Clostridium septicum, an anaerobic spore-forming Gram-positive bacillus, is a rare opportunistic pathogen that causes severe infections with rapid progression and high mortality. Trauma with unmanaged contaminated wounds is a major risk factor, and atypical presentations such as tetanus-like neuromuscular symptoms often lead to misdiagnosis. Herein, we report a 72-year-old male farmer with C. septicum and Enterobacter cloacae co-infection following a traumatic toe laceration, presenting with trismus, limited mouth opening, and right toe gangrene. This case highlights the diagnostic challenges …

Giant cell arteritis (GCA), also known as temporal arteritis, is a chronic vasculitis affecting large- and medium-sized arteries in patients over 50 years old. Untreated GCA can cause serious morbidity like irreversible vision loss, aneurysms, or stroke. We present a case of a patient who developed an acute headache and trismus within hours of undergoing a root canal treatment. Because GCA typically presents more gradually, the sudden onset of symptoms raised concern for dental-related complications or infection, contributing to a delay in considering GCA. This case highlights the importance o…

Ludwig's angina is a severe diffuse cellulitis that presents an acute onset and spreads rapidly and bilaterally. It can affect the submandibular, sublingual or submental spaces resulting in a state of emergency. Early diagnosis and urgent management could be a life-saving procedure. We report a case of wide spread sialadenitis infection extending to the neck with trismus and elevation of the floor of the mouth that caused an obstruction of the airway and resulted in an inspiratory dyspnea and a stridor. The patient was directed to maintain the airway by elective tracheostomy. An appropriate us…

Although peritonsillar abscesses (PTAs) are a common presentation in pediatric patients, there are very few reports on a pediatric patient with both a PTA and uvular hydrops. Our patient presented to the emergency room after being unsuccessfully treated for pharyngitis, with odynophagia, dyspnea, muffled voice, drooling, and trismus. On physical examination, we observed a PTA as well as an edematous and erythematous uvula. Following the standard of care, the patient underwent a needle aspiration in the emergency department and subsequently was admitted overnight for observation. The patient ha…

Head and neck squamous cell carcinoma is one of the most common neoplasms. Radiotherapy (RT) plays an essential role in the management of such cases. Despite advances in the technique, hyposalivation, xerostomia, dysphagia, trismus, radiation caries, and osteoradionecrosis remain significant late complications of RT. The aim of this narrative review was to summarize and update the main findings related to late side effects of radiotherapy in survivors of head and neck cancer (HNC). Such effects limit the ability to speak, ingest food and medications, and breathe and also exert a negative impac…

Peritonsillar abscess is the most common complication of acute bacterial tonsillitis, defined as the presence of purulent exudate between the peritonsillar capsule and the superior constrictor muscle of the pharynx. Usually, its presentation is unilateral; its bilateral involvement is rare. It is more common in adults, being uncommon in children. Clinically, it presents odynophagia, trismus, sialorrhea, fever, guttural voice, symptoms that it shares with the unilateral abscess, evidencing in the physical examination bulging of both anterior pillars, edema of the uvula and soft palate, but with…

In this report, we describe a case of brain abscess due to odontogenic infection. A 53-year-old female who had been suffering from headache and trismus for two weeks visited the Department of Oral and Maxillofacial Surgery at the Sun Dental Hospital (Daejeon, Korea). Even after several routine tests, we still could not make a diagnosis. However, after the combined multidisciplinary efforts of oral surgeons and neurosurgeons, the patient was treated for odontogenic infection and made an uneventful recovery. Therefore, patients with infections in the head and neck region showing symptoms such as…

A case of craniosynostosis with joint contractures, ear deformity, cleft palate, scoliosis, and many other features is presented. The patient's skull was round with craniosynostosis of the coronal suture. The fingers were slender and long; the finger joints were contracted. Trismus was also present. Contracture of the temporomandibular joint was suspected. The helices were flat, antihelices were minimal, and the ears protruded. Furthermore, the facial and cranial features of this patient included mild hypertelorism, ocular proptosis, short pons nasi, flat radix nasi, mild retrognathia, and sma…

Limited mouth opening (LMO) in oral submucous fibrosis (OSF) has been attributed to both the submucosal and muscle fibrosis (MF). While reflectory trismus was proposed before as an auxiliary mechanism by another group, the stretch-mediated muscle damage (MSD), histopathological changes in blood vessels (such as endothelial dysfunction, endothelial hypertrophy, and endarteritis obliterans), and upregulated anaerobic isoforms of lactate dehydrogenase (LDH) have been proposed by us as complementary events leading to MF. Additionally, the amount of hypoxia-mediated upregulation of anaerobic isofor…

Understanding the mechanisms by which clinical signs and symptoms are produced is a prerequisite to the correct appreciation of radiographic features. Radiographs of facial trauma are no different in this respect. This paper describes the specific clinical findings associated with each displaced bony fragment of the zygomatic complex fracture. Limitation of jaw movement and flattening of the cheek are produced by depressed fractures of the temporal process or zygomatic arch; unilateral epistaxis is a result of fractures of the zygomatic process of the maxilla or the floor of the orbit; paresth

Septic arthritis of the temporomandibular joint is a rare pathology. There is no consensus in the literature regarding etiopathogenesis and treatment. Pain, swelling, trismus, and mandibular deviation are common. If diagnosis is delayed and appropriate treatment is not given, complications such as osteomyelitis, ankylosis, and systemic spread may develop. This article presents the case of an 85-year-old male patient with septic arthritis of the temporomandibular joint, including clinical, laboratory, and radiological features, accompanied by relevant literature. Since treatment was started ear…

Lemierre's syndrome, also called septic thrombophlebitis of the internal jugular vein, necrobacillosis or postanginal sepsis, is an infection that begins in the oropharyngeal space, is complicated by septic thrombophlebitis of the internal jugular vein and infectious metastases. The rapid progression to serious clinical conditions that compromise the patient's life and its low frequency justify the disclosure of clinical cases. We present the case of a 27-year-old woman who developed facial edema and trismus 48 hours after completing treatment with phenoxymethylpenicillin for an odontogenic in…

Neck mass is a common condition in primary care. The most common affected area is the cervical lymph node. The neck region is also prone infection as structurally the nodes and spaces are in close contact with the upper respiratory tract and the alimentary tract. Oral cavity is one of the most common route for harbouring infection. Poor oral hygeine and periapical dental problems are the main causes. Thus, it is important to perform a complete oral cavity examination even when trismus is present. Besides dental caries, floor of the mouth should be inspected for oedema that may impose threat to…

BACKGROUND: Normal saline remains the most commonly used irrigant during third molar surgery despite evidence that balanced crystalloids may reduce inflammation and acidosis in other surgical fields. This study compared the effect of intraoperative irrigation with lactated Ringer’s solution versus normal saline on postoperative pain, trismus, and facial swelling after bilateral impacted mandibular third molar removal. METHODS: A prospective, double-blind, split-mouth randomized clinical trial was conducted in 28 healthy patients. Each patient received 10 mL of normal saline on one side and 10 …

Primary osteomyelitis of the mandibular condyle is exceptionally rare. The case of a 5-month-old infant with primary osteomyelitis of the condyle, who was followed up over 6 years and avoided the devastating sequelae of temporomandibular joint ankylosis and mandibular growth impairment, is reported here. The patient presented with acute swelling, fever, and trismus, leading to the diagnosis of acute primary osteomyelitis through clinical and radiological findings. This was treated successfully with a minor surgical intervention and antibiotics. The reported case, in combination with a literatu…
