Exotic poisonous animals such as snakes, marine animals, spiders, and scorpions are a rarity in Central Europe, but are kept as pets by some … , and other poisonous animals (4%). Severe poisoning was involved in 8% of the cases, all caused by snake bites. The severe poisonings were in the form of coagulopathies, severe local symptoms, and a respiratory insufficiency requiring intubation. In six cases of severe poisoning, an immune serum (antivenom) was administered and in three cases a surgical proc …

Multi-Territorial Ischemic Stroke Following Russell's Viper Envenomation: A Case Report.

Snake envenomation is a major health concern in tropical countries. Russell's viper, one of the "big four" venomous snakes in India, is known for its hemotoxic venom that commonly causes coagulopathy, spontaneous bleeding, and local tissue damage. However, ischemic strokes following viper envenomation are rare and underreported. We describe a case of a 52-year-old healthy male who sustained Russell's viper bite, and he presented with profuse sweating, altered sensorium, and respiratory distress within hours of the bite and required mechanical ventilation and anti-snake venom (ASV) therapy. Des

Tigecycline-Induced Severe Hypoglycemia in a Non-Diabetic Individual: A Case Report and Brief Review of Tigecycline-Induced Severe Hypoglycemia.

BACKGROUND Tigecycline is a broad-spectrum antibiotic belonging to the glycylcycline class. Hypoglycemia is a rare adverse effect associated with its use. We report a case of multiple episodes of severe hypoglycemia in a non-diabetic patient on treatment with tigecycline for cellulitis following snake bite. CASE REPORT A 45-years-old male was admitted with cellulitis of left leg due to snake bite with sepsis, acute kidney injury, and disseminated intravascular coagulation. A transtracheal aspirate culture showed infection with Klebsiella pneumoniae, and parenteral tigecycline was started based

Establishment and first experiences of the National Serum Depot in the Netherlands.

Since 2008, a National Serum Depot is operational in the Netherlands, guaranteeing antivenom supply, 24 h per day, during medical emergencies. In this article the organisation structure, choice of antivenoms, problems encountered during the establishment, and the results from establishment in 2008 till December 2011 are discussed. The Serum Depot is organised by the National Institute for Public Health and the Environment in cooperation with the Dutch National Poisons Information Center. During establishing and maintaining of the Serum Depot several antivenom purchase difficulties were encount

Blindness and scalp haematoma in a child following a snakebite.

BACKGROUND: Snake envenomation is a major public health problem of the Savannah regions of West Africa. Ocular manifestations of snakebites are rare with few reports documenting blindness as a complication. OBJECTIVE: To highlight an unusual manifestation of snake bites and its attendant problems. METHODS: A report of scalp haematoma and blindness in a 10 year old child presenting 2 weeks after a snake bite (presumably carpet viper) is a rare manifestation. Local swelling, epistaxis, bilateral proptosis, exposure keratopathy and use of traditional eye medications were associated findings. An…

Diagnosis of snakebite and the importance of immunological tests in venom research.

In many cases of envenoming following snake bite, the snake responsible for the accident remains unidentified; this frequently results in difficulty deciding which antivenom to administer to the systemically-envenomed victim, especially when only monospecific antivenoms are available. Normally the specific diagnosis of snake bite can be conveniently made using clinical and laboratory methods. Where clinical diagnosis depends upon the recognition of specific signs of envenoming in the patient, laboratory diagnosis is based on the changes which occur in envenomed victims including the detection

Snakebite Associated Thrombotic Microangiopathy and Recommendations for Clinical Practice.

Snakebite is a significant and under-resourced global public health issue. Snake venoms cause a variety of potentially fatal clinical toxin syndromes, including venom-induced consumption coagulopathy (VICC) which is associated with major haemorrhage. A subset of patients with VICC develop a thrombotic microangiopathy (TMA). This article reviews recent evidence regarding snakebite-associated TMA and its epidemiology, diagnosis, outcomes, and effectiveness of interventions including antivenom and therapeutic plasma-exchange. Snakebite-associated TMA presents with microangiopathic haemolytic anae

Neurotoxicity in Russell's viper (Daboia russelii) envenoming in Sri Lanka: a clinical and neurophysiological study.

