[Torsional shift operation, a tool in complete early childhood strabismus].

By means of torsional shift operation, or "rotatory Kestenbaum procedure" - in which both ocular bulbs are rotated to the side to which the head is inclined - head malpostures can be eliminated or alleviated in three out of four cases. This applies both to the smaller group of patients with true nystagmus compensation as well as to the large group with "tilt" as a part-symptom of Lang's early-onset strabismus syndrome. The procedure for all four oblique muscles is described in detail, including the strategies for preventing side effects. The results of 98 operations are presented in tabular fo…

Familial dextrocardia, divergent strabismus and situs inversus of optic disc.

A family is described in which three of four siblings have major congenital cardiac defects in association with ocular abnormalities. The eldest sibling has isolated dextrocardia and the second has total situs inversus. The fourth child has both atrial and ventricular septal defects, with pulmonary hypertension and right ventricular hypertrophy. The mother and third sibling have normal hearts but exhibit a number of ocular defects. The principal ocular anomalies demonstrated in this family are divergent strabismus, bilateral situs inversus of the optic disc, and myopia. The hereditary aspect o…

Strabismus surgery charges at ambulatory facilities across the United States.

PURPOSE: To identify factors associated with variation in strabismus surgery charges at hospital-owned facilities across the United States. METHODS: This cross-sectional study included all strabismus-related patient encounters in the National Ambulatory Surgery Sample over a 5-year period (January 2016 to December 2020). The primary outcome was total charge per encounter. Multivariable linear regression was used to estimate the association of charge per encounter with patient, hospital, and regional characteristics, adjusting for procedure complexity and inflation. Sampling weights were used …

AI-Assisted Strabismus Diagnosis Using Eye-Tracking and Machine Learning.

Background: Strabismus diagnosis via the Alternate Cover Test (ACT) lacks quantitative standardization. This study proposes an AI-assisted framework using eye-tracking and machine learning for objective screening. Methods: Gaze coordinates were captured using a 60 Hz infrared eye tracker during ACT. Of the 291 initially screened individuals considered, 50 participants were ultimately included after quality filtering, yielding 335 valid samples. Seven algorithms were evaluated, with the dataset split into 294 training and 41 testing samples. Performance was measured by accuracy, sensitivity, sp…

Virtual reality-based Harms tangent screen test for strabismus measurement.

PURPOSE: Our study presents a virtual reality-based tangent screen test (VTS) to measure subjective ocular deviations including torsion in nine directions of gaze. The test was compared to the analogous Harms tangent screen test (HTS). METHODS: We used an Oculus Go® controller and head-mounted-display with rotation sensors to measure patient's head orientation for the VTS. The software was developed with Unity®. We assessed subjective squint angles of adult patients with eye motility disorders of different origin in nine positions of gaze by means of HTS and VTS. We calculated mean difference

Exploring WNT2 polymorphisms in comitant strabismus: A genetic association study.

BACKGROUND: Strabismus is a complex oculomotor condition characterized by a misalignment of the visual axis. The genetics of strabismus are poorly defined although a few candidate genes have been identified, among which is the WNT2 gene. Our study was designed to assess the association of single nucleotide polymorphisms (SNPs) of WNT2 in Pakistani strabismus patients. METHODS: A total of six SNPs, three intronic and three in the 3́ untranslated region, were screened in the current study. Logistic regression was performed using a dominant, recessive and additive model to determine the associat…

Wearable apparatus for correction of visual alignment under torsional strabismus.

SIGNIFICANCE: A wearable optical apparatus that compensates for eye misalignment (strabismus) to correct for double vision (diplopia) is proposed. In contrast to prism lenses, commonly used to compensate for horizontal and/or vertical misalignment, the proposed approach is able to compensate for any combination of horizontal, vertical, and torsional misalignment. PURPOSE: If the action of the extraocular muscles is compromised (e.g., by nerve damage), a patient may lose their ability to maintain visual alignment, negatively affecting their binocular fusion and stereo depth perception capabili…

Inferior Rectus Muscle Y-Split for Thyroid-Related Vertical Strabismus.

Incomitant hypotropia in thyroid eye disease can be difficult to manage, especially in the presence of orthotropia with fusion in down gaze … eral passes posteriorly reducing the risk of globe perforation. We report a patient with incomitant strabismus secondary to thyroid eye disease who underwent a combination of traditional recession and y-splitting recession of the contralateral inferior rectus muscle, resulting in good functional alignment in primary gaze and in the reading position.

Silicone band loop myopexy for treatment of myopic strabismus fixus.

