Case Report: Central Nervous System Strongyloidiasis: Two Cases Diagnosed Antemortem.

Central nervous system (CNS) strongyloidiasis is a known but rare form of disseminated infection. The diagnosis is often made postmortem, with only five published cases of an antemortem diagnosis. We report two fatal cases of CNS strongyloidiasis diagnosed antemortem, with Strongyloides stercoralis larvae visualized in the CNS sample in one case. Risk factors for disseminated strongyloidiasis common to both cases included origination from the Caribbean, underlying human T-lymphotropic virus-1 infection, and recent prednisone use. Both cases occurred in Canada, where the occurrence of Strongylo…

Chronic strongyloidiasis with recurrent asthma exacerbations and steroid-associated 'hives'.

A 74-year-old man experienced worsening asthma for several years. Oral steroids were required on multiple occasions for asthma treatment. During his steroid courses, he developed a hive-like rash, which would resolve after completion of each steroid course. He was from Romania, and had lived in the USA for many years. Laboratory testing had shown eosinophilia. He was eventually diagnosed with strongyloidiasis by serology. Treatment with ivermectin led to marked improvement but not resolution of his long-term asthma. His hive-like rash, which was likely larva currens, did not recur with a subse…

Human strongyloidiasis: identifying knowledge gaps, with emphasis on environmental control.

Strongyloides is a human parasitic nematode that is poorly understood outside a clinical context. This article identifies gaps within the literature, with particular emphasis on gaps that are hindering environmental control of Strongyloides. The prevalence and distribution of Strongyloides is unclear. An estimate of 100-370 million people infected worldwide has been proposed; however, inaccuracy of diagnosis, unreliability of prevalence mapping, and the fact that strongyloidiasis remains a neglected disease suggest that the higher figure of more than 300 million cases is likely to be a more ac…

Strongyloidiasis presenting as larva currens 38 years after presumed exposure.

BACKGROUND: We present a case of strongyloidiasis presenting as larva currens 38 years after potential exposure in a country with what is typically thought of as low epidemiologic risk. CONCLUSION: This case highlights several important features of strongyloidiasis, including its varied epidemiology, long incubation period, and cutaneous manifestations.

Severe and treatment resistant strongyloidiasis--indicator of HTLV-I infection.

The association between severe and persistent strongyloidiasis with human T cell lymphotropic virus type I (HTLV-1) infection is well documented in reports from HTLV-1 endemic regions like Japan and Jamaica. But there are no reports from non-endemic areas like India. We report a case of severe intestinal strongyloidiasis in a 45-year old Keralite man, living in Sikkim. Despite standard treatment with many courses of albendazole, his stool persistently showed Strongyloides stercoralis larvae. In the absence of other immunosuppressive conditions, human T cell lymphotropic virus type I infection …

Strongyloidiasis with gastric mucosal invasion presenting with acute interstitial nephritis.

We report an atypical occurrence of invasive Strongyloides stercoralis infection of the stomach mucosa in an elderly female patient from Bangka Island, northwestern Indonesia. The patient presented with severe epigastric pain, edema of the legs, proteinuria and severe hypoalbuminemia. Gastric and duodenal biopsies found eggs, larval and adult forms present in the superficial mucosa with mild inflammation. The Harada-Mori filter paper culture technique revealed S. stercoralis filariform larvae and free-living adult worms, corroborating the diagnosis. The infection was associated with acute inte

Successful ivermectin treatment of hepatic strongyloidiasis presenting with severe eosinophilia.

A 49-year-old, previously healthy nurse presented with hepatic lesions and severe peripheral eosinophilia due to strongyloidiasis. Imaging studies of the abdomen showed predominantly peripheral, confluent hepatic lesions. The hepatic lesions and eosinophilia did not show any improvement with albendazole, but completely resolved with ivermectin treatment. Our findings suggest that Strongyloides stercoralis can present with isolated focal hepatic lesions and severe eosinophilia, and resolves with ivermectin treatment.

Strongyloidiasis in Sfax (Tunisia): a survey over 13 years period.

