Dame Agnes Hunt: pioneer of orthopaedic nursing.

As a young surgeon in the early 1950s, I trained in various orthopaedic units. Much of the work was concerned with children and young adults suffering from two devastating diseases: tuberculosis and poliomyelitis. These were to vanish almost entirely from our hospitals soon afterwards, thanks to Bacillus Calmette-Guérin (BCG), antibiotics and the polio vaccines. However, the dedicated nursing that long term orthopaedic patients of today still require--those with congenital deformities, spinal injuries, multiple traumas and so on--owes much to the teaching of Agnes Hunt, pioneer of orthopaedic

The evaluation and treatment of the sexually assaulted patient.

There are several significant problems encountered in treating the assault victim in the Emergency Department setting. In a busy Emergency Department, the patient who has been assaulted may be prioritized behind other patients with more significant life-threatening illnesses or injuries. The delay in treatment that ensues may aggravate the emotional trauma already experienced. Additionally, the legal and technical aspects involved in the evaluation and subsequent testimony may erect a psychological barrier on the part of the physician or nurse. This article provides guidelines and handling all

Supporting healthcare workers in times of COVID-19 with eye movement desensitization and reprocessing online: A pilot study.

We report the results of a pilot study regarding the adaptation of the group eye movement desensitization and reprocessing (EMDR) protocol for the treatment online, for the management of trauma associated with the COVID-19 Pandemic in Italy. The target group were healthcare workers in a nursing home (Residenza sanitaria assistita, RSA) who decided to live and stay on site during the most acute phase of the Pandemic in order to protect the residents of the home. Scores for perceived post traumatic stress disorder (PTSD) symptoms and quality of emotional experience improved significantly followi

NEGATIVE PRESSURE WOUND THERAPY (NPWT) FOR THE MANAGEMENT OF LAPAROSTOMY WOUNDS: CASE SERIES.

Laparostomy or the open abdomen can be a lifesaving intervention in surgical emergencies for abdominal compartment syndrome, wound dehiscence, trauma and intra-abdominal sepsis. However, the open abdomen imposes a significant burden on nursing staff caring for these critically ill patients due to the large volume of exudate and fluid loss.To achieve mechanical containment of abdominal viscera and active removal of exudate, we used NPWT to manage five patients with complex intra-abdominal sepsis laparostomy wounds. It took between 12 to 28 days to achieve full granulation for secondary closure

Initial extremity war wound debridement: a multidisciplinary consensus.

A joint meeting of the Limb Trauma and Wounds Working Groups resulted in the establishment of 29 consensus recommendations for the conduct of initial extremity war wound debridement. Pre-operative, operative and post-operative phases of debridement were considered along with wound irrigation and dressings. Wounds where a different surgical approach is required, such as superficial soft tissue wounds at one end of the spectrum and complex wounds sustained in close proximity to explosions at the other, were also discussed. The recommendations represent the consensus opinion of orthopaedic, vascu

Care of the patient undergoing robotic-assisted prostatectomy.

Prostate cancer has many treatment options. In addition to open retropubic and perineal approaches to radical prostatectomy, laparoscopic robotic prostatectomy is available as a newer surgical option. Potential advantages of robotic surgery include reduced pain and trauma, less blood loss, reduced infection risk, shorter hospital stay, faster recovery, and less scarring (Intuitive Surgical, 2005). A variety of nursing care considerations involving pre-operative education and preparation, intra-operative and immediate postoperative care issues, and long-term followup must be understood to meet

Human immunodeficiency virus and invasive external otitis--a case report.

Acute invasive external otitis is an uncommon life-threatening infection of the external auditory canal (EAC), most often affecting the elderly diabetic patients. Although few reports have been made in HIV-positive/Aids patients among the caucasians. We present here a 25 year old nursing mother with a month history of fever, persistent otalgia with acutely inflammed EAC, gross facial cellulitis, mastoid abscess and facial paresis, following a minor left ear trauma with a matchstick. This unusual course of ear infection in an otherwise healthy young adult prompts a search for an immunodepressin

Post-gastrectomy bone disease undiagnosed for forty years.

Polya partial gastrectomy was performed for peptic ulcer in a previously healthy woman aged 28 years. She complained afterwards of a variety of non-specific symptoms including weakness, tiredness, debility, slowness of walking, poor appetite and constipation. Within ten years her back became bent. She was treated for intercurrent hypertension and epilepsy. Bone fractures on low-impact trauma occurred in her fifties. At 57 years, she was unable to care for herself and had to be admitted to a nursing home. She could still walk slowly with the aid of a stick. For three months at the age of 65 yea

Clinical Utility of Routine Chest X-Rays During the Initial Stabilization of Trauma Patients.

