickets].

In a 7 year old girl presenting with bone deformities, dwarfism, and a history of recurrent fractures osteogenesis imperfecta had been diagnosed at birth. Although she had been hospitalized several times, radiologic signs of rickets remained unnoticed. Laboratory data proved existence of hypophosphatemic vitamin D-resistant type of rickets, which was effectively treated with 1 alpha-hydroxycholecalciferol and phosphorus substitution. The combination of osteogenesis imperfecta type III and hypophosphatemic rickets may be coincident. It proves, however, the necessity to consider the possible sim…

Spironolactone-reversible rickets associated with 11 beta-hydroxysteroid dehydrogenase deficiency syndrome.

A 7-year-old girl had growth retardation, hypertension, and hypokalemic alkalosis. Baseline serum aldosterone concentration and plasma renin … one mineral metabolism and parathyroid function, and skeletal radiographs, revealed the presence of rickets and secondary hyperparathyroidism. Treatment with spironolactone alone for 2 months corrected hypertension, hypokalemic alkalosis, and all laboratory and radiologic evidence of rickets and hyperparathyroidism, resulting in acceleration of growth rat …

Vitamin D deficiency rickets and vitamin B12 deficiency in vegetarian children.

During the years 1978-83 four vegetarian children have been admitted to the pediatric departments of Ullevaal and Aker Hospitals in Oslo and Haukeland Hospital, Bergen, with the diagnosis of vitamin D deficiency rickets. One had vitamin B12 deficiency as well. All had been fed a vegetarian diet with some cows' milk, but without vitamin supplementation. All had marked hypocalcemia, and three had tetany or convulsions. All responded well to conventional doses of vitamin D therapy. Two of the mothers had vitamin D deficiency, and one of them also had vitamin B12 deficiency. This report describes …

Low-calcium/high phosphorus rickets in rats. I. Mast cell changes.

Weanling rats were given a low Ca (0.003%)/high P (0.64%) diet with and without vitamin D for periods up to 5 weeks. This was associated with hypocalcaemia, rachitic bone changes and increased bone resorption. These changes preceded the accumulation of large numbers of mast cells in long bone metaphyses. Mast cells did not increase in the epiphyses of long bones or in caudal vertebrae, tooth pulp, skin and other organs. A light and electron microscopic study showed that most mast cells had raised secretory activity, as evidenced by variability in the structure of granules and loss of granule c

Autosomal dominant hypophosphatemic rickets caused by a novel pathogenic variant in FGF23.

We report a child with autosomal dominant hypophosphatemic rickets (ADHR) due to a novel pathogenic variant in fibroblast growth factor 23 (FGF23). A 1 year 10 month old girl was referred for rickets that did not respond to a single mega dose of vitamin D (600 000 IU orally). Laboratory work-up revealed: calcium, 9.8 mg/dL (2.5 mmol/L) [reference range (ref.) 8-11 mg/dL (2-2.7 mmol/L)]; phosphorus, 2.1 mg/dL (0.7 mmol/L) [ref. 2.5-4.5 mg/dL (0.8-1.5 mmol/L)]; alkaline phosphatase, 1453 IU/L (24.2 µkat/L) [ref. 142-335 U/L (2.4-5.6 µkat/L)]; 25-hydroxyvitamin D, 20.6 ng/mL (51.4 nmol/L) [ref. 3…

Secondary Deformity After Hemiepiphysiodesis in Hypophosphatemic Rickets: The Role of Metabolic Control.

BACKGROUND: Overcorrection during hemiepiphysiodesis has been a common practice to prevent deformity recurrence resulting from the rebound phenomenon. This study aims to investigate the deformity progression following hemiepiphysiodesis for children with rickets. METHODS: Records of children with X-linked hypophosphatemic rickets, who underwent hemiepiphysiodesis, were reviewed. Data included demographics, age, body mass index (BMI) percentile, and metabolic status as the alkaline phosphatase percentage above upper normal limits (%ALP-UNL). Radiographs were measured. Children who did not achi…

Alopecia with Vitamin D-Dependent Rickets Type 2 A: A Case Report.

Vitamin D-dependent rickets type II (VDDR2) is a rare inherited autosomal recessive disorder wherein the genetic mutation results in a defect in the vitamin D receptor (VDR), thus leading to target resistance to 1.25-dihydroxy vitamin D. This results in hypocalcemia, hypophosphatemia, secondary hyperparathyroidism, and rickets. We report a case of a four-year-old female child with diffuse alopecia starting 2 weeks after birth, along with frontal bossing, hypoplastic teeth, and an unusual presentation of multiple skin-colored papules over the back. Genetic testing confirmed vitamin D-dependent …

Diagnostic approach to rickets: an Endocrine Society of Bengal (ESB) consensus statement.

Rickets, one of the leading causes of bony deformities and short stature, can be calciopenic (inciting event is defective intestinal calcium absorption) or phosphopenic (inciting event is phosphaturia). Early diagnosis and timely treatment of rickets are crucial for correction of the limb deformities. Guidelines exist for nutritional rickets, but the diagnosis and management of the relatively uncommon forms of rickets are complex. This consensus aims to formulate a simplified diagnostic approach for rickets, especially in resource-limited settings. The consensus statement has been formulated b…

Experience with the targeted next-generation sequencing in the diagnosis of hereditary hypophosphatemic rickets.

OBJECTIVES: Hereditary Hypophosphatemic Rickets (HHR) is a heterogeneous group of disorders characterized by hypophosphatemia. Although the X-linked dominant HHR is the most common form, the genetic etiology of HHR is variable. Recently, developed next-generation sequencing techniques may provide opportunities for making HHR diagnosis in a timely and efficient way. METHODS: We investigated clinical and genetic features for 18 consecutive probands and their 17 affected family members with HHR. All patient's clinical and biochemical data were collected. We first analyzed a single gene with Next…
