Pure alexia is a rare disorder usually caused by an ischemic or hemorrhagic stroke. We describe a case of pure alexia due to nonconvulsive status epilepticus (NCSE). This 57-year-old man presented in a hyperosmolar, nonketotic state with fluctuations in mental status and an inability to read. Neuroimaging may detect structural and functional changes following seizures. Magnetic resonance imaging (MRI) studies showed increased signal intensities in T2- and diffusion-weighted images, especially in the temporal cortex. METHODS: SE was induced in 12-day-old rats (P12 group) and 25-day-old rats (P25 group) using the lithium-pilocarpine model. We report 2 girls, aged 10 and 13 years, with juvenile myoclonic epilepsy (JME) who had episodes of nonconvulsive status epilepticus (NCSE). Symptoms included only mild lethargy, slow responses, and trembling of the eyelids. There was no ataxia or myoclonus of the limbs or body, and they were always able to respond. Electroencephalograms during these episodes showed almost continuous generalized polyspike-waves in both patients. Patients with JME can develop NCSE, which may be overlooked because of the subtle clinical symptoms.

Nonconvulsive status epilepticus (NCSE) accounts for approximately 20% of all status epilepticus (SE). A patient presenting complex partial status epilepticus (CPSE) had a clinical history and neurological picture on admission that mimicked a cerebrovascular insult. Two cases of pneumoperitoneum following status epilepticus are reported. In both patients perforation of a hollow viscus was excluded by laparotomy and status epilepticus had been accompanied by haematemesis. An increase in upper digestive tract pressure resulting from high abdominal pressure during seizures might have caused leakage of air into the peritoneum through lacerations in the lower oesophageal mucosa. Awareness of this mechanism of non-surgical peritoneum would avoid unnecessary exploratory laparotomy in these seriously ill patients.

A 12-year-old Bedouin boy presented with sudden asymmetrical status epilepticus. The presence of a scalp wound, rubbery cervical lymphadenopathy and a history of contact with cats suggested the diagnosis of cat-scratch disease complicated by encephalopathy. The diagnosis was supported by histological examination of a cervical lymph node and a positive skin test with cat-scratch antigen. The rapid establishment of the diagnosis avoided the necessity of elaborate and complicated neuroradiological investigations.
