Serial magnetic resonance imaging and ultrasound examinations demonstrate differential inflammatory lesion patterns in soft tissue and bone upon patient-reported flares in rheumatoid arthritis.

BACKGROUND: Magnetic resonance imaging (MRI) and ultrasonography (US) are more sensitive than clinical evaluation in assessing inflammation … ing clinical examination, patient-reported outcomes, MRI, and US of wrists and hands. MRI synovitis/tenosynovitis/bone marrow edema (BME) and US synovitis/tenosynovitis were scored. MRI and US scores at and after the flare were compared to baseline before the flare, and associations were explored by linear mixed models for repeated measurements. RESULTS: Synov …

Role of toe flexor tendoscopy in management of an unusual cause of metatarsalgia.

The purpose of this study is to describe an endoscopic approach to toe flexor tendons at the level of metatarsal head region. And this study is a kind of retrospective case series. Three patients with toe flexor tenosynovitis were evaluated after a follow-up of 2 years after toe flexor tendoscopy. One patient suffered from infective toe flexor tendosynovitis resulting from a penetrating injury and other two patients suffered from idiopathic focal toe flexor tenosynovitis. Pre-operative metatarsalgia subsided after toe flexor tendoscopy. No endoscopy-related complication was observed. Toe flexo…

Tuberculous compound palmar ganglion: unravelling a rare diagnosis.

BACKGROUND: Tuberculous tenosynovitis is a rare manifestation of musculoskeletal tuberculosis, often misdiagnosed due to its indolent course and nonspecific presentation. Compound palmar ganglion, a chronic form of tuberculous tenosynovitis, can mimic benign conditions like ganglion cysts, leading to diagnostic delays, especially in immunocompromised individuals. CASE REPORT: We report a case of a 35-year-old HIV-positive female who presented with a painless swelling on the volar aspect of the wrist for six months. MRI revealed flexor tendon thickening and synovial proliferation, raising susp…

The value of MRI examination on bilateral hands including proximal interphalangeal joints for disease assessment in patients with early rheumatoid arthritis: a cross-sectional cohort study.

BACKGROUND: Bilateral hands including proximal interphalangeal joints (PIPJs) are recommended on physical, X-ray radiographic, or ultrasonog … % showing severe synovitis unilaterally in dominant MCPJs/PIPJs. Among these early RA patients, MRI tenosynovitis occurred the most frequently in wrist extensor compartment I, while MRI examination on bilateral hands demonstrated no overuse influence present. However, overuse should be considered in dominant PIPJ2, PIPJ4, and IPJ of thumb of which MRI tenosynov …

Hand and Finger Injuries: Chronic Hand and Wrist Pathologies.

Common chronic hand pathologies seen by family physicians include carpal tunnel syndrome, trigger finger, and de Quervain tenosynovitis. Carpal tunnel syndrome is the most common compressive neuropathy of the upper extremity and is caused by compression of the median nerve at the wrist. Trigger finger, also known as stenosing tenosynovitis, is caused by a metaplasia of the A1 flexor tendon pulley and the flexor tendon causing increased friction and decreased space for tendon gliding in the flexor tendon sheath. de Quervain tenosynovitis has a similar metaplasia of the tissues of the first dors…

The four-in-one arthroplasty for the painful arc syndrome.

The painful arc syndrome of the shoulder is a manifestation of rotator cuff tendinitis associated with tenosynovitis of the long head of the biceps under and just distal to the transverse humeral ligament. Eighty-nine patients with clinical signs of the painful arc syndrome were proven to have an associated biceps tenosynovitis by arthrography and at surgical treatment. The four-in-one arthroplasty consists of: (1) excision of the coracoacromial ligament; (2) acromioclavicular arthroplasty; (3) excision of the anterior inferior area of the acromion process; and (4) transfer and tenodesis of th…

Candida Parapsilosis associated rice bodies in the extensor compartment of the wrist--an emerging finding.

Candida parapsilosis has been considered an emerging pathogen with increasing incidence reported in the literature. As a normal commensal of human skin, it is likely that Candida species could gain access to soft tissues of the hand and wrist by direct inoculation, resulting in an infectious tenosynovitis. With the increased prevalence of intravenous drug use (IVDU), users are at increasing risk for musculoskeletal infections including soft tissue abscesses, cellulitis, tenosynovitis, and septic arthritis. Chronic tenosynovitis, with rice body formation in particular, is a comparatively rare m…

Disseminated gonococcal infection: an unusual presentation.

Gonococcus typically affects the mucosal surfaces but in rare cases can spread to bloodstream causing disseminated gonococcal infection (DGI). The usual presentation of DGI is rash, polyarthralgia, and tenosynovitis. We present the case of a 23-year-old female who presented to our hospital with pustular rash and tenosynovitis of hand and was sent home on Augmentin. Her symptoms worsened despite treatment and she presented back to the ED. On investigation, she was found to have DGI and was appropriately treated. DGI should be kept in mind in sexually active youngsters who have only one or two f…

Recurrent extensor tendon rupture in adult-onset dermatomyositis: a case report.

We report on a case in which recurrent extensor tendon rupture was caused by tenosynovitis without skin ulcer and calcinosis in adult-onset dermatomyositis involving the hand. Proliferative tenosynovitis of the extensor tendons led to recurrent elongation or rupture of the extensor tendons, and tendon graft produced the good postoperative result in this case.

Foreign body granuloma. A cause of trigger thumb.

Snapping or triggering of fingers is a common complaint, the most frequent cause being tenosynovitis of the flexor tendon sheath. Injury of the flexor tendon as a precursor of triggering is infrequent. Foreign body granuloma of the tendon sheath as an antecedent factor has not been yet recorded. We report a case of stenosing tenosynovitis of the flexor tendon of the thumb caused by local reaction to a foreign body.

Tendinopathy Pain Mimicking Neuropathy on Nerve Conduction Study Resolved with Ultrasound-Guided Steroid Injection Treatment: A Case Report.

We describe the case of a 41-year-old Asian man who was initially given a diagnosis of peroneal neuropathy but who later received a diagnosis of tenosynovitis of extensor digitorum longus (EDL). The patient initially presented with left lateral ankle numbness, pain, and decreased range of dorsiflexion after an 8-km walk. Peroneal neuropathy was first diagnosed on the basis of reduced compound muscle action potential (CMAP). Conversely, ankle ultrasound revealed normal peroneal nerve but considerable EDL tenosynovitis. Ultrasound-guided injection of triamcinolone and lidocaine into the tendon s…

Spontaneous Rupture of the Extensor Pollicis Longus Tendon: A Systematic Review.

BACKGROUND: Extensor pollicis longus (EPL) rupture and tenosynovitis of the third dorsal compartment is often described in association with a history of rheumatoid arthritis or in the setting of a distal radius fracture. However, the literature suggests multiple other potential factors that may lead to a seemingly spontaneous rupture. METHODS: We performed a systematic review following guidelines set forth by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. The search consisted of headings and keywords related to tendon injuries, tendinopathy, hand surgery, te…
