Diagnostic Accuracy with Total Adenosine Deaminase as a Biomarker for Discriminating Pleural Transudates and Exudates in a Population-Based Cohort Study.

BACKGROUND: An initial step in the evaluation of patients with pleural effusion syndrome (PES) is to determine whether the pleural fluid is a transudate or an exudate. OBJECTIVES: To investigate total adenosine deaminase (ADA) as a biomarker to classify pleural transudates and exudates. METHODS: An assay of total ADA in pleural fluids (P-ADA) was observed using a commercial kit in a population-based cohort study. RESULTS: 157 pleural fluid samples were collected from untreated individuals with PES due to several causes. The cause most prevalent in transudate samples (21%, n = 33/157) was co

Diagnostic thoracoscopy.

During a 5-year period, diagnostic thoracoscopy was attempted in 566 patients and successfully performed in 556. No deaths due to thoracoscopy occurred and complications were few. In patients with malignant disease of the pleura, positive biopsies were obtained in 95 of 137 patients (69%). A positive culture for tubercle bacilli was obtained from pleural biopsy in 27 of 36 patients (75%) with bacteriologically confirmed pleural tuberculosis. Thoracoscopy improved the diagnostic accuracy in these conditions. In patients with benign tumours as well as anatomical abnormalities thoracoscopy was di

Vulvar Tuberculosis-A Rare Manifestation of the Disease.

Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. According to data from the World Health Organization, this disease remains one of the leading causes of death worldwide. Although it most commonly affects the lungs, tuberculosis can compromise any organ. The present study reports a rare case of vulvar tuberculosis in a postmenopausal woman with a history of asymptomatic pulmonary and pleural tuberculosis, with no prior documented contact with the bacillus. Diagnosis was based on vulvar lesion biopsies, with histological findings suggestive of infection and isolation o

Progression of clinical tuberculosis is associated with a Th2 immune response signature in combination with elevated levels of SOCS3.

In this study, we explored the local cytokine/chemokine profiles in patients with active pulmonary or pleural tuberculosis (TB) using multiplex protein analysis of bronchoalveolar lavage and pleural fluid samples. Despite increased pro-inflammation compared to the uninfected controls; there was no up-regulation of IFN-γ or the T cell chemoattractant CCL5 in the lung of patients with pulmonary TB. Instead, elevated levels of IL-4 and CCL4 were associated with high mycobacteria-specific IgG titres as well as SOCS3 (suppressors of cytokine signaling) mRNA and progression of moderate-to-severe dis

Coinfection With Nocardia exalbida and Mycobacterium tuberculosis Following Steroid Pulse Therapy: A Case Report and Literature Review.

We report a case of pulmonary nocardiosis and pulmonary tuberculosis following steroid pulse therapy for presumed myelitis. A 74-year-old man with transthyretin amyloid cardiomyopathy, chronic heart failure, and type 2 diabetes mellitus received two courses of steroid pulse therapy for suspected inflammatory myelitis. One month later, he developed progressive dyspnea and was admitted to our hospital. Chest computed tomography revealed multifocal right-sided infiltrates and pleural effusion. Although empirical piperacillin/tazobactam was initiated for presumed bacterial pneumonia, sputum Gram a

[Malignant mesothelioma of the pericardium. An anatomo-clinical study of 10 cases].

Ten pericardial mesotheliomas (8 of which had associated unilateral pleural involvement) were observed over a 22 year period in subjects over 50 years of age. The diagnosis was only confirmed several months after the presenting symptoms (shortness of breath, chest pain), usually by histological studies of pericardial biopsies performed during construction of a pleuro-pericardial window because of tamponade or of pleural biopsy in cases of pleuro-pericardial disease. There is no specific diagnostic feature and even modern imaging methods are unable to distinguish mesothelioma from pericardial t

Syringo-pleural shunt: a rare cause of recurrent pleural effusion.

case of a patient with reported spinal tuberculosis (TB) that developed syringomyelia, a rare sequelae of the disease is presented in this report. He underwent syringo-pleural shunting for syringomyelia. After 15 years, the patient presented with recurrent pleural effusion. Diagnostic thoracentesis revealed fluid of transudative nature. Pleural fluid was positive for beta-2 transferrin. Pleural biopsy was negative for any malignancy or TB. Computed tomography scan focusing on the shunt showed that it was undisplaced. Video-assisted thoracoscopic exploration showed the distal tip of the shunt a

Polymerase chain reaction for Mycobacterium tuberculosis complex DNA. Use on negative archival Ziehl-Neelsen cytologic samples.

OBJECTIVE: To evaluate the usefulness of a nested polymerase chain reaction (PCR) for Mycobacterium tuberculosis complex on routinely stained cytologic samples from patients with extrapulmonary tuberculosis. STUDY DESIGN: Nested PCR for the detection of a fragment of the IS6110 insertion sequence of M tuberculosis complex was applied to Ziehl-Neelsen-negative archival cytologic slides of serous effusions (pleural [n = 7], peritoneal [n = 1] and pericardial [n = 1]) and a lymph node fine needle aspirate (n = 1) from nine human immunodeficiency virus (HIV)-positive patients with autopsy-proven

Cryptococcal pleural infection in a recurrent pleural effusion: a case report.

