Infantile myofibromatosis with orbital involvement: a case presentation and review of the literature.

Infantile myofibromatosis is a rare fibrous tumor that typically presents during infancy or early childhood. We describe a 4-year-old boy with a history of multicentric infantile myofibromatosis who presented with new-onset right-sided proptosis. Magnetic resonance imaging (MRI) demonstrated an expansile intracranial lesion arising from the greater wing of the sphenoid bone, producing mass effect at the right orbital apex and compressing the optic nerve. Histopathologic examination of a biopsy specimen confirmed a myofibroma with associated reactive bone changes. Over a 36-month follow-up peri…

A Rare Case of Mydriasis as an Immediate Complication After Orthognathic Surgery in a Patient with Cleft Lip and Palate.

This study aims to report a rare complication, anisocoria (mydriasis), in a patient with cleft lip and palate in the immediate postoperative period after orthognathic surgery and discuss the literature surrounding this event. Mydriasis after orthognathic surgery may be related to the separation of the pterygoid plate from the sphenoid bone, which can injure the cranial nerves. Therefore, knowledge of possible anatomical variations of the posterior region of the maxilla becomes extremely important to minimize complications in cleft patients. The reported case highlights the management of anisoc…

Prenatal diagnosis of a transsphenoidal meningoencephalocele.

This is probably the first case of a prenatally diagnosed transsphenoidal meningoencephalocoele. A protruding cystic mouth mass was detected at 21 weeks' gestation in conjunction with a palatal defect and a suspicion of herniation of the posterior fossa. Amniocentesis revealed a normal molecular karyotype and normal whole-exome analysis. Foetal MRI confirmed the presence of a sphenoidal bone defect and herniation of a meningeal sac containing parts of the cerebellum and medulla. Due to the poor prognosis, the patient opted for a termination of pregnancy but agreed to a full autopsy and postmor

Superior Orbital Fissure Syndrome in a Case of Breast Cancer.

Metastasis from breast cancer to the head and neck region is a rare occurrence. However, of all the cancers that metastasise to the orbit, breast cancer is the leading cause in women. Usually, breast cancer metastasises to the fat and muscles in the orbit, and involvement of the bony orbital wall is rare. We report a case of a 52-year-old woman who presented with metastasis to the left greater wing of the sphenoid and adjacent squamous temporal bone. Ophthalmoplegia and proptosis were the initial manifestations in this patient, which lead to the diagnosis of the underlying breast cancer.

[The Interesting Case No. 42. Differential diagnosis of epistaxis].

Epistaxis is a symptom and one of the most frequent medical emergencies. In most cases haemorrhages concern the anterior parts of the septum, in particular the Locus Kiesselbachi. Thus they are harmless and therapy is easy to handle. We report a case of a 55-year-old lady with relapsing epistaxis due to a pseudoaneurysm after surgery of a meningioma of the sphenoid bone. This type of epistaxis is rare and may culminate into a life-threatening event. The case demonstrates the importance of an exact differential diagnostic evaluation by use of modern imaging techniques for severe and life-threat…

Transient abducens nerve palsy following a Le Fort I maxillary osteotomy: report of a case.

A case of transient abducens nerve palsy following Le Fort I maxillary osteotomy is reported. The results of the CT scan strongly suggest the cause was a fracture of the body of the sphenoid bone but the cause of the fracture is unclear. The palsy occurred on the first postoperative day and recovery took approximately 5 months. The most likely explanation for the complication is a transmittal of force from the osteotome used to fracture through the pterygoid plates extending superiorly through the medial surface of the cavernous sinus. This case clearly demonstrates the importance of care in p…

Management of Recurrent Cerebrospinal Fluid Rhinorrhea Caused by Sequential, Anatomically Separated Skull Base Defects-A Case-Based Systematic Review.

OBJECTIVE: Recurrent cerebrospinal fluid (CSF) rhinorrhea caused by sequential, anatomically separated skull base defects is rarely reported … good in 8 of 10 cases. The most common location for primary and sequential CSF leaks was along the sphenoid bone (4/10 and 5/10 patients, respectively). All publications except one reported the presence of a meningo (encephalo)cele as cause of the sequential CSF leak. CONCLUSIONS: Occurrence of recurrent CSF rhinorrhea due to an anatomically separated sequent …

Heterotopic Neuroglial Tissue in Sphenoid Ridge.

