Webcam delivery of the Camperdown Program for adolescents who stutter: a phase I trial.

PURPOSE: This Phase I clinical trial explored the viability of webcam Internet delivery of the Camperdown Program for adolescents who stutter. Method and Procedure Participants were 3 adolescents ages 13, 15, and 16 years, with moderate-severe stuttering. Each was treated with the Camperdown Program delivered by webcam with no clinic attendance. Primary outcome measures were percentage of syllables stuttered and number of treatment sessions to maintenance. Secondary outcome measures were speech naturalness, situation avoidance, self-reported stuttering severity, and parent and adolescent satis…

Developing the therapeutic relationship: from 'expert' professional to 'expert' person who stutters.

UNLABELLED: This article looks back over the years and identifies some of the most influential thinkers, writers, and researchers who have h … ho stammered. After several unsuccessful attempts to find a 'cure' he found his own solution to his stuttering and then used what he had learned to help many others. Much of what he wrote was well ahead of his time and reminds us, the professionals, of the importance of listening to and taking account of the views of PWS in therapy and designing treatment th …

Group therapy for school-aged children who stutter: a survey of current practices.

UNLABELLED: Although group therapy is recommended for school-aged children who stutter (CWS), it is not widely researched. This study aimed to explore this provision, using a postal survey which investigated the current practices of Speech & Language Therapists (SLTs) in the UK. Seventy percent of SLT services provided some group therapy, but the level of provision was variable. There was a lack of consensus on what the main aims of group therapy should be. Important barriers to group therapy provision were identified, including a perceived lack of clients' interest in group therapy, and insuf

Intervention with the Lidcombe Program for a bilingual school-age child who stutters in Iran.

OBJECTIVE: In this study, the immediate and extended effects of the Lidcombe Program were investigated for the first time in Iran. Treatment in the Lidcombe Program is carried out by the child's parent (or carer) in the child's everyday environment. The program has been shown to be effective with preschool children who stutter (i.e. younger than 6 years) and to a lesser extent with older children. PARTICIPANT AND METHOD: The participant was a bilingual (Baluchi-Persian) boy aged 8 years 11 months. Treatment was conducted in both languages. Stuttering severity was measured in Baluchi with the p…

Acoustic duration changes associated with two types of treatment for children who stutter.

The purpose of this study was (a) to examine in young children the effects of Speech Motor Training (SMT) on selected temporal acoustic durations considered to be related to speech motor programming, (b) to compare the speech motor effects of that treatment with those of a treatment of childhood stuttering that did not directly incorporate speech motor control training (Extended Length of Utterance [ELU]), and (c) to examine the relation of acoustic duration changes to reduction of stuttering. Twelve children who stutter were recorded while repeating syllable sets /see text/ and /see text/ bef…

Adult onset of stuttering as a presenting sign in a parkinsonian-like syndrome: a case report.

Stuttering onset in adulthood is rare. With no prior history of stuttering or demonstrable neurological insult, diagnosis is often that of a conversion reaction. Stuttering as the first sign of a parkinsonian-like syndrome in extrapyramidal disease has only been reported once in the previous 30 years (Koller, 1983). The present case study differs from and builds upon the case reports in the literature, and describes a 29-year-old white male who began stuttering purportedly secondary to psychological stress. The fluency evaluation revealed severe stuttering characterized by multiple repetitions…

Voice-onset time and electroglottographic dynamics in stutterers' speech: implications for a differential diagnosis.

Three stuttering subjects were used to test findings that voice-onset time (VOT) in the fluent utterances of stuttered speech are slower than those of their normal-speaking peers. Acoustic analysis revealed the stutterers' VOTs to be slower, but when analysed by a t test the difference was found not to be significant at the P = 0.05 level. Electroglottographic (EGG) dynamics were also studied to provide comparative data for recent research which notes specific EGG activity amongst stutterers following severe blocks. Analysis revealed that both experimental and control subjects exhibited this p…

How Simple Can a Treatment for Early Stuttering Be? A Proposed Two-Factor Early Intervention.

