Ectopic pregnancy is a common problem usually diagnosed at an early stage and often in an emergency situation. Both medical and surgical treatments can be used for its management. In case of surgical treatment, laparoscopy rather than open surgery must be practiced. Concerning the choice between salpingostomy and salpingectomy, it depends of the controlateral tubal patency. In case of altered controlateral tube, if a sparing surgery is possible it should be preferred. However, this question is still debated if the controlateral tube seems patent.

Treatment of tubal disease can be challenging and complex. It requires accurate assessment of the extent of disease and evaluation of all fertility parameters. Although traditionally treatment involved salpingectomy or IVF or a combination of the two, less invasive treatment options may prove beneficial. We evaluated the various diagnostic tools for disease assessment and the treatment options available.

There is growing acceptance of conservative surgery for ectopic (tubal) pregnancy. An uncommon, yet potentially fatal, complication is delayed hemorrhage due to persistence of gestational tissue. An illustrative case is presented. Routine postoperative determination of serum human chorionic gonadotropin levels and the use of frozen sections when a partial salpingectomy is carried out are recommended to identify and prevent this condition.

OBJECTIVE: The aim of this study is to evaluate the use of hysteroscopic Essure device placement for the treatment of hydrosalpinx (HS)-related infertility in patients with laparoscopic contraindications and compared their pregnancy outcomes following IVF-ICSI treatment with those patients having had laparoscopic tubal occlusion (LTO).

Isolated fallopian tube torsion is a rare cause of acute abdominal pain in adolescent girls, often mimicking ovarian torsion. We report the case of a 15-year-old girl with acute abdominal pain who was diagnosed intraoperatively with isolated left fallopian tube torsion and a large paratubal cyst. Laparoscopic salpingectomy was performed due to tubal nonviability.

DESIGN: Case report.
SETTING: Outpatient clinic.
PATIENT(S): A 27-year-old woman.
INTERVENTION(S): Salpingectomy.
MAIN OUTCOME MEASURE(S): Molar ectopic pregnancy.
RESULT(S): Ectopic partial molar pregnancy.
CONCLUSION(S): Molar pregnancy can occur in ectopic pregnancy. Molar pregnancy clinically mimics normal tubal ectopic pregnancy.

Post-hysterectomy fallopian tube prolapse is a rare complication with only 80 cases described since 1902. Symptoms are non-specific and often of delayed onset. Final diagnosis is confirmed by vaginal biopsy with salpingectomy being the treatment of choice, preferably performed laparoscopically. Following surgery, complete symptom resolution is usually observed and no recurrence has been reported.

A 29-year-old woman with tubal adenocarcinoma stage IA was treated only with bilateral salpingectomy, pelvic lymphadenectomy, and omentectomy. Two years later the patient successfully underwent in vitro fertilization and embryo transfer, and at 39 weeks gave birth to a healthy son by cesarean section. The result of oncologic follow-up 3 years after surgery is negative.

The authors report a case of ovarian hyperstimulation leading to 6 endo-uterine pregnancies and one extra-uterine pregnancy. After embryonic reduction, and salpingectomy, a twin pregnancy was carried through, in a bifid uterus. Review of the literature enables to emphasize the difficulties in diagnosing heterotopic pregnancies, and to discuss the indications and techniques of selective embryonic reduction, along with the ethical and legal problems it raises.

A 24-year-old primigravida presented with subacute ectopic pregnancy and had salpingectomy for a ruptured tubal pregnancy involving the ampullary portion of the left fallopian tube. Post-operative follow-up revealed continuing symptoms of pregnancy and increasing uterine size. A diagnosis of multiple pregnancy was confirmed by abdominal ultrasonography. The patient was managed to term and had normal delivery of twins.

An abdominal pregnancy in a 28-year-old nulliparous woman with a fertility problem was treated conservatively. The clinical antenatal course was unremarkable. A healthy 2000-g infant was delivered at term by laparotomy. The placenta was removed at the same time. Three years later the patient had a ruptured tubal pregnancy which was treated by salpingectomy. The literature on extrauterine pregnancy is reviewed briefly.

We report the successful management of a 31-year-old female, treated by cornual wedge resection. The patient suffered from vaginal spotting and lower abdominal pain. Transvaginal ultrasonography revealed a 4-5 cm right cornual pregnancy and beta-human chorionic gonadotropin was measured to be 614.7 IU/L. This ectopic pregnancy was removed via a laparotomy with cornual wedge resection and right salpingectomy.
