Relationship between design and control of artificial heart for protection of the right/left balance.

Right/left matching in the total artificial heart (TAH) is essential to prevent fatal volume displacement into the pulmonary circuit. Measurements were made with three different sized Rostock pneumatic artificial ventricles incorporated in the Donovan mock circulatory system together with the heart driver AKT 86. First for each ventricle we determined the dependence of the maximum effective stroke volume on the systolic driving pressure and the afterload. The right ventricle (RV) is about 10% more effective than the left ventricle (LV). Control of the TAH permits different or equal frequencies…

Normal-appearing white matter microstructural injury is associated with white matter hyperintensity burden in acute ischemic stroke.

BACKGROUND: White matter hyperintensity of presumed vascular origin is a risk factor for poor stroke outcomes. In patients with acute ischemic stroke, however, the in vivo mechanisms of white matter microstructural injury are less clear. AIMS: To characterize the directional diffusivity components in normal-appearing white matter and white matter hyperintensity in acute ischemic stroke patients. METHODS: A retrospective analysis was performed on a cohort of patients with acute ischemic stroke and brain magnetic resonance imaging with diffusion tensor imaging sequences acquired within 48 h of

Endomyocardial fibrosis with ischaemic stroke.

A Caucasian man in his mid-50s presented to the emergency department with sudden onset right-sided weakness and dysarthria. CT angiogram demonstrated a small volume of left middle cerebral artery territory infarction after which the patient underwent successful thrombolysis. Initial ECG showed diffuse T wave inversion. He was admitted to an acute stroke ward where 72 hours of telemetry demonstrated normal sinus rhythm without arrhythmia. Transthoracic echocardiogram showed a reduced ejection fraction of 47% in the absence of any regional wall motion abnormality. An echogenic mass was noted in

Assessment of Myocardial Function by Speckle Tracking Echocardiography in Patients with Acute Ischemic Stroke.

BACKGROUND: Acute stroke is an important cause of morbidity and mortality. Myocardial injury is an important complication of acute cerebrovascular events. Neurogenic stress cardiomyopathy (NSC) is a condition of acute myocardial systolic dysfunction that can be observed after acute cerebrovascular events. OBJECTIVE: In this study, we aimed to investigate the relationship between myocardial function assessed by two-dimensional speckle-tracking echocardiography and National Institutes of Health Stroke Scale (NIHSS) score in patients with acute ischemic stroke. MATERIALS AND METHODS: This cross

A Review of the Role of Transthoracic and Transesophageal Echocardiography, Computed Tomography, and Magnetic Resonance Imaging in Cardioembolic Stroke.

Stroke is a major source of morbidity and mortality worldwide, accounting for the second largest cause of mortality and the third greatest cause of disability. Stroke is frequently preceded by a transient ischemic attack (TIA). The etiologies of 20-30% of ischemic strokes are unknown, and thus are termed "cryptogenic strokes". About 25% of ischemic strokes are cardioembolic. Strokes occur at a rate of around 2% per year in individuals with heart failure with reduced ejection fraction (HFrEF), with a strong correlation between stroke risk and the degree of ventricular impairment. Furthermore, s

The usefulness of bladder reconditioning before indwelling urethral catheter removal from stroke patients.

OBJECTIVE: The aim of this study was to determine the effects of bladder reconditioning by indwelling urethral catheter (IUC) clamping before IUC removal in stroke patients. DESIGN: Sixty patients with stroke were randomized to 0-, 1-, and 3-day IUC clamping groups. IUCs were removed without clamping in the 0-day group. In the other two groups, IUCs were clamped for 4 hrs followed by 5 mins of urinary drainage, a cycle repeated over 24 hrs in the 1-day and over 72 hrs in the 3-day clamping groups. Time to first voiding (FV), first voided volume (FV-vol), residual urine volume after FV, mean v

Stroke-induced immunosuppression is associated with brain injury and poor outcomes in patients receiving intravenous thrombolysis: A multicenter prospective cohort study.

Following a stroke, a local inflammatory cascade is triggered, resulting in the systemic diminishment of immune function, known as stroke-induced immunosuppression. The association of stroke-induced immunosuppression with brain injury and stroke prognosis after intravenous thrombolysis remains unclear. In this cohort study, we prospectively enrolled 1023 patients across 16 centers. Lymphocyte counts and the neutrophil-to-lymphocyte ratio were used as indicators of the severity of stroke-induced immunosuppression. The primary outcomes were the 90-day functional outcome, assessed using the modif

Association between arterial calcifications and nonlacunar and lacunar ischemic strokes.

BACKGROUND AND PURPOSE: Nonlacunar cerebral infarcts are presumed to be caused by thromboembolism from the heart or extracranial arteries, whereas lacunar infarcts are thought to be caused by small vessel disease. We investigated to what extent arterial calcifications differ between nonlacunar and lacunar ischemic strokes. METHODS: We studied 820 consecutive patients with transient ischemic attack or ischemic stroke in the anterior circulation who underwent multidetector computed tomography angiography and had no rare cause of stroke. The presence of likely cardioembolic pathogenesis was dete

The relative cerebral blood volume (rCBV) < 42% is independently associated with hemorrhagic transformation in anterior circulation large vessel occlusion.