PURPOSE: In this clinical focus article, we draw attention to the need for immediate intervention shortly after stuttering onset. More than half of stuttering onsets occur before 3 years of age. We argue that existing interventions for preschool children who stutter require varying levels of cognitive engagement from children; hence, they are not suitable for children of that age. For this reason, we argue that there is no clinical trials evidence for treatment efficacy with children younger than 3 years of age. CONCLUSIONS: There are many recommended parent strategies that do not require any…

Multidimensional Individualized Stuttering Therapy Outcomes At 24 Months Post Clinic: An Embodiment and Awareness Perspective.

PURPOSE: Multidimensional Individualized Stuttering Therapy (MIST) is based on proactive collaborative work and shared decision making between client and clinician with the goal of identifying individual case-appropriate procedures and therapy elements in a manner that is meaningful and context-sensitive for each person. MIST combines value- and awareness-based elements from acceptance and commitment therapy (ACT) with stuttering and speech modification interventions. In keeping with the principles of ACT, we locate mindfulness as part of an embodied practice facilitated through a collaboratio…

A three-arm randomized controlled trial of Lidcombe Program and Westmead Program early stuttering interventions.

PURPOSE: To compare two experimental Westmead Program treatments with a control Lidcombe Program treatment for early stuttering. METHOD: The design was a three-arm randomized controlled trial with blinded outcome assessments 9 months post-randomization. Participants were 91 pre-school children. RESULTS: There was no evidence of difference in percentage syllables stuttered at 9 months among groups. Dropout rates were substantive and may have been connected with novel aspects of the trial design: the use of community clinicians, no exclusion criteria, and randomization of children younger than…

What do people search for in stuttering therapy: Personal goal-setting as a gold standard?

PURPOSE: Stuttering affects people in individual ways, and there are multiple factors which may influence a person's goals when seeking therapy. Even though there is a common consensus that speech-language pathologists should discuss the individual's goals and expectations for stuttering therapy and outcomes, few studies have systematically investigated this issue. The aims of the present study were to investigate individual motivations and goal-setting related factors in stuttering therapy. The associations between self-reported impact of stuttering and the participants' perceptions of stutte…

Frequency of speech disruptions in Parkinson's Disease and developmental stuttering: A comparison among speech tasks.

OBJECTIVE: To analyze the frequency of speech disruptions across different speech tasks, comparing the performance of individuals with Parkinson's Disease (PD) and DS. METHOD: Participants were 20 people with PD, 20 people with DS and 40 fluent individuals. Speech samples were recorded during monologue speech, choral and solo oral reading. Transcriptions of 200 fluent syllables were performed to identify stuttering-like disfluencies (SLDs) and other disfluencies (ODs). RESULTS: People with PD presented significantly less speech disruptions when compared to people with DS, but significantly m…

Association study of stuttering candidate genes GNPTAB, GNPTG and NAGPA with dyslexia in Chinese population.

BACKGROUND: Dyslexia is a polygenic speech and language disorder characterized by an unexpected difficulty in reading in children and adults despite normal intelligence and schooling. Increasing evidence reveals that different speech and language disorders could share common genetic factors. As previous study reported association of GNPTAB, GNPTG and NAGPA with stuttering, we investigated these genes with dyslexia through association analysis. RESULTS: The study was carried out in an unrelated Chinese cohort with 502 dyslexic individuals and 522 healthy controls. In all, 21 Tag SNPs covering …

The effects of self-generated synchronous and asynchronous visual speech feedback on overt stuttering frequency.

PURPOSE: Relatively recent research documents that visual choral speech, which represents an externally generated form of synchronous visual speech feedback, significantly enhanced fluency in those who stutter. As a consequence, it was hypothesized that self-generated synchronous and asynchronous visual speech feedback would likewise enhance fluency. Therefore, the purpose of this study was to investigate the effects of self-generated visual feedback (i.e., synchronous speech feedback with a mirror and asynchronous speech feedback via delayed visual feedback) on overt stuttering frequency in t…

A new antistuttering device: treatment of stuttering using bone conduction stimulation with delayed temporal feedback.

OBJECTIVES: Stuttering is a communication disorder affecting approximately 1% of the adult population, some with severe manifestations. Speech therapy improves stuttering, but many do not receive enough benefit to communicate fluently. Antistuttering devices have been available for several years, but available technology has been limited in long-term success and reliability. The current study evaluates the effects of a prototype device using a modification of a currently used bone conduction hearing device with delayed auditory feedback on adult patients with significant stuttering problems. …
