Transient but very intense oedema of the cervical spinal cord was observed in two patients with obstruction of the cerebrospinal fluid (CSF) pathways. Both presented with hydrocephalus, one due to an infratentorial obstructing mass and the other due to postmeningitic adhesive obstruction of the outlet foramina of the fourth ventricle. In animal experiments with obstruction of CSF pathways (due to outlet foramina obstruction or to downward tentorial herniation) flattening and stretching of the ependymal cells along the central canal is observed, followed by disruption and splitting of the epend…

Secondary tethering of the spinal cord in people with myelomeningocele (MMC) is almost universal but there is a relatively low incidence of the secondary tethered cord syndrome (S-TCS). In view of this, we wish to explore the notion that cord pulsation, as demonstrated by ultrasound of the spinal cord, can become an independent measure for increased tension of the spinal cord in patients with MMC. One-hundred and six patients with MMC underwent ultrasonographic examination of their back, a thorough physical examination, and a careful review of their clinical history. Of these patients, 53 had …

In order to study the changes that occur in the spinal cords of the aged, 19 centenarian spinal cords from two men and 17 women (age range, 100-116 years at death; mean, 103 years) were pathologically examined. Cross-sections at each segmental level of the 19 autopsied spinal cords were examined for histopathological changes. The cross-sectional area and flattening ratio at the level of the C7 segment were measured. The size of the cords had some negative correlation with age, but the individual variation was considerably large. Among the 19 centenarians, macroscopic anteroposterior flattening…

The authors present 3 rare cases of metastases to the spinal cord and cauda equina. In 2 cases the tumours were of epithelial, and in the third neuroepithelial origin. In one case the tumour was localized in the thoracic segment of spinal cord, and in the second case in the conus and cauda equina, whereas in the third at the cauda equina. The diagnosis was established by CT and myelography. Total removal of the tumour in the cauda equina was carried out. The most significant neurologic symptoms in the two cases were pain and paresis in one case. In our series the rate of metastases to intramed…

The authors experienced a case of idiopathic spinal cord herniation with duplicated dura mater. A 63-year-old woman presented right dominant slowly progressive spastic paraplegia and dissociated sensory disturbance. Magnetic resonance imaging (MRI) demonstrated an enlarged dorsal arachnoid space associated with an apparently focally narrowed thoracic cord. The cord was kinked towards the anterior and closely applied to vertebral body at the level of Th3-4. Computed tomographic myelography (CTM) revealed homogeneous filling at dorsal arachnoid space immediately after injection and no defects. A…

The syndrome of syringomyelia in children has been extensively described in the literature and is said to most commonly involve the cervical and thoracic spinal cord. We here present two children who had an unusual constellation of signs and symptoms, characterized by bowel and bladder disturbance in one, and in the other with intermittent paroxysmal severe pain and cramping and flexion of the lower extremities accompanied at times by episodes of incontinence. Both were found on magnetic resonance imaging (MRI) to have a syrinx localized to the most distal cord. This was not associated with Ar…

Cervical spina bifida, either occult or overt, is relatively uncommon. These lesions tend to be associated with other abnormalities of the c … the occult dysraphic states and spina bifida cystica, there is underlying tethering of the cervical spinal cord, which can cause late neurologic deterioration, and surgical treatment is indicated both for cosmetic reasons and to untether the spinal cord. Children with spina bifida cystica may have neurologic dysfunction that may not be evident at birth or in …

We describe a 31-year-old diabetic man, with granulomatous angiitis confined to the spinal cord, who developed rapidly progressive spastic paraplegia, clinically interpreted as being secondary to a spinal cord infarct. At the time of autopsy, vasculitis was limited to the spinal cord, without involvement of cerebral vessels. The inflammatory cells were predominantly CD4+ T lymphocytes, with few CD8+ T and B lymphocytes. The phenotypical composition of the inflammatory infiltrate is similar to that described in other granulomatous disorders such as sarcoidosis and tuberculin reaction.

Spinal dysraphism is relatively common in children and includes a wide spectrum of congenital anomalies in the normal closure of the posterior elements of the spine. The prognosis will depend mostly on early diagnosis and treatment. Occult spinal dysraphism may present without external anomalies and the diagnosis could be suspected lately, when neurological symptoms are present and often irreversible. Occult spinal dysraphism is frequently associated to a tethered cord, most commonly secondary to the presence of a lipoma. Ultrasonography has been proven highly sensitive in the detection of int

The author describes one-stage investigation of the epidural and preneural spaces at the lumbar level using gas and a needle for lumbar puncture with a longitudinal side hole at the working end with subsequent roentgenography (pneumospondylography). Altogether 140 patients with lumbar vertebral osteochondrosis with neurological disorders were examined. Pneumospondylography gives 3 levels of contrast and ensures information on the sizes of the epidural and arachnoidal spaces, function of the invertebral holes and paravertebral tissues. The phenomenon of differentiation of the dural sac wall hel

The tethered spinal cord is an entity with potentially grave neurourological implications. Presenting urological symptoms include incontinence, symptomatic urinary tract infections and stool soiling. These symptoms may be the first to herald a tethered cord. The patients with previously repaired myelodysplastic defects are particularly prone to development of this lesion. We correlated urological history with preoperative and postoperative cystometry in 20 myelodysplastic children. In this study we noted that clinical symptomatology and urodynamic parameters improved in 60 per cent of those ch…

Everything that is living needs zinc. Because of its multiple cellular functions, many symptoms and signs have been associated with zinc deficiency in humans including poor wound healing, rashes, diarrhea, decreased sexual function, and growth retardation. These problems are usually caused by other factors and indiscriminate use of zinc supplements is associated with some risk. Because inaccurate diagnosis can prevent alternative appropriate therapy and evidence is lacking that zinc supplementation beyond normal levels is generally beneficial, zinc status should be assayed prior to its adminis

Two cases of tetraparesis due to cervico-occipital spinal cord compression in infants with achondroplasia are reported. Neurological disorders in achondroplasia are produced by structural anomalies of the cranium and spinal canal. They can be divided into two categories: hydrocephalus and radicular and spinal cord compression syndromes. Cervico-occipital compression is more frequent in childhood and may occur very early, in the first months of life. Early diagnosis of this complication and anatomic evaluation can now be obtained by computed tomography, which shows the exact dimensions of the f…

The authors report the radiographic and pathologic findings in 10 Great Dane dogs with the wobbler syndrome. In all 10 dogs it was possible to demonstrate myelographically that there was cervical spinal cord compression at 1 or 2 sites. The spinal cord compression was mainly dynamic in nature, as degree of compression increased in extension and decreased in flexion of the neck in 8 dogs. In 1 dog with deformed vertebral bodies (G6 and C7), compression increased slightly in flexion of the neck. In another dog, compression was lateral and could only be seen in the ventrodorsal view. The macrosco…
