We describe a case of dumbbell hydatid cyst of the T(10) pedicle. This cyst had both extradural and retroperitoneal components, leading to spinal cord compression and abdominal pain. Following surgical excision and irrigation with hypertonic saline the patient made a full recovery, and is recurrence-free after 18 months follow-up.

An overview of MR imaging of the pediatric spine is presented with a systematic approach to the analysis of spinal dysraphic disorders. An introduction to the most commonly used MR imaging techniques, with an emphasis on the newer pulse sequence of cine MR flow of the cerebrospinal fluid and embryogenesis of the spine and spinal cord, provides the orientation for diagnosis of this common pediatric disorder.

The vertebral abnormalities present in 35 patients diagnosed with Goldenhar's syndrome are reported. There was no absolute correlation between the presence of any of the formative or segmentation vertebral defects and that of other concomitant malformations. This study supports the conclusion that patients with various groupings of Goldenhar-related anomalies should be considered as a single entity, to which the name Goldenhar association could be applied.

A wide variety of lumbar spinal conditions have been managed by spinal fusion. Results appear better when the diagnosis is specific and related to definable instability or deformity in patients with a stable psychologic state. Success rates are higher in isthmic spondylolisthesis, unstable spinal stenosis syndromes (degenerative spondylolisthesis, degenerative scoliosis), and in patients with objective segmental instability. Variable success rates are reported for disk-related low back pain conditions and in patients with failed previous surgery. Instrumentation appears to be beneficial in sit

A wide variety of congenital pathology affects the cervical spine and underlying spinal cord. This article discusses commonly encountered conditions under the categories of osseous anomalies, Chiari malformations, dysraphic conditions, and skeletal dysplasias. Osseous anomalies tend to involve the upper cervical spine to a greater degree than the lower cervical spine. Chiari malformations warrant inclusion on the basis that they have many manifestations within the cervical spine. Dysraphic conditions resulting from failure of either primary or secondary neurulation are distinctly uncommon in t…

We analyzed the prevalence, inheritance, progression, and functional implications of spinal deformity in Marfan syndrome using four different groups of patients. We studied 113 patients who had Marfan syndrome, eighty-two of whom were skeletally immature, in order to characterize the alignment and function of the spine. The patients were selected from a clinic that provides total care with no bias toward the presence of orthopaedic conditions. Scoliosis was identified in fifty-two of the eighty-two patients, and the prevalences for the sexes were equal. The thoracic portion of the curve was co…

Spinal tuberculosis, the most common form of skeletal involvement, is increasing in prevalence because of the resurgence of tuberculosis dur … ollapse, and extension into the spinal epidural space. Significant instability and deformity of the spine can result, mandating prompt diagnosis and treatment to prevent permanent neurologic damage. The purpose of this essay is to illustrate the broad spectrum of imaging findings on plain radiographs, bone scans, CT scans, myelograms, and MR images of p …

Bone changes in sickle cell disease occur due to marrow hyperplasia, tissue ischaemia and infarction due to vaso-occlusion. Between 1982 and … on seen in 8 (24%) patients; the majority needed anterolateral decompression and bone grafting. The spine is often affected in sickle cell disease and aggressive treatment with close follow-up is required to avoid disabling complications.

OBJECTIVE: To present an uncommon case of an isolated lamina fracture in the cervical spine found by taking stress films when a routine cervical series failed to demonstrate it.
CLINICAL FEATURES: A 63-yr-old male was seen after suffering from mild neck pain and stiffness of 4 days' duration after a car accident. A review of emergency room X-rays demonstrated a small fragment of bone adjacent to the anterior superior end-plate of C6. Flexion/extension stress views were then done that demonstrated anterior slippage of C5 on C6 during flexion, and a cervical lamina fracture of C5 was diagnosed …

Fracture of the intercondylar eminence of the tibia in childhood is a relatively common injury, but to our knowledge a bilateral case has never been reported. A patient with bilateral fractures is presented and the literature is reviewed. An 8-year-old girl fell from a tree and sustained bilateral fractures of the tibial spine. According to Meyers' and McKeever's classification normal x rays revealed a type III fracture in the left knee and a type II fracture in the right knee. The left knee was treated by open reduction and internal fixation, and the right knee was treated conservatively. At …

Operative management of the patient with cervical spinal instability depends on the specific anatomy and location of the injury. The unique features of the atlanto-axial complex require stabilizing procedures that are quite different from those used in the subaxial spine. A clear idea of the etiology of the instability, a definition of the pathological anatomy, and knowledge of the presence and extent of neural compression are essential for optimizing a successful outcome. In some instances the clinical circumstances mandate the use of an anterior approach, and in others a posterior operation …

Disc herniation in athletes is uncommon; however, it requires prompt diagnosis and treatment. Arthroscopic microdiscectomy is associated with minimal postoperative morbidity and it is particularly suitable for the surgical treatment of unremitting radiculopathy associated with herniated lumbar disc in athletes. The addition of the arthroscope and the deflecting instruments allows for visualization, identification, and extraction of posterior and posterolateral collagenized nuclear fragments that are believed to be the common cause of nerve root compression and sciatica following disc herniatio

Radiographs of the cervical spine--in many cases longitudinal--were available for study in 68 cases of Apert syndrome. Autopsy material was available in one of these cases, and a 3-dimensional reconstruction from a CT scan was also studied in one case. Variable degrees of fusion were observed, involving the articular facets, the neural arch or transverse processes, or block fusion of the vertebral bodies. Ossification may not always be evident in some early radiographs. However, early radiographic signs of impending fusion may be irregularity in vertical orientation of the vertebral bodies and…

The author investigated the firmness and elasticity of the lower lumbar vertebrae and intervertebral discs. They confirmed that the interver … intervertebral disc is torn from the body of the vertebra. The most vulnerable place of the lumbar spine is thus the connection of the vertebra and the intervertebral disc, the site which is also the borderline of the vascular supply which then changes into diffuse supply. Strain at the borderline of firmness or above it can cause degeneration of the i …

Eleven patients with spinal osteoid osteoma and six patients with spinal osteoblastoma treated between 1975 and 1990 were reviewed to characterize the tumors as they affect the spine and to define the important differences between the two tumors. All patients with cervical osteoid osteoma presented with pain, limited range of motion of the neck, and torticollis. Four osteoblastomas had soft-tissue components in the epidural space, necessitating dissection of the tumor from the dura. No soft-tissue component was found in any of the osteoid osteomas. Our results were similar to a metaanalysis of…

Imaging studies and operative records of 55 patients who had undergone lumbar pseudarthrosis repair were studied, providing a total of 85 surgically documented pseudarthroses. Preoperative imaging studies were carefully analyzed for the anatomic characteristics of the involved levels and the morphology of the fusion mass. The pseudarthroses were found to fall into four distinct morphologic categories: atrophic, transverse, shingle, and complex. The atrophic pseudarthrosis type was noted to be strongly associated with the presence of intact facet joints or spinal hardware. This retrospective an