OBJECTIVES: The Advanced Trauma Life Support (ATLS) course encourages the use of chest x-ray (CXR) to identify injuries that may change clinical management during the initial stage of trauma resuscitations. Several studies have failed to show benefit for the routine use of CXR without a clinical indication, however. We sought to validate these findings by determining the incidence of clinically significant findings discovered on a portable single-view CXR during the initial stabilization of trauma patients at a Level 1 trauma center. METHODS: Using our electronic medical record system, we sea

Petechial rash in children: a clinical dilemma.

Children with a petechial rash commonly present to emergency departments. The rash can be associated with serious illnesses, such as invasive meningococcal disease (IMD), but is usually associated with less severe conditions. This article discusses the common and important causes of petechial rash, including IMD, viral illnesses, trauma, Henoch-Schönlein purpura and idiopathic thrombocytopenic purpura. It also analyses the National Institute for Health and Care Excellence (NICE) ( 2014 ) pathway for management of petechial rash in children and examines treatment of the various causes. The arti

Comparative prevalence of anogenital injury following sexual assault in women who have had recent consensual sexual contact.

OBJECTIVE: The purpose of this study was to compare the frequency and types of anogenital trauma in rape victims as a function of the time interval between the assault and recent (72 h) consensual sexual intercourse. METHODS: This retrospective cohort trial evaluated consecutive female patients, age 13 years or older, presenting to a community-based nurse examiner clinic (NEC) during a 5-year study period. The NEC facility is staffed by forensic nurses trained to perform medical-legal examinations using colposcopy with nuclear staining and digital imaging. Eligible patients were classified in

The Life Pattern of Incarcerated Women: The Complex and Interwoven Lives of Trauma, Mental Illness, and Substance Abuse.

PROBLEM: The fastest growing population in the U.S. prisons consists of women whose rate rose nearly twice that of men (Sipes, 2012). Incarcerated women are oftentimes victims of abuse and have high reported rates of mental illness and substance abuse, yet their life pattern is not well understood. PURPOSE: The purpose of the study was to develop the life pattern of incarcerated women to gain a better understanding of the life situations that have led to their incarceration and gain a better understanding of their needs during their imprisonment. METHOD: In-depth interviews were conducted wi

Child Maltreatment Red Flags: Two Cases of Bruising in Premobile Infants.

Child maltreatment is a serious public health concern in the United States. Young infants and children younger than 3 years are at the highest risk of being abused and can experience both acute injuries and long-term developmental, behavioral, and mental health problems. Health care providers are mandated reporters of suspected abuse but may misdiagnose potentially abusive injuries because of lack of knowledge in recognizing maltreatment. Premobile infants rarely have bruising or intraoral injuries without a reported accident or underlying systemic disease and should raise concern for abuse. I

Pediatric Firearm Survivors as a Medically Complex Population.

Firearm injury is now the leading cause of death among children and adolescents in the United States, yet far more children survive gunshot wounds and encounter substantial long-term consequences. This article argues that pediatric firearm survivors constitute a population of children with medical complexity, a group defined by severe chronic health conditions, significant functional limitations, substantial service needs, and high health care utilization. Survivors often experience multisystem trauma, neurological impairment, chronic pain, organ dysfunction, and enduring functional deficits r

Utility of Repeat Head Computed Tomography for Intracranial Hemorrhage After Trauma and Importance of Direct Patient Care.

At many institutions, it is common practice for trauma patients with traumatic intracranial hemorrhage (ICH) to receive routine repeat head computed tomographic (CT) scans after the initial CT scan, regardless of symptoms, to evaluate progression of the injury. The purpose of this study was to assess quantifiable risk factors (age, anticoagulation, gender) that could place patients at greater risk for progression of injury, thus requiring surgical intervention (craniotomy, craniectomy) for which serial CT scanning would be useful. From January 2014 to June 2015, a total of 211 patients present

Prioritizing delirium risk factors in nursing: a cross-sectional study using the analytic hierarchy process.

BACKGROUND: Delirium is a complex neuropsychiatric syndrome characterized by acute and fluctuating disturbances in attention, cognition, and consciousness. Despite its significant impact on patient outcomes, a gap persists between clinical practice and evidence-based approaches for delirium prevention and management. This study aims to systematically explore nurses' perceptions of delirium risk factors, identify which factors they prioritize, and examine the reasoning behind their prioritization using the Analytic Hierarchy Process (AHP). By addressing these objectives, this study identified p

Retrospective evaluation of the PRE-DELIRIC score in a Chinese mixed ICU: implications for nursing practice.

Delirium is a common complication in intensive care units (ICU). The PRE-DELIRIC model has shown promise in early delirium prediction, but its performance in Chinese ICU settings remains unclear. The objective of this study was to validate the PRE-DELIRIC model in a Chinese mixed medical-surgical ICU and evaluate its utility in guiding nursing interventions for delirium prevention. In this single-center retrospective cohort study, adult patients admitted to the ICU between January 2023 and October 2024 were included. The PRE-DELIRIC score was calculated within 24 h of admission. Delirium was a