CONTEXT: Russell's viper is more medically important than any other Asian snake, due to number of envenoming's and fatalities. Russell's viper populations in South India and Sri Lanka (Daboia russelii) cause unique neuromuscular paralysis not seen in other Russell's vipers. OBJECTIVE: To investigate the time course and severity of neuromuscular dysfunction in definite Russell's viper bites, including antivenom response. METHODOLOGY: We prospectively enrolled all patients (>16 years) presenting with Russell's viper bites over 14 months. Cases were confirmed by snake identification and/or enzy

Epidemiology of snakebite and use of antivenom in Argentina.

BACKGROUND: The incidence and case fatality rate of snakebite in Argentina are poorly known. METHODS: The authors used questionnaires provided with antivenoms by the primary manufacturer of anti-venoms in Argentina. RESULTS: A total of 8083 completed questionnaires was collected between 1978 and 1998. The annual incidence of snakebite was 1.8 bites per 100,000 inhabitants, with a high geographical heterogeneity; in the northern provinces of the country, the incidence can exceed 150 snakebites per 100,000 people per year. Bothrops (pit viper) bites predominated, accounting for 96.6% (6720/695

Bacteriological profile of secondary infections following snakebite envenomation's: A systematic literature review.

INTRODUCTION: Secondary infection is a significant complication of snakebite envenomation, with considerable impact on clinical outcomes. Identifying the bacterial agents involved is essential for appropriate case management. Notably, the published literature on bacteriological profiles of snakebite secondary infections is geographically skewed toward South and Southeast Asia and Brazil, with marked underrepresentation of data from sub-Saharan Africa and Central America - regions that bear a disproportionate share of the global snakebite burden. METHODS: Two systematic literature reviews were

Snakebite envenomation-associated acute kidney injury: a South-Asian perspective.

Snakebite is a neglected public health problem in tropical countries. Snakebite envenomation-associated acute kidney injury (SBE-AKI) is a major complication accounting for significant morbidity and mortality. The pathogenesis of SBE-AKI may be multifactorial, including prerenal AKI secondary to hemodynamic alterations, intrinsic renal injury, immune-related mechanisms, venom-induced consumptive coagulopathy and capillary leak syndrome. Epidemiological factors include snake species, duration and severity of snakebite, traditional healers and native medication and accessibility to modern health

High-Throughput Venomics.

In this study, we present high-throughput (HT) venomics, a novel analytical strategy capable of performing a full proteomic analysis of a snake venom within 3 days. This methodology comprises a combination of RP-HPLC-nanofractionation analytics, mass spectrometry analysis, automated in-solution tryptic digestion, and high-throughput proteomics. In-house written scripts were developed to process all the obtained proteomics data by first compiling all Mascot search results for a single venom into a single Excel sheet. Then, a second script plots each of the identified toxins in so-called Protein

Review of the Mechanisms of Snake Venom Induced Pain: It's All about Location, Location, Location.

Pain-acute, chronic and debilitating-is the most feared neurotoxicity resulting from a survivable venomous snake bite. The purpose of this review is to present in a novel paradigm what we know about the molecular mechanisms responsible for pain after envenomation. Progressing from known pain modulating peptides and enzymes, to tissue level interactions with venom resulting in pain, to organ system level pain syndromes, to geographical level distribution of pain syndromes, the present work demonstrates that understanding the mechanisms responsible for pain is dependent on "location, location, l

Pharmacological re-assessment of traditional medicinal plants-derived inhibitors as antidotes against snakebite envenoming: A critical review.

ETHNOPHARMACOLOGICAL RELEVANCE: Traditional healers have used medicinal plants to treat snakebite envenomation worldwide; however, mostly without scientific validation. There have been many studies on the therapeutic potential of the natural products against snake envenomation. AIM OF THE STUDY: This review has highlighted snake venom inhibitory activity of bioactive compounds and peptides from plants that have found a traditional use in treating snakebite envenomation. We have systematically reviewed the scenario of different phases of natural snake venom inhibitors characterization covering

Exploring the Utility of ssDNA Aptamers Directed against Snake Venom Toxins as New Therapeutics for Snakebite Envenoming.

Snakebite is a neglected tropical disease that causes considerable death and disability in the tropical world. Although snakebite can cause a variety of pathologies in victims, haemotoxic effects are particularly common and are typically characterised by haemorrhage and/or venom-induced consumption coagulopathy. Antivenoms are the mainstay therapy for treating the toxic effects of snakebite, but despite saving thousands of lives annually, these therapies are associated with limited cross-snake species efficacy due to venom variation, which ultimately restricts their therapeutic utility to part
