Sleep and Substance Use Disorders: A Primer for the Clinician.

The neurophysiology of sleep and the pathophysiology of substance use disorders (SUDs) involve overlaps in neurotransmitters and pathways. SUDs and substance withdrawal are accompanied by significant impairments in sleep. Conversely, sleep disturbances are known to predispose for SUDs. Sleep disorders (SDs) such as insomnia, short sleep duration, and excessive daytime sleepiness have been causally related to SUD initiation and maintenance. Management of SUDs integrally depends on the successful management of the associated SDs, and vice versa.

Do sleep variables predict mood in bipolar disorder: A systematic review.

INTRODUCTION: Most people with bipolar disorder (BD) experience sleep disturbances across mood states and many identify sleep changes before both depressive and manic episodes. Nearly half of all patients have multiple relapses of BD and identifying early warning signs of relapse, such as sleep changes, could benefit both patients and clinicians as a preventive strategy. METHODS: A systematic search of the databases Embase, APA PsychINFO, and MEDLINE was performed to identify studies that investigated the relationship between sleep changes and mood in BD. The review was registered with PROSPE

Sleep and borderline personality disorder: a review.

Sleep problems are very common among psychiatric patients. Borderline personality disorder, as a common and severe mental disorder, is associated with different types of sleep disturbances, such as disturbances of sleep continuity, altered REM sleep regulation and nightmares. These disturbances are the result of interaction of the personality traits, concomitant and comorbid diseases and environmental factors. Despite the high prevalence of sleep related disorders in BPD patients, this aspect of BPD is still neglected in clinical and research settings. To date there has been little agreement o

Sleep architecture and the apnoea-hypopnoea index in children with obstructive-sleep apnoea syndrome.

This study aimed to examine the sleep architecture and craniofacial morphology in a group of children divided by different levels of apnoea-hypopnoea index (AHI), 5, 4.5, 4, 3.5, 3 and 2.5, and to determine an AHI threshold value at which sleep architecture is most affected. 23 children, who were selected from a preliminary questionnaire survey about sleep-related breathing disorders, were evaluated with cephalometric radiographs and overnight polysomnography. The findings indicated that the children with AH1 > or = 2.5 and > or = 3 showed significantly larger numbers of waking (p < 0.005) and

Scheduled feeding improves sleep in a mouse model of Huntington's disease.

Sleep disturbances are common features of neurodegenerative disorders including Huntington's disease (HD). The sleep and circadian disruptions are recapitulated in animal models, and these models provide the opportunity to evaluate whether circadian interventions can be effective countermeasures for neurodegenerative disease. Time restricted feeding (TRF) interventions successfully improve activity rhythms, sleep behavior and motor performance in mouse models of HD. Seeking to determine if these benefits of scheduled feeding extend to physiological measures of sleep, electroencephalography (EE

[Sleep and wakefulness drugs and their indications].

This paper is a review of the use of drugs in sleep and wakefulness disorders. Insomnia is more often a symptom than an autonomic disorder. Good knowledge of the clinical facts is required before prescribing hypnotics. Sedative drugs are potentially hypnotics; yet, melatonin is not sedative and may be considered to resynchronise of sleep phases. Stimulant drugs are prescribed in attention disorders; methylphenidate is the more frequently used. Narcolepsy, which is characterized by daytime sleepiness and irresistible episodes of sleep, is treated by an alpha1 noradrenergic stimulant modafinil w

The silent threat in mucopolysaccharidosis: assessment of sleep quality and disorders.

OBJECTIVES: Mucopolysaccharidoses (MPS) are disease characterized by the accumulation of glycosaminoglycans, which leads to involvement of multiple systems. Respiratory involvement can lead to obstructions, causing difficulties with daily vital functions and sleep problems. This study aims to assess the sleep quality and disorders of patients with MPS using questionnaires. METHODS: The study comprised 24 patients diagnosed with MPS and 44 healthy children aged between 6 and 18 years. Patients' age, gender, height, weight, MPS subtype, age at diagnosis, enzyme replacement therapy status, and r

Expression of a poly-glutamine-ataxin-3 transgene in orexin neurons induces narcolepsy-cataplexy in the rat.

