Necrotizing soft tissue infections after injection therapy: Higher mortality and worse outcome compared to other entry mechanisms.

OBJECTIVES: Necrotizing Soft Tissue Infections represent a rare entity of infection associated with a high mortality. The aim of this retrospective study was to analyze patients with an iatrogenic etiology of injection or infiltration to compare the outcome with other etiologies. METHODS: The study group consisted of 21 patients treated with a Necrotizing Fasciitis caused by injection or infiltration. Risk factors and outcome were compared to 134 patients with a Necrotizing Fasciitis caused by other entry mechanisms. RESULTS: Overall mortality in our study group was 14 of 21 (67%) with an am…

Use of methylnaltrexone to induce laxation in acutely injured patients with burns and necrotizing soft-tissue infections.

The routine use of high-dose opioids for analgesia in patients with acute burns and soft-tissue injuries often leads to the development of opioid-induced constipation. The opioid antagonist methylnaltrexone (MLTX) reverses narcotic-related ileus without affecting systemic pain treatment. The authors' burn center developed a bowel protocol that included administration of MLTX for relief of opioid-induced constipation after other methods failed. The authors performed a retrospective review of patients with acute burns or necrotizing soft-tissue infections, who had been given subcutaneous MLTX to

Microbiology of skin and soft tissue infections in the age of community-acquired methicillin-resistant Staphylococcus aureus.

The objectives of this study were to determine the etiology of skin and soft-tissue infections (SSTIs) in a general population, and to describe patient characteristics, SSTI types, frequency of microbiologic testing, and the role of methicillin-resistant Staphylococcus aureus (MRSA) over time. Using electronic databases, we identified SSTI episodes and microbiologic testing among members of a large US health plan. Between 2006 and 2009, 648699 SSTI episodes were identified, of which 23% had a specimen, of which 15% were blood. A pathogen was identified in 58% of SSTI cultures. S. aureus was th

Current role of community-acquired methicillin-resistant Staphylococcus aureus among children with skin and soft tissue infections.

Community-acquired methicillin-resistant Staphylococcus aureus has become a well-established pathogen with alarming rates during the last decade. The current situation of this bacteria in pediatric infections is very limited and motivated us to conduct this study. This is a retrospective and analytical study including patients less than 18 years of age with the diagnosis of skin or soft tissue infections in 2008 and 2009 meeting the criteria of Community-acquired infection. A prevalence of 41.9% among skin and soft tissue infections was found. Inducible resistance to clindamycin was detected i…

Severity assessment of skin and soft tissue infections: cohort study of management and outcomes for hospitalized patients.

BACKGROUND: Skin and soft tissue infections (SSTIs) are caused by bacterial invasion of the skin and underlying soft tissues and can present with a wide spectrum of signs, symptoms and illness severity. They are a common indication for antimicrobial therapy. However, there are few data on treatment outcomes or the validity of clinical severity scores. METHODS: Two hundred and five adult patients admitted to Ninewells Hospital, Scotland in 2005, and treated with antibiotics for SSTI, were identified. They were stratified into four classes of severity (class IV = most severe) based on sepsis, c…

Daptomycin versus other antimicrobial agents for the treatment of skin and soft tissue infections: a meta-analysis.

BACKGROUND: Skin and soft tissue infections (SSTIs) are common in everyday clinical practice. Daptomycin has been shown to achieve very good concentrations in skin and soft tissues. OBJECTIVE: To compare the effectiveness and toxicity of daptomycin with that of other antimicrobials for the treatment of SSTIs. METHODS: PubMed, Scopus, and the Cochrane Central Register of Controlled Trials were searched for articles published up to March 2009. Comparative studies in which daptomycin was used in the intervention group were included in this meta-analysis. The primary outcome of interest was clin…

Community-onset methicillin-resistant Staphylococcus aureus skin and soft-tissue infections: impact of antimicrobial therapy on outcome.

BACKGROUND: Conflicting data exist on the role of antimicrobial therapy for the treatment of uncomplicated community-onset methicillin-resistant Staphylococcus aureus (MRSA) skin and soft-tissue infections (SSTIs). METHODS: We performed a retrospective cohort study of 492 adult patients with 531 independent episodes of community-onset MRSA SSTIs, which consisted of abscesses, furuncles/carbuncles, and cellulitis, at 2 tertiary care medical centers. The purpose of the study was to determine the impact of active antimicrobial therapy (i.e., the use of an agent to which the organism is susceptib

Necrotizing soft-tissue infections and sepsis caused by Vibrio vulnificus compared with those caused by Aeromonas species.

BACKGROUND: Vibrio and Aeromonas species, which can cause necrotizing fasciitis and primary septicemia, are members of the Vibrionaceae family and thrive in aquatic environments. Because the clinical symptoms and signs of necrotizing fasciitis and sepsis caused by these two bacteria are similar, the purposes of this study were to describe the clinical characteristics of Vibrio vulnificus and Aeromonas infections, to analyze the risk factors for death, and to compare the effects of surgical treatment on the outcome. METHODS: The cases of thirty-two patients with necrotizing soft-tissue infecti

Skin, soft tissue, bone, and joint infections in hospitalized patients: epidemiology and microbiological, clinical, and economic outcomes.

