AIM: Procalcitonin is a marker of bacterial and fungal infection and sepsis. The present study evaluated the relationship between serum procalcitonin levels and disease activity in patients with ankylosing spondylitis (AS).

New emerging drugs in the treatment of ankylosing spondylitis (AS), and spondyloarthritis in general, should be compared to anti-TNF agents, which provided clear evidence of efficacy in these conditions. To date, other biologic agents used in rheumatoid arthritis failed to demonstrate efficacy in AS, even in anti-TNF naïve patients. Some new potential options may target cytokines such as IL-17, or molecules involved in entheseal ossification or signaling pathways, but need confirmatory evaluation.

Ankylosing spondylitis (AS) is a systemic inflammatory rheumatic disease characterized primarily by axial joint involvement, sacroiliitis and various extra-articular manifestations. High cardiovascular mortality in AS has led many researchers to investigate possible risk factors involved with cardiovascular disease in these patients. This review summarizes published data concerning endothelial dysfunction and atherosclerosis in patients with AS. The author discusses current limitations and problems related to a better assessment of these two possible changes in AS.

Cryptosporidiosis is a parasitic disease caused by a protozoan called Cryptosporidium sp. An increased number of diagnoses were made in the last 20 years, especially in patients with immunodeficiency like the acquired human immunodeficiency syndrome and induced immunodeficiency, such as in transplant patients and those who need frequent hemodialysis, has been observed. We report the case of a young patient with ankylosing spondylitis treated with adalimumab who developed chronic diarrhea secondary to cryptosporidiosis.

We present a 57-year-old man with ankylosing spondylitis (AS) and significant dorsal lumbar dural ectasia (diverticulum). An L5 nerve root monoradiculopathy associated with AS has not been previously reported. The quantity and extent of such ectasia is variable but may be related to cauda equina syndrome. Long-term follow-up of these cases may reveal whether or not solitary nerve lesions gradually lead to cauda equina syndrome. We recommend that asymptomatic or symptomatic patients with dural ectasia should be closely observed without need for immediate surgical intervention.
