Three-dimensional ultrasonography: Advancing the clinical evaluation of adolescent idiopathic scoliosis.

The clinical efficacy of 3D ultrasound for spinal curvature assessment was evaluated in adolescent idiopathic scoliosis (AIS) patients through comparative analysis with conventional radiography. A prospective cohort of 230 preoperative and 22 postoperative AIS patients was evaluated over a 24-month period. Spinal curvature assessments were performed using a standardized 3D ultrasound system (SCN801), with reliability quantified via intraclass correlation coefficients (ICC) and validity assessed through Spearman correlation analysis. Ultrasound measurements demonstrated good reliability (ICC >

Postoperative Change of Thoracic Kyphosis after Corrective Surgery for Adult Spinal Deformity.

INTRODUCTION: Correction of lumbar lordosis is the primary goal of surgical treatment of adult spinal deformity. However, only limited research has evaluated the effects of this correction on the adaptive curvature of the thoracic spine. The purpose of this study is to evaluate the change in thoracic curvature after corrective surgery to restore lumbar lordosis in patients with adult spinal deformity. METHODS: We completed a retrospective analysis of the radiological data of 65 patients, ≥50 years old, who underwent corrective surgery of lumbar spine lordosis from any level below T8 to the il

Postural Rehabilitation for Adolescent Idiopathic Scoliosis during Growth.

Long-term follow-up of untreated patients with adolescent idiopathic scoliosis (AIS) indicates that, with the exception of some extremely severe cases, AIS does not have a significant impact on quality of life and does not result in dire consequences. In view of the relatively benign nature of AIS and the long-term complications of surgery, the indications for treatment should be reviewed. Furthermore, recent studies have shown that scoliosis-specific exercises focusing on postural rehabilitation can positively influence the spinal curvatures in growing adolescents. Experiential postural re-ed…

Spinal deformity in children treated for neuroblastoma.

Of seventy-four children who were treated at a mean age of seventeen months for neuroblastoma and survived more than five years, fifty-six (76 per cent) had spinal deformity due either to the disease or to the treatment after a mean follow-up of 12.9 years. Of these fifty-six, 50 per cent had post-radiation scoliosis (mean, 18 degrees; range, 5 to 79 degrees), and 16 per cent had post-radiation kyphosis, most frequently at the thoracolumbar junction (mean, 39 degrees; range, 13 to 61 degrees), at the time of follow-up. Two kyphotic thoracolumbar curve patterns were identified: (1) an angular k

Beyond the Curve: Intra-spinal Rib Head Herniation as a Critical Complication of Dystrophic Scoliosis in Pediatric Patients With Neurofibromatosis Type 1.

Neurofibromatosis type 1 (NF1) is frequently associated with a multitude of skeletal abnormalities including dystrophic scoliosis. A rare but severe complication of dystrophic scoliosis in NF1 is the herniation of rib heads into the spinal canal, potentially leading to devastating spinal cord compression. We present two pediatric cases of NF1-associated dystrophic scoliosis with intra-spinal herniation of rib heads. Case 1 involves a teenage male with progressive thoracolumbar scoliosis and protrusion of T10 and T11 rib heads into the spinal canal, who underwent successful posterior spinal fus

Cervical corpectomy and plate fixation for postlaminectomy kyphosis.

Between 1987 and 1991, 20 patients with symptomatic postlaminectomy kyphosis were treated with anterior decompression, bone graft, and anterior cervical plate. The patients were predominantly male (14:6) with a mean age of 58 years. The initial laminectomy was performed for either spondylosis (80%) or spinal tumor (20%). All patients had anterior compressive pathology, which was associated with instability (45%), neck pain (75%), myeloradiculopathy (90%), or severe neck deformity (30%). The mean degree of kyphosis was 38 degrees. Treatment consisted of a trial of cervical traction (75%), anter

Strategies for Minimizing the Effects of Observer Variability on Sagittal Parameter Measurements of the Spine.

STUDY DESIGN: Exploratory-descriptive study. OBJECTIVE: Evaluate the influence of observer variability on the measurements of both thoracic kyphosis (TK) and lumbar lordosis (LL) obtained with anatomic and functional spinal segmentation methods. BACKGROUND: Parametric analysis for spinal surgery planning typically relies on anatomic parameters. However, incorporating functional parameters that consider the vertebrae orientation is important to minimizing surgical calculation errors. METHODS: The authors developed parametric analysis software that integrates traditional and functional method

Deep learning based vertebra localization and Cobb angle estimation using x-ray images.