BACKGROUND: Infections involving skin, soft tissue, bone, or joint (SSTBJ) are common and often require hospitalization. There are currently few published studies on the epidemiology and clinical and economic outcomes of these infections, whether acquired in the community or healthcare setting, in a large population. OBJECTIVE: To characterize outcomes of culture-proven SSTBJ infection in hospitalized patients, using information from a large database. DESIGN: We identified patients hospitalized in 134 institutions during 2002-2003 for whom specific International Classification of Diseases, N

Oral antibiotic treatment for methicillin-resistant Staphylococcus aureus skin and soft tissue infections: review of the literature.

BACKGROUND: The emergence of methicillin-resistant Staphylococcus aureus (MRSA) as a cause of hospital- and community-associated infection has been reported worldwide and has become an increasing health care problem. Treatment options for MRSA infection are limited, complicated and expensive. Oral antibiotics have been used in the outpatient setting for less severe MRSA infections such as skin and soft tissue infections (SSTIs), but their efficacy has not been well studied. METHODS: We reviewed the literature and Internet information sources as well as recent abstracts for factors relevant to…

A severity score for complicated skin and soft tissue infections derived from phase III studies of linezolid.

BACKGROUND: Severity scoring systems are useful for assessing patient risk and predicting prognosis. METHODS: We developed a scoring system using data from a phase III study comparing antibiotics in hospitalized patients with complicated skin and soft tissue infections (study A), and used logistic regression analysis to identify factors contributing to treatment failure. We tested this system using data from a similar study (study B). RESULTS: In study A (n = 682), cure rates were lower in patients with at least 1 comorbid condition (P <0.05) and in the highest risk class (P = 0.05); elevate…

Association between postoperative surgical site infection and oncologic outcomes in soft tissue sarcoma: a propensity score-matched analysis using the bone and soft tissue registry.

PURPOSE: Postoperative surgical site infection (SSI) is a prevalent complication. The impact of SSI on oncologic outcomes in soft tissue sarcoma (STS) remains debatable. This study evaluates the correlation between SSI and prognosis following STS resection through analysis of a national registry and propensity score matching (PSM). METHODS: We conducted a retrospective cohort study using data from the Japanese Nationwide Bone and Soft Tissue Tumor (BSTT) Registry between 2006 and 2019. The study included patients aged 18 years or older with localized primary STS who underwent surgical resecti

Above-Knee Amputation in the Setting of a Chronic Soft Tissue Infection and Periprosthetic Joint Infection With Exposed Hardware.

Total knee arthroplasty is one of the most common and successful orthopedic surgeries for the treatment of osteoarthritis. While infection is a common postoperative complication, periprosthetic joint infection is a rarer type of infection requiring aggressive treatment. This case report reviews the clinical course of a 71-year-old female with a history of recurrent prosthetic joint infections and osteomyelitis of the left knee and femur following a left total knee arthroplasty. Ten years after the index procedure, she developed a soft tissue ulcer and non-healing wound that led to exposure of

Metastatic community acquired Klebsiella pneumonia infection, secondary to skin and soft tissue infection: A case report.

Klebsiella pneumonia is known to cause hospital-acquired infections, primarily in immunocompromised patients. Recently, a distinct syndrome of community-acquired invasive Klebsiella pneumonia infection has been observed, mainly in the Southeast Asian population. This syndrome is associated with disseminated infection and the formation of multiple organ abscesses. Affected organs include the liver, the meninges, the brain, the eyes, and rarely the skin and soft tissue. Most of the affected patients suffer from diabetes mellitus. We present a case of invasive community-acquired Klebsiella pneumo

Validation of necrotising infection clinical composite endpoint in a retrospective cohort of patients with necrotising soft tissue infections.

INTRODUCTION: Rapidly progressive necrotising soft tissue infections (NSTIs) are associated with high mortality and morbidity. Low incidence and disease heterogeneity contribute to low event rates and inadequately powered studies. The Necrotising Infections Clinical Composite Endpoint (NICCE) provides a binary outcome with which to assess interventions for NSTIs. Partly with a view towards studies of hyperbaric oxygen treatment in NSTIs we aimed to validate NICCE in a retrospective cohort of NSTI patients. METHODS: Eligible patients were admitted between 2012 and 2021 to an adult major referr…

Management of infected fractures of the tibia with associated soft tissue loss: experience with external fixation, bone grafting and soft tissue reconstruction using pedicle muscle flaps or microvascular composite tissue grafts.

The aim of this study was to assess the results of treatment given by a team of orthopaedic and plastic surgeons in a series of infected unhealed fractures of the tibia associated with loss of adjacent soft tissues. Twenty-five lower leg fractures, treated during a 10 year period, entered the study and were grouped according to the principles of treatment followed. In the earlier Group A (nine patients), the osteosynthesis implants were retained or changed to more stable internal fixation devices, the soft tissue defects were closed by conventional muscle or musculocutaneous flaps and bone gra