Cryptococcal pleural infection is rare with about 50 cases reported. It tends to occur in immunocompromised individuals. We describe a 38-year-old male who presented with a lymphocytic exudative right pleural effusion and a raised pleural fluid adenosine deaminase (ADA) level. He was initially treated for pleural tuberculosis, but presented again with worsening pleural effusion 6 weeks later. A thoracoscopic pleural biopsy revealed chronic nodular granulomatous pleuritis with cryptococcal organisms present. The repeat pleural fluid culture was positive for Cryptococcus neoformans. He was start

[A CASE OF SUBPHRENIC ABSCESS WITH PARADOXICAL RESPONSE CAUSED BY MYCOBACTERIUM TUBERCULOSIS].

A 40-year-old woman was admitted to our hos- pital with cough and sputum production. A chest computed tomography (CT) scan revealed a diffuse nodular shadow in the upper lung. The patient was diagnosed with pulmonary tuberculosis, based on a positive T-SPOT®.TB test result of peripheral blood and a positive polymerase chain reaction (PCR) test result for Mycobacterium tuberculosis in gastric aspirates. M.tuberculosis was subsequently isolated from the gastric aspirate specimen. After 2 months of treatment with antituberculous medication, the patient developed a low grade fever and left-sided c

Demonstration of intrapleural immunoglobulin synthesis by agarose gel isoelectric focusing. A new specific method for the diagnosis of pleural effusions of inflammatory origin?

In order to demonstrate an intrapleural immunoglobulin (Ig) synthesis, pleural effusions and serum from 14 patients with pleural effusions of various causes (inflammatory condition, malignancies, primary lung cancers, pleural metastasis, and congestive heart failure) were investigated by agarose gel isoelectric focusing (AIEF), antiserum immunofixation with monospecific antiserum against IgG and IgA, and quantitative estimation of IgG and IgA. Intrapleural immunoglobulin synthesis was assumed to occur in patients with oligoclonal Ig limited to the pleural compartment or in patients with a rais

[Usefulness of thoracoscopy under local anesthesia in the diagnosis of tuberculous pleurisy occurring during infliximab administration: a case report].

We report the case of a 71-year-old man with rheumatoid arthritis. Infliximab administration was started as treatment for rheumatoid arthritis in January 2009. As he showed a positive result for the tuberculin test, he was treated with isoniazid for 9 months. He was subsequently referred to our department in October 2011 with a right-sided pleural effusion. When thoracoscopy was performed under local anesthesia, white and red protruding lesions of various sizes were observed in the pleural cavity. A biopsy revealed fibrous granulation tissue, and tissue culture and all sensitivity tests were p

The role of thoracoscopy in the evaluation and management of pleural effusions.

Diagnostic thoracoscopy is indicated in every patient where the usual investigations (including biochemistry, cytology, bacteriology, occasionally needle biopsy of the pleura) do not achieve a precise diagnosis. The percentage of so-called "idiopathic" effusions, amounting to approximately 20% in many published series, can be reduced to 4% after thoracoscopy. The sensibility of thoracoscopic biopsy reaches 93-97% of malignant or tuberculous pleural effusions. The procedure requires a short hospitalization of about 36 hr, and complications are rare. Therapeutic thoracoscopy is frequently perfor

Mycoplasma pneumoniae infection mimicking tuberculous pleurisy in a young woman: a case report and literature review.

A 30-year-old woman was admitted to a different hospital with a 2-day history of fever, cough, and expectoration. She had a history of left pulmonary tuberculosis 8 years previously. Chest computed tomography showed an infiltrate in the inferior lobe of the left lung and spot-like calcifications in the anterior lobe of the upper left lobe and lower lobe of the left lung. After antibacterial treatment, the patient’s condition deteriorated and she developed significant pleural effusion on the left side. The pleural effusion assay showed a lymphocyte-predominant exudate with a significantly incre

The Global Burden of Pleural Diseases.

Pleural diseases include a spectrum of disorders broadly categorized into pneumothorax and pleural effusion. They often cause pain, breathlessness, cough, and reduced quality of life. The global burden of diseases reflects regional differences in conditions and exposures associated with pleural disease, such as smoking, pneumonia, tuberculosis, asbestos, cancer, and organ failure. Disease burden in high-income countries is overrepresented given the availability of data and disease burden in lower-income countries is likely underestimated. In the United States, in 2016, there were 42,215 treat-

[Surgical treatment of chest wall tuberculosis].

Of 5000 patients with skeleton tuberculosis, 85 also had chest tuberculosis (= 1.7%). Chest tuberculosis in most cases begins with local pleural tuberculosis originating from a former moist pleurisy. The 5th rib is preferably involved, occasionally several ribs simultaneously. The gradual development of the abscess of the thoracic wall is mostly not recognized. Abscess incision only does not lead to recovery. Fistulas will be the result. Partial resection of the involved rib together with local decortication of the pleura is according to our experience the optimal therapy to obtain good result
