Somatization disorder (SD) patients are difficult to treat and produce negative feelings in health professionals. Smith et al.'s guidelines have demonstrated cost-effectiveness in the treatment of these patients, but family doctors consider it difficult to put these into practice in the long term. The objective of this paper is to design and assess a pilot course, based on Smith's norms, to train general practitioners for the everyday management of SD patients in primary care. We have designed a 20-h practical course, using role-playing and video recording with standardized patients, and focus

Respiratory disease in childhood is common but not all children presenting to the paediatrician have an underlying organic cause for their symptoms. This article reviews the spectrum of non-organic somatization disorders that might be encountered and advises about the diagnosis and treatment of habit cough, laryngeal dysfunction and hyperventilation.

A case of somatic delusional disorder with a severe secondary eating disorder is described. The patient's psychotic symptoms and emaciation were markedly improved with separate trials of antipsychotic and antiobsessional medications. Further epidemiologic studies of eating disorders with coexisting psychotic disorders are suggested. Classification of eating disorders to include a subset with psychotic or obsessional features (or both) may prove useful in future diagnoses. Pharmacotherapy may be a beneficial therapeutic modality in treating eating disorders with concurrent psychotic features.

Patients with somatization disorder (SD), subthreshold somatization, and somatization that is comorbid with another mental or physical disorder use a remarkably high level of general medical services. Because these patients view themselves as more seriously ill than do patients in the general medical population, their use of health care services may be as high as nine times greater than that of the general population. It is important to understand the course and prevalence of SD because 1) these patients tend to overuse health care resources and services, and 2) the associated costs are enormo

Somatization disorder (SD), a chronic psychiatric illness that affects about 1% of adult women, is characterized by multiple somatic complaints. It should be suspected in any woman who presents with a vague or complicated history; unaccountable non-responsiveness to therapy; dramatic, seductive or demanding personality style; family history of personality disorder; sexual abuse as a child; substance abuse; or depression with atypical features. Its cause is unknown, although both genetic and environmental factors have been implicated. At follow-up, patients with SD continue to have somatic symp

Individuals considered to suffer from multiple chemical sensitivities comprise a heterogeneous grouping. Regardless of the original etiology of symptoms, these individuals tend to amplify their symptoms and to develop the mistaken belief that these symptoms are indicative of severe disease. Cognitive processes and anxiety contribute to amplification of symptoms and disability, and cognitive and behavioral techniques appear to be clinically useful in alleviating symptoms and disability. Specific psychiatric disorders, in particular major depression, panic disorder, post-traumatic stress disorde

Functional or psychogenic movement disorders are common and disabling, and sometime difficult to diagnose. The history and physical exam can give positive features that will support the diagnosis, which should not be based solely on exclusion. Some clues in the history are sudden onset, intermittent time course, variability of manifestation over time, childhood trauma, history of other somatic symptom and secondary gain. Anxiety and depression are common, but not necessarily more than the general population. On examination, distraction and suggestibility may be present. There are specific sign

The background for the removing of the concept of neurosis from the American diagnosing system is discussed. Results are presented showing how cases diagnosed as neurotic depression according to ICD-9, are distributed on various DSM-III diagnoses. It appears that half of the sample is diagnosed as major depression, one-fifth as dysthymic disorder and one-fifth as depressive adjustment disorder. Concerning the delineation between different unipolar depressive diagnoses in DSM-III, results from a twin study are presented showing that many cotwins have a different depressive diagnosis than their

BACKGROUND: Children born of war are a common phenomenon of conflict. In the aftermath of World War II, more than 200,000 German occupation children (GOC) were fathered by occupation soldiers and born to local women. GOC often grew up under difficult conditions and showed high prevalence rates of mental disorders even decades later.
METHODS: Experiences of childhood maltreatment and their association with Posttraumatic Stress Disorder (PTSD), depression, and somatization in GOC (N = 146) are investigated and compared with a representative birth-cohort-matched sample (BCMS) from the German gen

