Homozygous GBA1 p.T82I variant in type 1 Gaucher disease: clinical and biochemical characterization.

BACKGROUND: Gaucher disease (GD) is the most common lysosomal storage disorder caused by biallelic pathogenic variants in GBA1, resulting in deficient β-glucocerebrosidase activity. Clinical characterization of rare GBA1 variants is crucial for understanding disease heterogeneity. We report an extremely rare GBA1 variant previously catalogued at the sequence level in HGMD and UniProtKB, and recently listed in ClinVar without a clinical significance assertion, but lacking phenotypic data. CASE PRESENTATION: A 23-year-old male presented with fatigue, chills, shivering, and diffuse bone pain. Ph…

Unusual Drug Fever Caused by Imipenem/Cilastatin and a Review of Literature.

INTRODUCTION: Drug fever is a febrile reaction caused by initiation of one drug or varieties of drugs and often disappears after cessation of the drug(s). Clinically, drug fever is frequently induced by antibiotics, anticonvulsants, and antineoplastics. There are few previous reports about drug fever caused by imipenem/cilastatin. CASE PRESENTATION: Here, we described a 66-year-old man undergoing the Ivor Lewis esophagectomy for esophageal carcinoma, who developed drug fever. The patient had a high temperature with shivering after administration of imipenem/cilastatin for 7 days. Furthermore,…

Fenitrothion, an organophosphorous insecticide, impairs locomotory function and alters body temperatures in Sminthopsis macroura (Gould 1845) without reducing metabolic rates during running endurance and thermogenic performance tests.

Endemic Australian mammal species are exposed to pesticides used for locust control as they occupy the same habitat as the target insect. Th … , time taken to reach peak metabolic rate on cold exposure, cumulative total oxygen consumed during shivering thermogenesis, and body temperature before and after cold exposure were unaffected by fenitrothion. Dunnart rectal temperatures showed a reduction of up to 5 °C after exposure to fenitrothion but returned to pre-exposure levels by 10 d postdose. Suc …

Preliminary study on modulation of the biological effects of tumor necrosis factor-alpha in advanced cancer patients by the pineal hormone melatonin.

Previous experimental studies have suggested the possibility to modulate the biological activity and toxicity of cytokines by immunomodulating neurohormones. In particular, the pineal hormone melatonin (MLT) has been proven to amplify the immune effects of IL-2 and to reduce its toxicity. On this basis, we decided to investigate the effect of MLT on biological activity and toxicity of another important antitumor cytokine, TNF. The study was performed in 14 metastatic solid tumor patients, for whom no effective standard antitumor therapy was available. Informed consent was previously obtained f

Thermoregulatory responses to thermal stimulation of the preoptic anterior hypothalamus in the red fox (Vulpes vulpes).

The preoptic anterior hypothalamus (POAH) thermoregulatory controller can be characterized by two types of control, an adjustable setpoint temperature and changing POAH thermal sensitivity. Setpoint temperatures for shivering (Tshiver) and panting (Tpant) both increased with decreasing ambient temperature (Ta), and decreased with increasing Ta. The POAH controller is twice as sensitive to heating as to cooling. Metabolic rate (MR) increased during both heating and cooling of the POAH. Resting temperature of the POAH was lower than internal body temperature (Tb) at all temperatures. This indica…

Arterial and venous plasma levels of bupivacaine following peripheral nerve blocks.

Mean arterial plasma (MAP) and peripheral mean venous plasma (MVP) levels of bupivacaine were ascertained in 3 groups of 10 patients each for: (1) intercostal nerve block, 400 mg; (2) block of the sciatic, femoral, and lateral femoral cutaneous nerves, with or without block of the obturator nerve, 400 mg; and (3) supraclavicular brachial plexus block, 300 mg. MAP levels were consistently higher than simultaneously sampled MVP levels, the highest levels occurring from bilateral intercostal nerve block. No evidence of systemic toxicity was observed. The results suggest that bupivacaine has a muc

Regulation of Neural Functions by Brain Temperature and Thermo-TRP Channels.

