Pulmonary Infections and Surgical Complications in a Young Girl with Signal Transducer and Activator of Transcription 3 Loss-of-Function Mutation Hyperimmunoglobulin E Syndrome: A Case Report.

Introduction: Hyperimmunoglobulin E syndromes (HIESs) are characterized by a high serum immunoglobulin E (IgE) level, eczematoid rashes, recurrent staphylococcal skin abscesses, and recurrent pneumonia and pneumatocele formation. Autosomal dominant HIES is the most common form of HIES and mainly occurs due to loss-of-function mutations in the Signal Transducer and Activator of Transcription 3 (STAT3) gene (STAT3 LOF). Case Presentation: We report the case of an 11-year-old Peruvian girl diagnosed with STAT3 LOF caused by p.R382W mutation. She presented with recurrent staphylococcal pneumonia a

A human monoclonal antibody targeting the conserved staphylococcal antigen IsaA protects mice against Staphylococcus aureus bacteremia.

Due to substantial therapy failure and the emergence of antibiotic-resistant Staphylococcus aureus strains, alternatives for antibiotic treatment of S. aureus infections are urgently needed. Passive immunization using S. aureus-specific monoclonal antibodies (mAb) could be such an alternative to prevent and treat severe S. aureus infections. The invariantly expressed immunodominant staphylococcal antigen A (IsaA) is a promising target for passive immunization. Here we report the development of the human anti-IsaA IgG1 mAb 1D9, which was shown to bind to all 26 S. aureus isolates tested. These

Clinical Risk Factors for Infective Endocarditis in Staphylococcus aureus Bacteremia.

Crucial to the management of staphylococcal bacteremia is an accurate evaluation of associated endocarditis, which has both therapeutic and prognostic implications. Because the clinical presentation of endocarditis can be nonspecific, the judicious use of echocardiography is important in distinguishing patients at high risk of developing endocarditis. In the presence of high-risk clinical features, an early transesophageal echocardiogram is warranted without prior transthoracic echocardiography. The purpose of this study was to investigate the clinical risk factors for staphylococcal infective

Management of Acute Hematogenous Infection Following Total Knee Arthroplasty: A Case Series of 11 Patients.

OBJECTIVE: Acute hematogenous infection is a devastating complication that can occur after total knee arthroplasty (TKA). The best strategies for management of this infection remain controversial. Two-stage revision has been well described as the gold standard for the management of chronic late infections. However, there is a paucity of information presently available on the management and outcomes of patients treated for acute hematogenous infections. The purpose of the present study was to report the outcome of acute hematogenous infections following TKA with the treatment of irrigation, deb

Toxic shock syndrome after pilonidal cystectomy. Report of a case.

Toxic shock syndrome is an uncommon disease associated with staphylococcal infections. Although most frequently reported in menstruating women and associated with tampon use, toxic shock syndrome has been described following many types of surgical procedures. In this report we describe a case of toxic shock syndrome occurring in a previously healthy young man after elective surgery for a pilonidal cyst.

Evaluation of topical povidone-iodine versus gentamycin in staphylococcus coagulase positive corneal ulcers--an experimental study.

Povidone iodine was found highly effective in controlling the infection and reducing the period of morbidity caused by experimentally produced staphylococcal coagulase positive corneal ulcers in 50 eyes of rabbits in comparison to Gentamycin sulphate. Povidone-iodine was also found to be safe, without any adverse reaction. Hence it can be considered for use in human beings for effective control of corneal infections.

Immunologic changes in healthy probands and HIV infected patients after oral administration of Staphylococcus aureus 12c: a pilot study.

The tumoricidal and antiviral effects of Staphylococcal toxins are well documented. In a preliminary study we investigated the immune modulating properties of these toxins by administering single oral doses of a 12c potency of a lysate of Staphylococcus aureus Cowan I, to 4 healthy probands and 12 HIV infected patients with clinical symptoms. We observed a decrease of circulating immune complexes in the healthy probands as well as in the HIV positive patients, accompanied in the latter by a significant increase of CD4 lymphocytes, CD4/CD8-ratio and an improvement of the HIV related symptoms. Erythromycin: a microbial and clinical perspective after 30 years of clinical use (2).

Erythromycin, first introduced for clinical use 30 years ago, was found to be effective for the treatment of gram-positive bacterial infections. Emergence of resistance and the advent of penicillinase-resistant penicillins limited the use of erythromycin for serious staphylococcal infections; however, erythromycin remains among the drugs of choice for the treatment of acne, infections of the skin and soft tissues, streptococcal pharyngitis, bronchitis, pneumonitis, diphtheria, carriers of pertussis, and, when administered with a sulfonamide, otitis media. Erythromycin is the drug of choice for…

Novel Insights into the Classification of Staphylococcal β-Lactamases in Relation to the Cefazolin Inoculum Effect.

Cefazolin has become a prominent therapy for methicillin-susceptible Staphylococcus aureus (MSSA) infections. However, an important concern is the cefazolin inoculum effect (CzIE), a phenomenon mediated by staphylococcal β-lactamases. Four variants of staphylococcal β-lactamases have been described based on serological methodologies and limited sequence information. Here, we sought to reassess the classification of staphylococcal β-lactamases and their correlation with the CzIE. We included a large collection of 690 contemporary bloodstream MSSA isolates recovered from Latin America, a region

Asbestos in toys: an exemplary case.