BACKGROUND: Extracerebral neuroglial heterotopias are rare manifestations of cerebral tissue outside the brain whose most common form is the so-called nasal glioma. In this case report we illustrate the first case of heterotopic neuroglial tissue within the bone of the skull. CASE DESCRIPTION: Our patient underwent surgery for a sphenoid ridge meningioma. Aside from the expected meningioma, histopathologic examination showed a small amount of intraosseous heterotopic neuroglial tissue. CONCLUSIONS: The pathogenesis of cerebral heterotopias is diverse. Most of the midline lesions are probably

[Styloid syndrome; Sluder's syndrome; Charlin's syndrome].

The styloid syndrome is caused by an irritation of the glossopharyngeal nerve from an excessive development of the styloid apophysis. Treatment consists of the resection of the same. The Sluder syndrome represents a nevralgy with origin in the sphenopalatine ganglion and a dysfunction of the parasympathetic system. The Charlin syndrome, much less frequent, manifests itself by pains in the territory of the nasociliar nerve and the ciliar ganglion.

The Tessier number 9 cleft.

The clinical, radiologic, and operative examination of two patients with the Tessier number 9 cleft has allowed the first complete description of this, the rarest of the rare craniofacial clefts. Soft-tissue disruption extends from the lateral third of the upper eyelid through a distorted lateral end of the eyebrow to the temporal scalp, with an associated hairline indicator. The skeletal disturbance similarly radiates superolaterally from the superolateral orbital rim across the greater wing of the splenoid to the upper squamous temporal. The three-dimensional extent of the bony disruption is

Spheno-Orbital Meningioma: Surgical Outcomes and Management of Recurrence.

OBJECTIVE: Spheno-orbital meningioma (SOM) is a rare intracranial tumor that arises at the sphenoid wing, extends into the orbit, and is associated with hyperostosis of the sphenoid bone. These tumors often invade important neurovascular structures around the orbital apex, superior orbital fissure, and cavernous sinus. Aggressive tumor removal could achieve acceptable control; however, residual tumor can regrow. In this article, our surgical management and long-term outcomes are described. METHODS: Retrospectively, 12 patients with SOM who were treated surgically over a 21-year period were in…

Sphenoid "drill-out" for chronic sphenoid rhinosinusitis.

BACKGROUND: Chronic sphenoid rhinosinusitis (CSR) refractory to both medical management and 1 or more sphenoidotomies is a difficult entity to treat. In contrast to the surgical hierarchy that exists for the frontal sinus, there is no systematic approach for addressing persistent disease in the sphenoid. Sphenoid marsupialization has been advocated as a method of addressing recurrent sphenoid sinusitis. OBJECTIVE: We present a new technique called the sphenoid drill-out, which we place between traditional sphenoidotomy and sphenoid marsupialization in the surgical hierarchy for management of

Endoscopic extradural supraorbital approach to the temporal pole and adjacent area: technical note.

The authors' initial experience with the endoscopic extradural supraorbital approach to the temporal pole and adjacent area is reported. Fully endoscopic surgery using the extradural space via a supraorbital keyhole was performed for tumors in or around the temporal pole, including temporal pole cavernous angioma, sphenoid ridge meningioma, and cavernous sinus pituitary adenoma, mainly using 4-mm, 0° and 30° endoscopes and single-shaft instruments. After making a supraorbital keyhole, a 4-mm, 30° endoscope was advanced into the extradural space of the anterior cranial fossa during lifting of t

Bony abnormalities of the paranasal sinuses in patients with Wegener's granulomatosis.

Wegener's granulomatosis is a systemic vasculitis that can affect any organ system, but primarily involves the upper and lower respiratory tracts and the kidneys. Chronic sinusitis is a well-known clinical feature of the disease. Mucosal abnormalities of the nose and paranasal sinuses have been well-characterized and range from granulomatous lesions to diffuse mucosal thickening. In contrast, abnormalities of the underlying bone of the paranasal sinuses in this disease have not been well-described, and reports have been limited. To characterize bony abnormalities of the nose and paranasal sinu
