The wide-awake approach to flexor tendon repair has decreased our rupture and tenolysis rates and permitted us to get consistently good results in cooperative patients. We report a case of simultaneous reconstruction of a quadriceps tendon rupture after total knee arthroplasty (TKA) and neglected Achilles tendon rupture, which occurred before TKA with an ipsilateral hamstring autograft. This case report describes the use of a peroneus brevis allograft to reconstruct a neglected Achilles' tendon injury in a 75-year-old woman. She had difficulty walking, had stiffness, and was unable to perform a heel raise from a single-leg standing position. Magnetic resonance imaging confirmed a 6.8-cm wide defect 13 months after the initial injury. We report a method of Achilles tendon reconstruction using a free quadriceps bone-tendon graft. The patient had a prior repair of a re-ruptured Achilles tendon, following which he developed massive necrosis of his skin and Achilles tendon leaving a 10 cm defect. First stage reconstruction consisted of soft tissue coverage of the skin defect with a sural fasciocutaneous flap. Reconstruction of the Achilles tendon followed, with the patellar bone block fixed to the calcaneus and the quadriceps tendon sutured proximally.

INTRODUCTION: The following article deals with the problem of peroneal clicking. This means a clicking of the two peroneal tendons against each other. It has to be differentiated from peroneal tendon dislocation or subluxation.
METHOD: In three patients, a peroneal click was found without symptoms of dislocation of the tendons over the fibula. In these cases the indication for resection of the peroneal brevis tendon was seen.
RESULTS: Postoperatively no pain, swelling, and peroneal click were present. The partial loss of function of the peroneal brevis tendon seems to be negligible.

Rheumatoid arthritis is not as common a problem in Taiwan as it is in western countries. Compared with traumatic tendon rupture, tendon rupture in rheumatoid arthritis has its complex and peculiar pathophysiological mechanism which includes chemical erosion by hypertrophic synovium and mechanical attrition by deformed joints and bony spurs. In management, all these pathological foci must be eradicated to prevent recurrent failure. Tendon transfer is a simple and effective method for reconstructing lost tendon function.

CASE: We present the case of a 27-year-old man who sustained a quadriceps tendon rupture within 3 months after surgical repair of an ipsilateral patellar tendon rupture repaired with the transosseous technique. Popliteus tendon sheath ganglia represent a rare and frequently overlooked cause of persistent lateral knee pain. This case report describes a patient with chronic symptoms unresponsive to conservative treatment, in whom advanced imaging revealed a ganglion at the lateral insertion of the popliteus tendon. Ultrasound-guided corticosteroid injection resulted in complete resolution of symptoms. The case highlights the importance of thorough differential diagnosis using advanced imaging and supports the effectiveness of targeted, minimally invasive interventions. Achieving tendon healing is one of the objectives in surgical repair of the rotator cuff, and many factors could influence this healing.
