The principle of COPD diagnosis is based on establishing the presence of obstructive post-bronchodilator spirometry in an at-risk individual with appropriate symptoms. Most individuals diagnosed with COPD have substantial cigarette smoke exposure, more than 10 and usually 20 pack years. Patients with COPD usually have symptoms of cough and phlegm, exertional breathlessness, wheezing, recurrent winter bronchitis or a combination of these symptoms. A current or ex-smoker, aged over 35, with respiratory symptoms should undergo spirometry testing. The hallmark of COPD is the presence of airflow ob

BACKGROUND: We aimed to identify which aspects of palliative and end-of-life care are most important to people with lived experience of life-limiting conditions and can be measured using routine data currently available in the UK.
METHODS: Informed by a rapid umbrella scoping review and search of routine data available, we ran three online Patient and Public Involvement (PPI) workshops. 12-14 people from diverse backgrounds with experience of palliative and end-of-life care attended each workshop.
RESULTS: We identified nine domains of care that are important to people with lived experience

During palliative care, proper symptomatic care of terminally ill patients may require well-thought-out drug dosing. Choosing the method of drug administration is essential, as most patients want to spend the last period of their lives at home, avoiding unnecessary hospitalization. Oral medication is preferred, but as the patient approaches end-of-life care, this may become temporarily or permanently impossible for a variety of reasons. Considering the wishes of the patient and family and taking into account the previous and present symptoms, a reliable and effective drug administration route

Parenteral nutrition (PN) is a life-sustaining therapy for patients unable to meet nutrition needs via enteral routes, but its use presents significant ethical complexity. This narrative review provides clinicians with a practical, principle-based framework to navigate the ethical dilemmas inherent in PN across diverse clinical contexts. Grounded in the four pillars of medical ethics-autonomy, beneficence, nonmaleficence, and justice-the review explores decision-making challenges in vulnerable populations, long-term PN, and end-of-life care. Clinically relevant scenarios are examined, includin

INTRODUCTION: Frail older people with diabetes will need regular medications review and individualized care, especially at the final phase of life. Although guidelines are detailed on escalation of medications, there is little detail on how to de-escalate therapy when need arises. In addition, there is no description of patients' criteria in whom de-escalation should be considered.
AREA COVERED: In the guidelines, frailty is referred to as one homogeneous group. However, frailty is a metabolically heterogeneous condition with a span of variability in insulin resistance depending on proportion

Death is inevitable. Dying is a whānau/family, social, spiritual and cultural experience, not just a medical event. A lack of access to holistic palliative care services in Aotearoa New Zealand means a good death may be out of reach for some. The population in Aotearoa New Zealand is ageing and the number of deaths is set to roughly double in the next 30 years. The current stretched resources to care for people who are dying are not sufficient to meet this increase in need.1 Palliative care is a human right, with laws in many European countries ensuring comprehensive delivery.2,3 Despite palli

Hospitalized older adults with serious illness may be discharged to a skilled nursing facility (SNF) with the hope of improving strength to live independently and receive further disease-directed treatment, or because it is one of the few insurance-covered options for care that provides room and board coverage. For those that do not improve, discharge to a SNF can begin a cycle of costly care transitions between hospital, home with home health care, SNFs, and long-term care facilities, leading to fragmented care and missed opportunities for discussions about preferences for care, illness under

This paper presents the official position statement of the Japan Geriatrics Society (JGS), updating the "2012 JGS Position Statement" regarding end-of-life (EOL) care for older adults. The "2025 JGS Position Statement," which was recently published in Japanese, introduces an original definition of EOL, conceptualizing it in two stages: first, based on the individual's medical condition, and subsequently, from the perspective of the individual's life story. In this statement, EOL care refers to medical treatment and care provided at the end of life, grounded in appropriate medical judgment, res

BACKGROUND: Older adults often experience unplanned hospital admissions at the end of life, which may conflict with their wish to remain at home. Advance care planning (ACP) can help align care with patient preferences, but timely discussions and documentation are often lacking. Effective communication across healthcare settings is therefore essential.
AIM: To explore how ACP is delivered for community-dwelling older adults, focusing on intervention components, communication during care transitions and related barriers and facilitators.
DESIGN: A scoping review conducted according to Joanna