The sleep disorder narcolepsy has been linked to loss of hypothalamic neurons producing the orexin (hypocretin) neuropeptides. Here, we report the generation of transgenic rats expressing a human ataxin-3 fragment with an elongated polyglutamyl stretch under control of the human prepro-orexin promoter (orexin/ataxin-3 rats). At 17 weeks of age, the transgenic rats exhibited postnatal loss of orexin-positive neurons in the lateral hypothalamus, and orexin-containing projections were essentially undetectable. The loss of orexin production resulted in the expression of a phenotype with fragmented

From Dreams to Hallucinations: Jean Lhermitte's Contribution to the Study of Peduncular Hallucinosis and the Dissociation of States.

Jean Lhermitte (1877-1959) was one of the pioneers of behavioral neurology, including the field of hallucinations. This article focuses on his work concerning the relationship between hallucinations, sleep, and dreams. From 1910, Lhermitte became interested in sleep and its disorders, particularly narcolepsy and its accompanying symptoms. He also reported on sleep disorders and hallucinations occurring in people with lesions of the diencephalic region ("infundibular syndrome"), and later encephalitis lethargica. In 1922, he described a syndrome of complex, predominantly visual hallucinations i

The 5-HT6 receptor antagonist idalopirdine potentiates the effects of donepezil on gamma oscillations in the frontal cortex of anesthetized and awake rats without affecting sleep-wake architecture.

The 5-HT6 receptor is a promising target for cognitive disorders, in particular for Alzheimer's disease (AD). The high affinity and selective 5-HT6 receptor antagonist idalopirdine (Lu AE58054) is currently in development for mild-moderate AD as adjunct therapy to acetylcholinesterase inhibitors (AChEIs). We studied the effects of idalopirdine alone and in combination with the AChEI donepezil on cortical function using two in vivo electrophysiological methods. Neuronal network oscillations in the frontal cortex were measured during electrical stimulation of the brainstem nucleus pontis oralis

Composite Sleep Problems Observed Across Smith-Magenis Syndrome, MBD5-Associated Neurodevelopmental Disorder, Pitt-Hopkins Syndrome, and ASD.

Caregivers of preschool and elementary school age children with Smith-Magenis syndrome (SMS), MBD5-associated neurodevelopmental disorder (MAND), and Pitt-Hopkins syndrome (PTHS) were surveyed to assess sleep disturbance and to identify disorder-specific sleep problems. Because of overlapping features of these rare genetic neurodevelopmental syndromes, data were compared to reports of sleep disturbance in children with autism spectrum disorder (ASD). While similarities were observed with ASD, specific concerns between disorders differed, including mean nighttime sleep duration, daytime sleepin

Alterations in sleep, sleep spindle, and EEG power in mGluR5 knockout mice.

The type 5 metabotropic glutamate receptor (mGluR5) represents a novel therapeutic target for schizophrenia and other disorders. Schizophrenia is associated with progressive abnormalities in cortical oscillatory processes including reduced spindles (8-15 Hz) during sleep and increased delta (0.5-4 Hz)- and gamma-band activity (30-80 Hz) during wakefulness. mGluR5 knockout (KO) mice demonstrate many schizophrenia-like behaviors, including abnormal sleep. To examine the effects of mGluR5 on the maintenance of the neocortical circuitry responsible for such neural oscillations, we analyzed sleep/w

[Impaired daytime functions in children with sleep disorders].

Sleep in adequate amount and quality is essential for normal child development. Sleep disorders may affect the childs daytime functions resulting in behavioral problems such as attention deficit, aggressiveness and hyperactivity. Recent reports have suggested that chronic sleep disturbance can cause neurocognitive deficits and impaired learning abilities. Obstructive sleep apnea syndrome, asthma, atopic dermatitis and juvenile rheumatoid arthritis--are common childhood diseases that can impair normal sleep. This article reviews the sleep characteristics in these diseases and the relation betwe

Non-pharmacological management of problematic sleeping in children with developmental disabilities.

AIM: Sleep is important for underlying neural plasticity, and children with developmental disabilities suffer behavioural, emotional, cognitive, and sensory-motor issues that affect their wake and sleep states. Problematic sleeping can be hypothesized to have adverse effects on both of these areas in children with developmental disabilities. With this review, we aim to provide a benchmark in managing problematic sleeping in children with developmental disabilities. METHOD: A literature search was conducted and data on the study descriptives, patient characteristics, study design, study-relate