BackgroundPretreatment CT perfusion (CTP) marker relative cerebral blood volume (rCBV) < 42% lesion volume has recently shown to predict poor collateral status and poor 90-day functional outcome. However, there is a paucity of studies assessing its association with hemorrhagic transformation (HT). Here, we aim to assess the relationship between rCBV < 42% lesion volume with HT.MethodsIn this retrospective study, we included patients with acute ischemic stroke secondary to large vessel occlusion (AIS-LVO) of anterior circulation who had successful recanalization from two comprehensive stroke ce

Neuropsychological deficits in patients with favorable outcomes after middle cerebral artery infarction treated with thrombectomy.

INTRODUCTION: Functional outcomes following stroke thrombectomy are commonly measured by the modified Rankin Scale (mRS). Due to hemisphere-specific representation of neuropsychological function, the mRS might underestimate persisting functional deficits, particularly in large right-hemispheric strokes. This study aimed to assess infarct volumes and neuropsychological symptoms in patients with left- and right-sided middle cerebral artery (MCA) infarctions treated with thrombectomy, having a favorable outcome at 3 months. We hypothesized that patients with right-sided strokes are affected by la

Mean platelet volume: a prognostic marker in heart failure with preserved ejection fraction.

Heart failure (HF) with preserved ejection fraction (HFpEF) is associated with high burden of comorbidities known to increase the mean platelet volume (MPV). This parameter has been associated with morbidity and mortality in HF. However, the role of platelets and the prognostic relevance of MPV in HFpEF remain largely unexplored. We aimed to evaluate the clinical usefulness of MPV as a prognostic marker in HFpEF. We prospectively enrolled 228 patients with HFpEF (79 ± 9 years; 66% females) and 38 controls of similar age and gender (78 ± 5 years; 63% females). All subjects underwent two-dimensi

[Postinfusion complications by acute blood loss].

Highs and lows of infusion therapy for the treatment of the acute blood loss were studied experimentally and in clinic. Colloid and crystalloid solutions, being hemodilutants, do not transport oxygen, causing, therefore, several complications in recipient's organism. Dilutional anemic and lead to heart insufficiency, which, by-turn, cause the so called dilutional circulatory hypoxia.

Preloading history influences pressure-volume-derived indices of myocardial contractility in the ejecting canine left ventricle.

The dependency of indices of myocardial contractility on the immediate preloading protocol was assessed in 14 pentobarbital anesthetized open-chest dogs. An intracavitary micromanometer and epicardially placed ultrasonic dimension transducers allowed acquisition of dynamic left ventricular pressure-volume relationships while varying preload on right heart bypass. A resting cardiac output of 75 ml/kg/min was either increased twofold or reduced to zero flow over a 20-second interval. Linear regression analysis of the mechanical parameters permitted construction of the end-systolic pressure-volum

Artificial ventilation with PEEP; the influence on cardiac output in the simulated normo-, hypo- and hypervolemic state.

Observations of the influence of artificial ventilation with PEEP on cardiac output are unequivocal. Not only the measure of the end-expiratory pressure and lung compliance play a role, but also the circulating blood volume. In healthy laboratory animals a state of normo-, hypo- and hypervolemia was simulated by changes in position in neurolept-anesthesia. In all cases artificial ventilation with PEEP induced a decrease of cardiac output as a consequence of a decrease of stroke volume. These results are discussed. The conclusion is drawn that a change in stroke volume during artificial ventila…

Early Post-Stroke Electroacupuncture Promotes Motor Function Recovery in Post-Ischemic Rats by Increasing the Blood and Brain Irisin.

OBJECTIVE: Recent studies have shown that irisin, a novel peptide hormone derived from muscles, could be used as a potential therapeutic drug against ischemic stroke. Moreover, electroacupuncture (EA) is widely used in the treatment of ischemic stroke. Yet, whether irisin is involved in the EA neuroprotection remains unclear. The following study investigated the association between serum and peri-lesional cortex irisin and EA-induced post-stroke motor recovery in rats. METHODS: The middle cerebral artery occlusion (MCAO) method was used to induce ischemic stroke in rats. Rats were randomly di

Carotid plaque CTA analysis in patients with initial and recurrent ischemic stroke.

OBJECTIVES: Recurrent ischemic stroke leads to prolonged hospital stays, deterioration in functional outcomes, increased disability and mortality rates. The composition of plaques is closely related to the occurrence of stroke. Our aim was to explore differences in carotid plaque characteristics between patients with initial and recurrent ischemic stroke. METHODS: A retrospective study was conducted on 208 anterior circulation stroke patients who had completed head and neck CT angiography (CTA) examinations. Patients were divided into initial cerebral infarction (ICI) group and recurrent cere
