Malignant hyperthermia (MH) is a pharmacogenetic disorder of skeletal muscle presenting as a hypermetabolic response to potent volatile anesthetics such as sevoflurane, desflurane, isoflurane and the depolarizing muscle relaxant succinylcholine. Following introduction of the hydantoin derivative dantrolene, the previously high mortality of fulminant MH episodes could be reduced from > 80% to less than 10%. Pretreatment with 3 different doses of atracurium (0.035 mg/kg, 0.05 mg/kg, 0.07 mg/kg) was investigated in order to determine the optimal dose that would prevent suxamethonium-induced fasciculations with minimal side effects to the patient and without significant antagonism of suxamethonium block. Spasm of muscle in association with suxamethonium is not uncommon. As an accepted early sign of malignant hyperpyrexia (MH), patients have been referred for MH screening who have shown only this abnormality. Case histories of 277 probands have been analysed and grouped according to final diagnosis, depending on results of muscle biopsy and in vitro screening. When muscle spasm is induced with suxamethonium, malignant hyperpyrexia must be considered a probable diagnosis.

During malignant hyperthermia (MH) induced by halothane and succinylcholine, oxygen consumption (VO2) of whole body, muscle and the splanchnic area was calculated from measurements of blood flow and arteriovenous oxygen content difference.