A retrospective study of strongyloidiasis cases was carried out in Sfax University (Tunisia), between 1989 and June 2001. Diagnosis was done by direct stool examination, concentration methods, Baermann larval extraction and fecal culture. Strongyloidiasis was diagnosed in 27 patients of whom 77% were males living in rural areas under low socioeconomic level. Mean age of patients was 47 years (8 to 85 years). Hypereosinophilia was present in 75 % of cases, and 7 patients were treated by systemic corticosteroids. So, it is important to detect this parasite by repeated parasitological stool exami…

Nephrotic syndrome in strongyloidiasis: remission after eradication with anthelmintic agents.

Strongyloides stercoralis infection is known to be important because of its potential for life-threatening disseminated infection in immunosuppressed hosts. Apart from direct invasion into nearly every organ in systemic infection, evidence suggests that immunological reaction also plays a role in the pathogenesis of the disease, including both uncomplicated and disseminated infections. However, Strongyloides-related glomerulonephritis has not been well documented. We present a case of steroid- and cyclophosphamide-resistant nephrotic syndrome complicated by disseminated strongyloidiasis which …

The prevalence of antibodies against Strongyloides in the serum of newly-arrived Indochinese immigrants was determined. Sera that had been s … may persist for years. Furthermore, immunosuppression may result in fatal, disseminated infection. Strongyloidiasis is now recognized in three major Australian population groups: Aborigines who are living in the north of the country; World War II veterans who were prisoners in southeast Asia; and Indochinese immigrants. The salient features of the clinical manife …

Strongyloidiasis in immunosuppressed hosts. Presentation as massive lower gastrointestinal bleeding.

Two cases of massive lower gastrointestinal hemorrhage in immunosuppressed patients were due to complicated infestation with Strongyloides stercoralis. The very high mortality of disseminated strongyloidiasis may in part be attributed to delays in diagnosis and treatment resulting from the complex life cycle of this nematode. Successful therapy in the cases presented consisted of reduction of corticosteroid dosage, use of thiabendazole in excess of that recommended for uncomplicated infestation, parenterally administered nutrition, multiple transfusion of blood products, and vigorous supportiv…

Recurrent mycotic aortic aneurysm with Klebsiella bacteremia and superimposed strongyloidiasis.

Mycotic aortic aneurysms are rare, representing <5% of all aortic aneurysms with limited epidemiological data. To date, there are only three case reports on mycotic aneurysm with Klebsiella pneumoniae infection including one case of recurrent infection. There are no case reports on mycotic aortic aneurysm in the setting of a Strongyloides stercoralis coinfection, a helminth associated with intravascular dissemination of enteric bacteria. Although an open repair strategy is considered a gold standard for the treatment of these aneurysms, there is some consideration for endovascular strategies i

Fulminant disseminated strongyloidiasis mimicking inflammatory bowel disease: a case report.

BACKGROUND: Strongyloidiasis is a gastrointestinal parasitic infection caused by Strongyloides stercoralis (S. stercoralis). Its autoinfection capability allows the parasite to persist lifelong in the absence of treatment, exposing patients to the risk of hyperinfection and fatal dissemination in cases of immunosuppression. RESULTS: A 33-year-old man, presented in December 2024 with severe acute colitis. Endoscopy and histopathology findings were consistent with inflammatory bowel disease (IBD). The patient was treated with intravenous corticosteroids and antibiotics, then maintained on taper…

Concurrent strongyloidiasis and allergic bronchopulmonary aspergillosis complicating diagnosis: case report.

Herein, we report a case of allergic bronchopulmonary aspergillosis (ABPA) complicated by Strongyloides stercoralis infection. A 73-year-old man with a history of duodenal ulcer perforation and prior Billroth II gastrectomy presented with a 14-year history of recurrent cough and wheezing, recently accompanied by new gastrointestinal symptoms. He was initially diagnosed with chronic obstructive pulmonary disease (COPD). Subsequent laboratory evaluation revealed a markedly elevated total serum IgE (1,219 kUA/L) and increased Aspergillus fumigatus-specific IgE (0.38 kUA/L, positive cutoff >0.35 k

Case Report: Two Cases of Strongyloidiasis Presenting with Thrombotic Events.

We report two unusual cases of clinical strongyloidiasis that present as extensive thrombosis: a case of hyperinfection with concurrent eosinophilia and a case of disseminated infection as a complication of immunosuppression. We discuss risk factors for the development of Strongyloides stercoralis infection and thromboembolism, and the recommended management.