The so-called obsessive-compulsive spectrum disorder (OCD spectrum) is represented by a large variety of clinical entities. There are in general obsessive-compulsive disorder, trichotillomania or Gilles de la Tourette disease among the impulse control disorders, body dysmorphic disorder and hypochondriasis among the somatoform disorders. Updated overview of the OCD spectrum through a bipolar configuration with the impulsion-compulsion (I-C) spectrum, characterized by a failure to resist and control intrusive thoughts associated with repetitive behaviors that seem to be uncontrollable or requir…

STATEMENT OF PROBLEM: Temporomandibular disorders (TMDs) affect the muscles of mastication, the temporomandibular joint, and associated structures. They are generally classified as muscular or articular and are the main cause of pain in the orofacial region, eventually causing psychological problems. However, the real burden of psychological disorders is unknown.
PURPOSE: The purpose of this systematic review and meta-analysis was to assess the prevalence of psychological disorders in patients with TMDs.
MATERIAL AND METHODS: The study was conducted through a bibliographic search carried out

Psychogenic movement disorders are a daily challenge for the neurologist. A mistake in its recognition may have important consequences for the patients. As a result, the diagnosis must be considered very carefully in clinical practice. However, psychogenic movement disorders are not unusual, are mainly tremors, and a wrong diagnosis is common. Psychogenic is an unspecific term that usually masks the real mental disorder, and should be called somatoform disorders, factitious disorders, malingering, depression, anxiety and histrionic personality disorder, although the absence of a psychiatric di…

The author lists the main theories of somatization and some of the pertinent empirical studies. The studies suggest that there is no single theory that can adequately explain somatization, which is not only multifactorially determined but is an exceedingly complex phenomenon. There is consistent empirical support for some of the theories, such as somatization being a function of depression or of anxiety, and little or inconsistent research support for some other theories, such as its being a symbolic method of communication or an attempt at conflict resolution. The contributions of the various

Atypical depression, somatoform disorder, neurasthenia and fibromyalgia seem to form a spectrum of disorders, who share a common biological basis, i.e. a reduced activity of the hypothalamus-pituitary-adrenocortical (HPA)-system. This is similar to the situation in Cushing's disease, where the central part of the hypothalamus-pituitary-adrenocortical-system is decreased by an increased feedback via increased intracerebral cortisol concentration. Cushing's disease is accompanied by features of atypical depression and of somatisation. Treatment with hypericum seems to disinhibit the hypothalamus

OBJECTIVE: To identify facilitating and hindering factors in the treatment of persistent somatic symptoms (PSS) in migrants in psychiatry and other health care settings in Western countries.
METHODS: A scoping literature review was conducted by searching PubMed and Embase, using combinations of search terms related to the treatment of PSS in migrants. Studies outside of the scope of current guidelines or limited to specific underlying diseases such as post-traumatic stress disorder (PTSD) or consequences of torture were excluded. Two independent reviewers verified that the studies met the inc

The classification of medically unexplained symptoms (MUS) could have negative consequences for patients with functional somatic syndromes (FSS). By grouping related but distinct syndromes into one label, the MUS classification fails to inform clinicians about their patients' health condition. In research settings, the MUS classification makes patient samples more heterogeneous, obstructing research into the underlying pathology of FSS. Long-term studies have shown that MUS are often appraised as medically explained symptoms at follow-up and vice versa, raising doubts about the reliability of

Pseudoseizures are paroxysmal alterations in behavior that resemble seizures but are without any organic cause. They are recognized by various terms. Pseudoseizures are found in about one fourth of all patients seen with hysteria and 20% of those referred to epilepsy clinic. Pseudoseizures are often difficult to differentiate because there are client based or clinician based factors leading to misdiagnosis. Detailed history, observation, psychological testing and laboratory investigations are used for correct diagnosis. Pseudoseizures are not only to be differentiated from various forms of epi