Body temperature is an important determinant in regulating the activities of animals. In humans, a mild 0.5 °C hyperthermia can cause headaches, demonstrating that the maintenance of normal body temperature is a key for our health. In a more extreme example, accidental acute hypothermia can lead to severe shivering, loss of consciousness, or death, although the details of these mechanisms are poorly understood. We previously found that the TRPV4 ion channel is constitutively activated by normal body temperature. The activation threshold of TRPV4 is >34 °C in the brain, which enables TRPV4 to c…

Rat behavioral thermoregulation integrates with nonshivering thermogenesis during postnatal development.

Rat pups are capable of behavioral thermoregulation, both in the nest and on a thermocline, as early as the 1st week of postnatal life, and these pups can also produce heat metabolically without shivering. The rat pup's primary source of nonshivering thermogenesis is the sympathetically mediated metabolism of brown adipose tissue (BAT). BAT is well formed in newborns and functions shortly after birth. While infant behavioral thermoregulation and BAT thermogenesis have been extensively studied, little is known about the extent to which thermoregulatory behavior can be influenced by BAT thermoge…

Neuroleptic malignant syndrome following cardiac surgery: successful treatment with dantrolene.

Neuroleptic malignant syndrome (NMS) is a rare idiosyncratic reaction to neuroleptic drugs, which is potentially fatal. It has been occasionally reported that NMS occurs subsequently after surgery. We report a case of a 53-year-old male patient who developed NMS following cardiac surgery due to the resumption of zotepine. The patient was attacked with hyperthermia, sweating, significant shivering, trembling of the fingers, disturbed consciousness and extreme muscle rigidity after the resumption of zotepine. Furthermore, laboratory measurements revealed increased levels of serum blood urea nitr…

Patients with coronary artery disease are particularly at risk perioperatively, as myocardial infarction, unstable angina, severe arrhythmia … ve period because increased global oxygen consumption associated with recovery, ventilator weaning, shivering and pain may lead to tachycardia and increased ventricular load. These factors must be taken into account in order to prevent MI and improve postoperative cardiac outcome.

[A case of steroid-responsive late deterioration in Cryptococcus meningoencephalitis followed by longitudinal extensive transverse myelitis].

A 58-year-old male was transported by ambulance to our hospital due to shivering, headache, and abnormal behavior, with a high fever, impaired consciousness, and neck stiffness. The cerebrospinal fluid (CSF) tests revealed increase in mononuclear cell count. Cryptococcal antigen tests were positive in both the serum and CSF samples, especially with remarkably high titer in the latter. According to these findings, he was diagnosed with cryptococcal meningoencephalitis (CM). Antifungal therapy was partially effective for the initial neurological symptoms and in the abnormal CSF findings, but on …

Thermoregulation Effects of Phoneutria nigriventer Isolated Toxins in Rats.

Body temperature is primarily regulated by the hypothalamus, ensuring proper metabolic function. Envenomation by Phoneutria nigriventer can cause symptoms such as hypothermia, hyperthermia, sweating, and shivering, all related to thermoregulation. This study aims to analyze and identify components of the venom that affect thermoregulation and to evaluate possible mechanisms. Rats were used for thermoregulation analysis, venom fractionation by gel filtration and reverse-phase chromatography (C18), and sequencing by Edman degradation. The venom exhibited hypothermic effects in rats, while its fr…

A case of septic arthritis caused by Capnocytophaga canimorsus in an HIV patient.

Invasive infections caused by Capnocytophaga canimorsus , a Gram-negative rod found in the oral cavity of healthy dogs and cats, are rare but they are increasing worldwide. We report a case of septic arthritis in a native knee joint due to this micro-organism. A 57-year-old man, with a well-controlled chronic HIV infection, attended the Emergency Department because of left knee pain and shivering without measured fever. A knee arthrocentesis and a computed tomography scan were performed, revealing septic arthritis with collections in the left leg posterior musculature. He was admitted to the I…