Staphylococcus aureus is an extremely adaptable pathogen causing a wide variety of infections. Staphylococcal surface proteins that directly interact with host extracellular proteins greatly contribute to virulence and are involved in adhesion, immune escape and nutrient acquisition. In our extensive search for highly immunogenic, in vivo-expressed, staphylococcal proteins, previously, we identified a novel member of the family of Gram-positive anchor motif proteins with a predicted 895 amino acid long sequence. In order to determine the ligand for this novel LPXTG cell wall protein, we employ

[Molecular genetic identification of Staphylococcus aureus strain, caused a foodborne illness outbreak in St. Petersburg in 2013].

BACKGROUND: Staphylococcus aureus is one of the most important human pathogens and causes over 100 nosologicalforms of diseases. The lack of data on the spread of S. aureus genetic types specific for different forms of staphylococcal infections in Russia makes it difficult to timely identify and control strains of this epidemiologically dangerous bacterial pathogen. OBJECTIVE: The aim of the study was to carry out a molecular genetic research of S. aureus isolates obtained during a widespread foodborne illness outbreak among builders at the Pulkovo airport in St. Petersburg in 2013. METHODS:…

[Dynamic changes in the microbial contamination of the exudate and the mucous membrane from the tympanic cavity in the course of experimental staphylococcal otitis media].

The objective of the present study was to determine the relationships between characteristics of the microbial contamination of the pyogenic exudate and the mucous membrane from the tympanic cavity in the course of experimental suppurative otitis media caused by staphylococcal infection. The experiments were carried out on 29 rabbits showing no signs of "spontaneous" otitis. Experimental staphylococcal suppurative otitis was induced in 26 of these animals. The degree of microbial contamination of the mucous membrane from the tympanic cavity was expressed either in colony-forming units per swab

Topical ointment for preventing infection in preterm infants.

BACKGROUND: Nosocomial sepsis is a frequent and serious complication of premature infants. The increased susceptibility of ELBW infants to infection has been attributed to less effective immune function compared to mature newborns and the invasive nature of necessary supportive care. Breakdown of the barrier function of the skin may be an additional risk factor for nosocomial sepsis. OBJECTIVES: To assess the effect of prophylactic application of topical ointment on nosocomial sepsis rates and other complications of prematurity in preterm infants. SEARCH STRATEGY: Searches were made of the C

The DAIR (debridement, antibiotics and implant retention) procedure for infected total knee replacement - a literature review.

Prosthetic joint infection (PJI) is a devastating complication in total knee arthroplasty (TKA) and third most common cause of revision of TKA with significant morbidity and surgical challenges. Treatment options include non-operative measures with long term antibiotic suppression, debridement and implant retention (DAIR), one- or two-stage revision arthroplasty, arthrodesis and amputation. Implant retention without infection is ideal and DAIR has been reported to have variable success rates depending on patient factors, duration of infection, infecting micro-organisms, choice of procedure, si

Intravenous Ceftaroline in Extremely Premature Neonates With Coagulase-Negative Staphylococci Septicemia: A Report of 2 Cases.

Sepsis is one of the primary causes of newborn morbidity and mortality, particularly in preterm infants, and coagulase-negative staphylococci (CoNS) is a major cause of bacterial infections in the neonatal intensive care unit (NICU). The treatment of late-onset neonatal staphylococcal sepsis is challenging owing to increased minimum inhibitory concentrations and the potential side effects of vancomycin. Herein, we describe 2 cases of extremely preterm newborns treated with intravenous (IV) ceftaroline (6 mg/kg/dose every 8 hours) for late-onset neonatal staphylococcal sepsis. Both cases were d

Mastitis and other diseases of the goat's udder.

Udder problems of modern dairy goats are similar to those seen in dairy cows. Anomalies of the goat's udder and teats are common, and many may be hereditary. Skin diseases of the udder include viral infections, mange, sunburn, wounds, and staphylococcal dermatitis. There are numerous known causes of caprine mastitis. These include streptococci, hemolytic and nonhemolytic staphylococci, corynebacteria, and mycoplasmas. Diagnosis of mastitis in goats is often difficult, as the udder secretion may remain grossly normal and somatic cell counts in nonmastitic goats are higher than the recognized no

Staphylococcus aureus Exotoxins and Their Detection in the Dairy Industry and Mastitis.

Staphylococcus aureus constitutes a major food-borne pathogen, as well as one of the main causative agents of mastitis in dairy ruminants. This pathogen can produce a variety of extracellular toxins; these include the shock syndrome toxin 1 (TSST-1), exfoliative toxins, staphylococcal enterotoxins (SE), hemolysins, and leukocidins. S. aureus expresses many virulence proteins, involved in evading the host defenses, hence facilitating microbial colonization of the mammary glands of the animals. In addition, S. aureus exotoxins play a role in the development of both skin infections and mastitis.

Hyper-IgE syndromes.

The hyper-immunoglobulin E (IgE) syndromes (HIES) are primary immunodeficiencies characterized by the clinical triad of recurrent staphylococcal abscesses, recurrent cyst-forming pneumonia, and an elevated serum IgE level of >2000 IU/ml. Most cases are sporadic; however, multiplex families displaying autosomal dominant (AD) and autosomal recessive (AR) inheritance have been described. In most sporadic and AD cases, the HIES clinical triad is part of a multisystem disorder including abnormalities of the soft tissue, skeletal, and dental systems. In contrast, those with AR-HIES have severe mollu