PURPOSE: Scoliosis (a spinal ailment) with improper lateral curvature and rotational abnormalities may have a significant impact on the general health and physical development of a person. For thediagnosis of this problem, Cobb angle measurement is required from Anterior-Posterior (AP) X-ray images. At present, a manual estimate is performed, which is time-consuming and prone to error.Usingthis dual techniques, the model is able to collect both structural and edge details information, and the IEM is able to combine these characteristics to improve vertebra localisation and tilt estimates. Mechanical analysis of deep tissue injury during sitting in patients with spinal cord injury via parametric finite element model.

Spinal cord injury patients are prone to develop deep tissue injury because of long-term mechanical load. However, there is a lack of statistical research on the influence of tissue characteristics on the internal mechanical state of soft tissue. This study aimed to investigate the influence of tissue characteristics on the internal mechanical state of buttock in spinal cord injury patients. A three-dimensional reference buttock model was established and a visualization program was generated to modify the parameter values. Through changing the muscle atrophy, body mass index and the radius of

The impact of anthropometric measures on plantar pressure distribution in male handball players and non-athletes: A cross-sectional study.

BACKGROUND: Increasing body anthropometry brings substantial spinal stress, which influences the spinal curvatures; this in turn may affect the foot plantar pressure distribution. OBJECTIVES: This study investigated the impact of body anthropometry on static plantar pressure distribution and their relationship among handball players and non-athletes subjects. METHODS: Thirty handball players aged from 21 to 26 years, and thirty age-matched non-athletes subjects aged from 21 to 28 years participated in this study. The spinal lordosis and kyphosis angles, trunk length, pelvic tilting, and pelv…

Three-Dimensional Measurement and Analysis of Rod Deformation for Spinal Deformity Correction: A Novel Methodology.

The amount of rod contour is a critical factor that influences sagittal and coronal alignment for posterior spinal fusion. Distraction techniques to correct spinal deformity, which are often three-dimensional (3D) in nature, are known to cause rod deformation from the ideal, planned rod curvature. Therefore, surgeons often overbend rods to account for this phenomenon. Currently, there is no agreed-upon method to scientifically quantify rod unbending, and no methods offer a 3D analysis. The proposed methodology addresses this gap. By aligning the 3D rod centerlines, the segmented area enclosed

Scoliosis associated with osteogenesis imperfecta.

A survey was conducted to document the results of bracing and spinal fusion for scoliosis associated with osteogenesis imperfecta. Observations were made of 121 patients who underwent treatment by bracing or spinal fusion and who had been treated by 51 orthopaedic surgeons in 14 countries. The average curve before bracing measured 43 degrees. The braces were ineffective in stopping progression even in small curves. We were unable to determine whether braces slowed the rate of progression of curvature. The average age at fusion was 15 years 7 months, the average curve before operation measured

[Diastematomelia (author's transl)].

21 cases have been observed and followed up for 10 years. All were children. Three types of anomaly were noted: Spinal curvature (mostly scoliosis), skin abnormalities in the mid line (hypertrichosis, angioma, skin dimples) and problems in the lower limbs (varus deformities of the feet, leg length discrepancy, muscular atrophy, neurological disorders). These signs, even if not all are present, must lead to a study of the vertebral bodies and to gas myelography whcih demonstrates a double spinal cord over a variable length from 2 to 14 segments. 15 children were treated surgically by mobilisati

A Framework for Patient-Specific Spinal Intervention Simulation: Application to Lumbar Spinal Durotomy Repair.

We present a functional and patient-specific lumbar phantom for the training of spinal durotomy and dura closure under microscopic view, consisting of a lumbar model, pressurized dural surrogate, together immersed in a tissue-mimicking layer simulating fat, muscle and skin. The lumbar model was derived from a patient computed tomography scan, preserving the natural shape and curvature of the lumbar column. The inclusion of the simulated soft-tissue layer was critical for preserving the surgical ergonomics and presented a realistic view under the surgical microscope. As the success of dura repa

S3 Sacral-Alar Iliac Screw: A Salvage Technique for Pelvic Fixation in Complex Deformity Surgery.

Sacral-alar iliac (SAI) screws constitute a relatively new technique for pelvic fixation. Since their initial description in 2007, SAI screws have gained wide popularity among the spine surgery community. In 2013, we first described the possibility of using both S1 and S2 SAI screws for pelvic fixation in revision surgeries for adult degenerative scoliosis. Although a previous radiological study has suggested the feasibility of S3 and S4 SAI screws, to the best of our knowledge, there has been no report in the literature on the clinical use of such techniques. In this brief technical note, we