A 20-year-old male presented with esotropia in both the eyes with limitation of abduction and elevation. He also had low vision in both the eyes since childhood. Radiological imaging demonstrated a nasally deviated superior rectus and inferiorly displaced lateral rectus. He was diagnosed to be a case of bilateral myopic strabismus fixus. Silicone band-assisted myopexy of superior and lateral rectus was done along with passing the band through the scleral tunnel. The patient had satisfactory alignment postoperatively and did not require any intervention over 1-year follow-up. Scleral fixation h…

Ornithine Transcarbamylase Deficiency Presenting as Acute Encephalopathy After Strabismus Surgery.

Acute encephalopathy with an unclear etiology is a common presentation to the hospital. We describe the case of a 50-year-old male who presented with a one-day history of slurred speech, nausea, insomnia, and altered mental status. His surgical history was notable for a strabismus surgery two days ago. He presented with elevated ammonia levels that continued to increase. Metabolic studies were suggestive of hyperammonemia secondary to ornithine transcarbamylase (OTC) deficiency triggered due to fasting prior to the strabismus surgery. OTC gene sequencing confirmed the diagnosis of OTC deficien…

Fixation eye movement abnormalities and stereopsis recovery following strabismus repair.

We evaluated the effects of strabismus repair on fixational eye movements (FEMs) and stereopsis recovery in patients with fusion maldevelopment nystagmus (FMN) and patients without nystagmus. Twenty-one patients with strabismus, twelve with FMN and nine without nystagmus, were tested before and after strabismus repair. Eye-movements were recorded during a gaze-holding task under monocular viewing conditions. Fast (fixational saccades and quick phases of nystagmus) and slow (inter-saccadic drifts and slow phases of nystagmus) FEMs and bivariate contour ellipse area (BCEA) were analyzed in the v…

Surgical planning of horizontal strabismus using multiple output regression tree.

Strabismus is an eye disease that affects about 0.12%-9.86% of the population, which can cause irreversible sensory damage to vision and psychological problems. The most severe cases require surgical intervention, despite other less invasive techniques being available for a more conservative approach. As for surgeries, the treatment goal is to align the eyes to recover binocular vision, which demands knowledge, training, and experience. One of the leading causes of failure is human error during the measurement of deviation. Thus, this work proposes a new method based on the Decision Tree Regre…

[Surgical treatment of vertical strabismus. Part 1. Classification and diagnostics].

Vertical strabismus caused by hyperfunction of the inferior oblique muscle is a common oculomotor disorder. It is found in one third of all patients with strabismus, and in 70% of cases it is combined with esotropia. To date, there is no single approach to determining the degree of hyperfunction of the inferior oblique muscle, and the most common classifications are very subjective and inaccurate. Thus, in connection with the need to understand the severity of the disease, as well as to determine the tactics of treatment and prognosis of the surgical outcomes, it is necessary to standartize th…

Extraocular muscle biopsy during surgery for strabismus of unknown etiology.

Most cases of strabismus have a chronic and stable or known etiologic diagnosis prior to surgery. In some cases, however, surgery is undertaken to correct a stable ocular misalignment without a definitive etiology. We present 2 cases, one with euthyroid Graves' orbitopathy and the other with amyloid light-chain amyloidosis, in which extraocular muscle biopsy performed intraoperatively allowed histopathologic confirmation of a clinical diagnosis.

Anatomic Factors Predicting Postoperative Strabismus in Orbital Wall Fracture Repair.

This study is aimed to determine the relationship between orbital fracture sites in each CT scan view and postoperative diplopia. Data for 141 patients of orbital wall fracture were analyzed retrospectively. One group of examiners reviewed sagittal, coronal and axial CT scans. Descriptive statistical analysis was used to assess each fracture area and its potential relationship with the occurrence of postoperative diplopia. Among the three anatomical views, sagittal sections were significantly associated with post-operative diplopia (PD) (p = 0.044). For orbital wall fractures in a single locat

Processed amniotic membrane for conjunctival reconstruction in complex strabismus surgery.

INTRODUCTION: In ophthalmology amniotic membrane is usually used for the treatment of corneal ulcerations and chemical burns of the ocular surface. Also, it is useful in the treatment of conjunctival defects. There are few publications on the long-term outcomes with the use of amniotic membrane to replace the conjunctiva. In our cases, we used it in patients with restrictive strabismus or conjunctival problems during or following complex eye muscle surgery. MATERIALS AND METHODS: In this retrospective case study, we reviewed the charts of the patients who had been treated with Ambio5® (IOP In…
