Acute massive rotator cuff rupture with posterior shoulder dislocation: arthroscopic novel repair of a rare injury. A case report.

Acute massive rotator cuff tears and posterior shoulder dislocations are an extremely rare association, and a vast majority are treated by open surgery. We present a case of a man in his 20s who suffered closed left shoulder trauma after a road traffic accident. He was initially diagnosed with posterior shoulder dislocation and a reduction was successfully performed. However, the patient still complained of persistent weakness during active movements. We found a massive rotator cuff tear involving all the rotator cuff tendons, with significant supraspinatus retraction and persistent posterior

Comparison of spinal accessory nerve transfer to supra-scapular nerve vs. shoulder arthrodesis in adults with brachial plexus injury.

BACKGROUND: Restoring shoulder mobility, stability, and strength is a key goal in patients with brachial plexus injuries. Shoulder arthrodesis is chiefly used as an adjunct to, or after failure of, initial direct nerve surgery. The objective of this study was to compare clinical and functional shoulder outcomes after direct nerve transfer vs. shoulder arthrodesis in adults with supra-clavicular brachial plexus injuries. HYPOTHESIS: Shoulder arthrodesis, currently used as a salvage procedure in brachial palsy injuries, deserves to be viewed to a valid alternative to direct nerve transfer. MAT

Biomechanical evaluation of shear force vectors leading to injury of the biceps reflection pulley: a biplane fluoroscopy study on cadaveric shoulders.

BACKGROUND: The clinical importance of the biceps reflection pulley (BRP), which stabilizes the long head of the biceps tendon (LHB) as it exits the joint, has been shown. However, there is controversy on the pathomechanism of injury to the BRP. The angular orientation of the LHB relative to its origin and distal course changes with joint positions and may place the BRP at risk for injury. PURPOSE: To measure the course of the LHB in common arm positions and to determine the shear and normal (stabilizing) force vectors as well as the excursion of the LHB. STUDY DESIGN: Descriptive laboratory

Dynamic ultrasound evaluation and treatment of subcoracoid bursitis after HPV vaccination: a case of shoulder injury related to vaccine administration (SIRVA).

Shoulder Injury Related to Vaccine Administration (SIRVA) is a complication caused by improperly administered vaccinations. It triggers an inflammatory cascade that damages shoulder structures, resulting in prolonged shoulder pain and restricted range of motion. Common diagnoses in SIRVA patients include shoulder bursitis (all classified as subacromial bursitis), adhesive capsulitis, and rotator cuff injuries such as tears or tendinopathy. Less frequent diagnoses include biceps tenosynovitis, shoulder synovitis, intramuscular hematoma, and polymyalgia rheumatica or polymyalgia rheumatica-like

A "Common Sense" Surgical Approach to Motor Shoulder Dysfunction Diagnosed as CRPS I/II Following Projectile Injuries to the Regio Deltoidea in the Late Stages of Ballistic Trauma.

BACKGROUND: High-velocity projectile injuries to the regio deltoidea (RD) can result in persistent pain and motor shoulder dysfunction (MSD). The underlying mechanisms may involve direct axillary nerve (Ax) trauma or remote nociceptor irritation caused by cavitation effects. OBJECTIVE: To characterize the anatomical basis of MSD and to establish a reliable diagnostic and surgical strategy for its management. METHODS: Twenty-seven patients presenting with painful MSD following projectile trauma to the RD were included. Eight patients exhibited deltoid palsy associated with Ax injury, of whom

Risk of radial nerve injury during deltoid intramuscular injection in shoulder internal rotation and abduction position-cadaveric anatomical study.

Radial nerve injury following intramuscular injection into the upper arm is rare but clinically significant. Although injection-site selection is important, the effect of shoulder position, particularly internal rotation, on radial nerve location remains unclear. Because the radial nerve courses within the triceps brachii and pierces the lateral intermuscular septum close to the humerus, we hypothesized that shoulder abduction combined with internal rotation would decrease the distance from the acromion to the radial nerve relative to the neutral arm-at-side position. In this cadaveric anatomi

The prevalence and clinical course of shoulder injury related to vaccine administration (SIRVA) after COVID-19 vaccines in Dutch hospital workers.

INTRODUCTION: Shoulder Injury Related to Vaccine Administration (SIRVA) is a rare disorder characterized by persistent shoulder pain and limited range of motion presenting within 48 h after vaccine administration. With the widespread distribution of the COVID-19 vaccine, the incidence of SIRVA is expected to rise. This sudden rise in vaccine administration presents an ideal opportunity to estimate the prevalence of SIRVA and to better characterize SIRVA. OBJECTIVE: This study aims to investigate the prevalence of SIRVA following COVID-19 vaccine administration among hospital workers in the Ne

Outcomes of Shoulder Functions in Spinal Accessory to Suprascapular Nerve Transfer in Brachial Plexus Injury: A Comparison between Anterior and Posterior Approach.

Background  Restoration of shoulder functions is important in brachial plexus injury (BPI). The functional outcomes of spinal accessory nerve (SAN) to suprascapular nerve (SSN) transfer by the anterior supraclavicular approach and the posterior approach is a matter of debate. This article aims to compare the outcomes of the shoulder functions by the SAN to the SSN transfer using the two approaches. Methods  Retrospective data was collected in 34 patients who underwent SAN to SSN transfer from January 2016 to June 2018. Group A included 16 patients who underwent nerve transfers by anterior appr

Efficacy of spinal accessory nerve to suprascapular nerve transfer to restore shoulder function in brachial plexus injury: A systematic review and meta-analysis.

INTRODUCTION: Transfer of the spinal accessory nerve (SAN) to the suprascapular nerve (SSN) is a common surgical intervention employed for restoring shoulder function in both obstetric and traumatic brachial plexus injury (TBPI). Despite widespread use, there is a paucity of evidence surrounding the efficacy of this procedure. METHODS: A systematic search of the literature in the National Institutes of Health MEDLINE and Embase databases was performed in accordance with the PRISMA guidelines. Patients had to have a minimum postoperative follow-up of 6 months. Cohorts containing patients with