BACKGROUND: Quality indicators are frequently used to measure the quality of care at the end of life. Whether quality indicators of potential overtreatment (i.e., when the risks outweigh the benefits) at the end of life can be reliably applied to routinely collected data remains uncertain. This study aimed to identify quality indicators of overtreatment at the end of life in the published literature and to investigate their tentative prevalence among older adults dying with solid cancer.
MATERIALS AND METHODS: Retrospective cohort study of decedents including all older adults (≥65 years) who

Objectives: Mesothelioma is a rare, progressive cancer with a short trajectory from diagnosis to the end of life. This study explores the experiences of palliative and end of life care for patients with mesothelioma from the perspective of bereaved family caregivers. Methods: A qualitative, descriptive approach was adopted comprising face-to-face, semi-structured interviews with bereaved caregivers of patients with mesothelioma in the UK. An inductive, thematic analysis was conducted using the 'Framework' approach. Results: Nine bereaved caregivers participated. Four themes emerged: understand

Patients who suffer from dyspnea while dying from COVID-19 are treated with opioids and benzodiazepines. In some instances, patients may experience refractory dyspnea at the end of life. Palliative sedation can be prescribed to alleviate such patients' suffering. We describe two patients being treated for severe COVID-19 pneumonia in a tertiary hospital. Both developed intractable dyspneic crises despite high-dose opioids and benzodiazepines. This led to their requirement of palliative sedation in the general ward using subcutaneous phenobarbitone (phenobarbital). We outline clinical considera

OBJECTIVE: The aim of this study is to examine why patients are hospitalised in the last stage of life.
METHODS: Our study was conducted in a large Dutch teaching hospital. We conducted a retrospective chart review of patients aged ≥18 years who died of cancer either during hospitalisation or after discharge to receive terminal care outside the hospital. We collected data about the characteristics of these hospitalisations and indicators of advance care planning.
RESULTS: Of the 264 deceased patients, 56% had died in the hospital and 44% after hospital discharge. Of all patients, 80% had bee…

BACKGROUND: COVID-19 has led to challenges in providing effective and timely communication in healthcare. Services have been required to adapt and evolve as successful communication remains core to high-quality patient-centred care.
AIM: To describe the communication between admitted patients, their families and clinicians (medical, nursing, allied health) during end-of-life care.
METHODS: This retrospective review included all patients (n = 230) who died directly due to COVID-19 at five Melbourne hospitals between 1 January and 31 December 2020. Contacts and modality used (face to face, vid

OBJECTIVE: To answer frequently asked questions about management of end-stage pneumonia, poor nutritional intake, and dehydration in advanced dementia.
SOURCES OF INFORMATION: Ovid MEDLINE was searched for relevant articles published until February 2015. No level I studies were identified; most articles provided level III evidence. The symptom management suggestions are partially based on recent participation in a Delphi procedure to develop a guideline for optimal symptom relief for patients with pneumonia and dementia.

Last year, >28,000 people received organ transplants from >14,000 donors in the United States. Unfortunately, the wait list now tops 91,000, with the gap between recipient needs and available donor organs at around 60,000. This has motivated a host of efforts to increase organ supply, including driver's license and donor registry initiatives, educational and advertising campaigns, and "required request" and mandatory Organ Procurement Organization notification when a patient's death is imminent. Other more controversial efforts to increase the donor pool include expanded criteria for cadaveric

William Munk (1816-1898) qualified as a physician in 1837 and practiced in London for more than 60 years. Munk became well known in the medical profession during the second half of the nineteenth century for his emphasis on the importance of narcotics for the relief of pain in incurable disease and was also a recognized authority on smallpox. In addition to his medical practice Munk was the Harveian Librarian of the Royal College of Physicians for almost 40 years and, among other biographical works, single-handedly produced the Roll of the Royal College of Physicians of London 1518-1700.
